      Im with u. I had my stroke at 43. It is my 4yr. I remember that time when i used all my strength just to move 1mm of my wrist. Omg. Now i almost got to 90 degree. I just need to flex my fingers as well now 😁😁 Let’s not give up 👍

    Dr. Tobias I have been following your vids for a few months, currently working toward ditching the cane. Survived massive hemmoraragic stroke and Cascading TIA in June last year.Combining the work my PT does with me and adding your vids on my own, I'm making strong lower body progress. I will run and practice martial arts again. I'm excited to see you are talking about the shoulder and arm. My short term goal is to wash dishes, long term is to get back to work (Forestryc Contractor. Using chainsaws, power tools 8hrs a day again is my aim. I have full range of motion in shoulder, looking forward to reviewing and incorporating your arm recovery methods. Bless you, and thank you.

    Thank you, thank you for showing the more advanced movement. I’ve “been with you” since six months post stroke AND going to formal therapy until mid March 2020. You have been and are my progress life saver! Sadly my insurance will not allow me to go back to formal therapy. You are most definitely my lifeline💖

      @@rehabhqofficial GUESS WHAT! I get to go back to formal therapy. Moving from a “regular” PT to a “neuro” PT, labeled as a fall risk and I don’t care😂 Because of my funny weird rotation walking form I am aggravating my affected knee. I had a knee surgery scheduled that got canceled because of the stroke. My dr was able to work it out that the pre existing knee issue is exasperated by compensatory walking form so insurance is allowing me back to therapy. At my evaluation I told them all about you, Dr. Tara. They are super impressed with my knowledge and understanding of my needs because of your teaching 💖💖💖

    Thank you Tara for sharing in your your lecture about how to regain shoulder movement and easing off on hand/ lower arm spasticity Exactly my problem a two folded nature…! I will definitely add to my rehab routine your recommended stretches/ exercises…!
    It seems that my frozen shoulder got better and so did my same arm - hand spasticity; but then-
    again in the last month or two it seems to have slided back into the same or even somewhat worse condition…why? And what can I do…?

    Thank you for being positive. I’ve just found your videoa. I had my stroke in 2017, i lost mobility on right side. I wish people who work in rehab are all encouraging like you. Noone told me that i could move my hand again. One person even told me to accept the new me. I was mad then, i said, “how could you say that when we just started?” Im walking almost normal now, im learning to walk fast. If i try to walk fast, there spasticity that makes my feet feel like glued to the floor, i still dont know how to get rid off it. I say my shoulder is at 90%. Still fighting spasticity in my biceps, forearm, wrist, hand. I will not give up. The info you shared help a lot. I will do your exercises. Thank u. I dont care if people say it is impossible, but I see myself playing piano again one day. Thank uu for sharing. Please post more hand exercises. ❤️❤️❤️

    Excellent explanation. Losing the ability to use your arm and hand is the most frustrating thing ever but hard work certainly pays off !

      RW. You are not alone and I feel for you and so many others who can't move their arm. There are so many things we take for granted....until we can't. But hang in there! "Frustration" can be a form of "healthy energy" to fuel your rehab!

    Hi, I have been following you for a little while. I’m based in London. I had a Jean injury via a road accident in my 20s, further emphasised by a stroke in my 40s. No one gave me your type of exercises, I mean I have even heard of an elbow immobiliser? I would need one because my muscles would make it impossible to even attempt what you show! Thank you and I can’t wait to start!!!

    Thanks for the very detailed explanation, so many important details, it only a top specialist notices and so selflessly conveys to us, it’s amazing. The problem with me is the neck muscles because they don’t allow the shoulder blade to move. Also stretching the arm backwards on the abdomen because standing we unconsciously lean forward. How hard should the blades be squeezed? This three-part series is the best thing on the Internet.

    Very interesting video and I will be try these exercises. I had 2 strokes whilst in iCU following a motorcycle accident where I broke my tibia, fibula, scapula and ribs 1 to 9, since then 2.5 years now I’ve been treated for a frozen shoulder and still have lots of pain and little range of motion. My question is that nobody has ever asked if my shoulder problems could be due to stroke.

