I saw tool in 2019, and I can confirm, Maynard James Keenan , the lead vocalist, can do this live with no artificial help.
Just as equally impressive: Danny Carey can also do this live.
Answering to your question, yes, he used to be able to hold that note for that long live in concert back in the time this album was recorded. It's been 20 years since that, so obviously now it's a different story. Time catches up with us all, but he still can make some stellar performances vocally wise.
I want to confirm this. I've seen them live several times, Maynard could hold a scream note for infinity time.
He never held it this long at a live show, only in the studio for the recording. He came pretty close at a few of the live shows though in the early 2000s.
This guttural release was 25 seconds... epic and very hard to copy. Now that Maynard has had covid he says that his breath control isn't what it used to be.

There was a time when Maynard could legit hold screams for that long. I've witnessed that soul shaking power and it's mind blowing to say the least. Sadly he can't anymore but his voice is still amazing. Someone on YT made a Maynard scream compilation video, it's fantastic!!

In my opinion I think the reason why he was able to hold his breath that long was because of going through special force training when he used to be in the military because all of that training strengthened his body and lungs.
I would love to see you guys react to some New Zealand content. Especially the Haka. The Haka was used by my ancestors (Maori is my race), as an intimidation tactic before any tribal war. These days we use it before sporting events, weddings, funerals or as a sign of respect to someone.
