    I had a high school friend on this flight. She was about 20 and returning to the U of Colorado after a weekend with her family. Her father was a pilot for United and she was their only daughter. Very sad.

      I remember her: Bonnie Cottle, Langley HS Class of 1967. She was one of my classmates, and the first one (as far as I know) to have an untimely death. Rest in peace, Bonnie.

      Bonnie Cottle? She was my brother's girlfriend and she was going to head back to McLean after this flight to stay with us and visit him. She was 19 going to University of Colorado.

    I was working in ual's LAX reservations office that night. My heart is in my throat now. We all felt the loss of lives personally.

    My father in law's mom died in this crash.... I feel so bad for him - he was a little boy. I hope she is resting in peace, and knows how great her son turned out - he's an amazing dad, grandpa, and father in law. He truly, truly, is.

    I love the music in this video - perfect for the atmospheric effect - I also love flight videos channels that don’t do the same old accidents over and over again and this is one I have never seen before - how very sad may they all RIP 🙏

    It has been difficult to find much written about this flight. I'm curious on what drew you to depict this event in only your 2nd video that you created (according to someone else)?

      Hi Charles! In fact, everything is very simple. I try to choose those accidents, describing which, in my opinion, will let me add something new. In the case of the UAL 266, you rightly pointed out that it's very difficult to find a lot of information about this event. Although I think, it's a very interesting story with a series of fatal randomnesses, coincidences, and a very tragic ending.

    My older brothers highschool girlfriend and fiance' Bonnie Jean Cottle, died on this flight. Originally we were told that the plan got hit by lightning and inverted crashing into the bay. Her name was Bonnie Cottle, her dad was a pilot for United Airlines and they flew for free. He was told by the tower that the plane got hit by lightning and flipped over during flight and dived into the bay.
    The bay is full of sharks any survivors were killed by sharks.

    Why did they not start the APU when they were down to one generator? Was engine 1 still running? The narration only states that the generator was switched off, not the entire engine (so they may have been able to switch it on again before generator 2 failed).. How did engine 1 have more hours than the airframe? Is Boeing shipping planes with used engines?

      I believe that in a stressful situation, they simply didn't have enough time to do anything.
      Engines # 2 and # 3 continued to work. Engine # 1 was turned off by the crew due to a fire report. According to the regulations, this engine can no longer be turned on.
      And yes, there really was such a thing that one of the engines was replaced with an older one!

    An amazing comparison of flight technology of 50 years ago, compared to today! Not just the aircraft, but also the flight investigators. They did not have the advanced systems that would have prevented this plane from ever taking off... plus, based on today's investigations, it seems incredible that they could not determine what caused the electrical failures. I guess this will always remain a true mystery. Good video!

    There are some errors in the depictions shown in this presentation. Firstly, the FE electrical panel is not as it would have been for this departure. While the #3 generator was inop and disconnected per the MEL, the #3 BTB would have been closed and it's associated light extinguished. Both fuel pumps for engine #3 would have been ON (light out). We cannot see in this video if one AC Pack was off or if the Galley Power switch was off as it should be. At the time the #1 Engine Fire Handle was pulled, you should have seen only the bottom two lights for Gen#1 and Gen #3 on. All Tie Bus lights should have been out.
    According to discussion with a senior Boeing instructor regarding this accident, it was assumed that the FE turned off the Battery Switch instead of the Galley Power switch. At the time, both were located adjacent to each other and were not guarded. With the Galley Power on, and the Battery OFF, it's likely that the single operating #2 Generator would overload and trip and Standby power would not be available.
    UAL procedure to reset the field of any generator, only #2 available, would have restored power but it would overload again with Galley power ON. Two resets were evident as were two trips.

Although the report didn’t name the cause of the incident, the FAA regulated that the Flight deck layout should be such that inadvertent or incorrect actions are prevented by positive design details (guarded switches, non-proximity of commonly used switches).

      @@flywithpenguin Back in 1987, I was taking a technical course on the 727 instructed by a former Boeing instructor. He detailed this accident at length to illustrate the electrical system design and modifications following this accident.