    Something else I use, that most people probably have already instead of a dowel rod is my cane Disclaimer: At least for me a tight grip can really kick in spastic patterns so something bigger than a cane may be best for some

    Thanks a lot. Great video. Can you show some more exercises to strengthen a weak serratus anterior as it is critical for the upwards rotation of the shoulder blade, isnt'it?

      Hi Marina! upward rotation is complicated. It depends on the position of the rest of the arm. In most cases, it is not a "weak serratus" but it is the ability of the serratus to "play nice" with other muscles. :) In either case, upward rotation IS critical to overhead motion.     Hi Tara. I appeciate a lot that you give so many background information in your videos! I believe it is very helpful not only to know how to do an exercise but also to understand it. I have this question: why is it important to do the thoracic spine rotation with the arms in that position? And not e.g. with the arms hanging down or crossed-arm in front of the body?

      Hi Marina! That is an excellent question. It is the position of the shoulders that I like about this stretch. It gives a deeper stretch to the pectoralis muscles which are usually extremely shortened after a stroke. :) I hope that helps!

    Tara, where do we find that looped Theraband? Also, I tried putting on the elbow immobilizer, yet it was anything but immobile with the Velcro latching onto itself against my skin & thus, I couldn't tighten it very much & returned it. Are there instructions to be found for putting it on properly?

    Dr T., is the elbow immobilizer preferable over the air splint or vice versa?

      Excellent question! I prefer the air splint however, I prefer the long airsplint and therefore you can't hold onto anything (because it goes past the hand). The air splint might also be a tad heavier. However, for spasticity, the even pressure on the arm (that the air splint provides) is a better set up. I hope that makes sense!

    Any tips and hhow to fix winged scapula and hemiplegic elbow ( elbow subluxation i feel, that i might have to movements but due to those conditions that doesn’t allow me to execute the movements like ife starts well but it gets stuck in the middle of the range .i. e reaching forward and over the head

    Dr T, one of the biggest challenges in therapy is replicating what the props did in the clinic at home. What was that box w/ wheels that you used both on & beside the mat-table?

      Hi Aaron! Yes, have the right toolkit of "props" is super helpful. This is a garden stool that I think I got at a sears outlet in my area. I tried to find one that is similar in price on amazon. Link is in the description of the video

    Because when we have a stroke, movement is very different to some trying to emulate the movements compared to a stoke survivor. Can we please see the movement/exercises with a stroke survivor client?

    Hello I am a hemiplegic patient. My problem is my upper limb and lower limb tension increases in awkward situation like barber shaving my beard or some on comes to close to me or I am in awkward situation. What is this syndrome and how to manage it (sorry for same comment on multiple videos)

    If the brain isn't telling the muscle to fire, then how can it improve ? My triceps are atrophied ( left side ), do I need an isometric work out ?

Check out the playlist titled "0-6 months". I am not 100% on board with saying "the brain isn't telling the muscles to fire". I think a deeper understanding of what is actually going on between your brain and your muscles might help you in your recovery. I hope that helps!

      Thanks for watching and for the question! Generally speaking, yes. Same type of movement retraining. As far as the adhesive capsulitis, lots of scapula joint mobs and glenohumeral joint mobs with mullligan techniques. emphasis on posterior capsule. (You can stretch posterior capsule in prone on elbows as well. If he/she can't get prone, I would start working on that.) I hope that helps

    Hello ma’am. My son is 4 year old and he suffered Pediatric stroke at the time /before /after birth. He has very mild hemiplegic spasticity in his right hand and leg. He don’t use much of his right hand. Always holds his right hang high and tight when ever is is about to do something. Has a gait problem. Plz make a video on Pediatric hemiplegia and also talk about how much chances are there for him to become normal.

      Hi. If he is having hemiparesis(mild hemiplegia) , it would be a good idea to go for CIMT (Constraint Induced Movement Therapy) wherein you constraint or block the normal extremity(hand) using a cloth or cover and induce the child to perform a functional task (picking up and transferring objects) using the weaker hand. It may initially be for 5-10 minutes and gradually increased as the tolerance level improves.