      Maybe it wasn’t galley power that overloaded generator #2. The checklist called for it to be switched off just before takeoff, so the FE would have done this. The culprit instead may have been a running air conditioning pack cooling fan. If a pack is switched off (as the FE would have done procedurally, one just before takeoff, the other at the onset of the emergency) but the pack valve does not fully close (a rare occurrence, but one which I have personally witnessed) the fan will continue to run with its high electrical load until the flaps are fully retracted. This never quite happened. With his attention focused on dealing with the fire warning procedure and loss of all electrical power, the FO never moved the flap selector the last critical notch from 2 degrees to up. They hit the water still in that configuration, although all the high lift devices had likely been torn off already by the terrific overspeed, probably creating roll asymmetry in the process.
      When #2 generator first tripped, the only cockpit light would have been the emergency exit light over the door, powered by its own internal battery. The FE probably thought of two things he needed to do immediately: move ESSENTIAL POWER to STANDBY to power the captain's flight instruments from the main battery, and reduce the electrical load so he could get main bus power back. Perhaps he tried the second one first, and in the dim light accidentally turned off the unguarded battery switch thinking it was galley power, and that he’d somehow missed it on the before takeoff checklist. With the battery switch off, the captain’s flight instruments could no longer connect to the battery, although the FE would have certainly tried to do that after being unable to reset the generator.
      Whatever remained of the FE’s panel was never found, so switch positions could not be determined. At any rate some may have been moved a few times while the FE was trying to sort things out. To validate my pack fan theory it would be necessary to recover the pack valves and find that one was not fully closed. Impact forces might have jarred a stuck valve closed anyway, making it appear blameless. The pack fans would not have been running at impact, as they would have been deactivated by the very power failure one of them had caused.
      So, they had the #3 generator already inoperative; #1 engine and its generator shut down due to a false fire warning just before the flaps would have been retracted completely and de-powered any pack fan that was still running due to a balky pack valve; two adjacent, unguarded switches with very different functions in an unfamiliar (QC) configuration; nighttime, in the clouds and rain with no working instruments to keep the blue side up. Just too many problems too fast.

    Looking forward to more of these highly well done videos. Maybe one day MSFS 2020 will allow user scenery changes like they did with FS2002 and 2004, so you can go full retro in the airport scenery. I had to modify FS2004 a bit with LAX, having removed TBIT and Terminal 1, parking, etc so it would look like 1974, since I had the AI running in retro airline schedules and livery. Couldn't do more than that for the most part as I was not a real designer. Tedious, but fun.

    Flying with 2 operational generators was obviously ill advised. That plane had 3 for a reason.

      Johnny, they apparently had an approved MEL to authorize going with 2. Airline pilots fly airplanes in accordance with MEL’s. If their personal minimums were more restrictive than the MEL then they would quickly be unemployed.

      If it was "ill advised" as you say to fly with only two generators then Boeing wouldn't allow it which means that the FAA wouldn't allow it which means that the Air Carrier wouldn't allow it. We have a "Minimum Equipment List" or (MEL) because the reality is that we can safely fly with things that are broken. The MEL provides the required actions for deactivation of the item as well as any special procedures or performance limitations that go along with that item being "deferred". Many items don't have anything special outside of a circuit breaker being pulled and a sticker being placed on the instrument panel because there is nothing special about it being broken. What this video doesn't explain about the accident is the theory about what happened which another commenter already explained. The video maker most likely didn't include the theory because it has not been proven. If you fly on an Airliner a lot I would almost guarantee that you have flown on one with one or more things deferred per the MEL.

      @@CaptainKidd707 - As I stated in my post, the plane has 3 generators - for a reason! If 1 or even 2 were all that was necessary, then Boeing would have built it that way. The point is don't take unnecessary chances, especially while flying a plane. Oh, BTW, the plane crashed!!

      That's your opinion, unfounded as it may be, and obviously you know as much about aviation as a welder would know about brain surgery. The plane has 3 generators but does not need all three working at once. Boeing puts three on because there are three engines. There was no risk involved. The working theory on the crash is the flight engineer mishandled the emergency.

    My mother always talked about this and the other Scandinavian one that crashed nearby 3 days earlier. I was 2 when my mother and twin brother all flew to central Mexico earlier that year around December or January. My dad said he saw a lot of ambulances and fire engines that night as he was traveling along a highway or freeway to LAX to pick us up. He later heard about sharks eating the bodies and thought we were on that flight. It was very frightening for him and others because of the previous plane crash that also occurred in Santa Monica Bay. 30 survived that one but all perished in this one.
