I believe the first limited list came out when the starter decks did, which was about 2 to 3 weeks after LOB launched in the TCG. That very first format, the LOB format there were no cards limited I am fairly sure. Been a while since I checked that format out, but there you can use 3 pot of greed and exodia pieces. But if I recall correctly, Upperdeck just took the OCG limited list and applied to the TCG when the starter decks hit the streets.
May 2002 I'm pretty sure, which was just over a month after the starter decks came out. Lord of D. had its text changed going from 1st Edition to Unlimited and the Unlimited text is used in this format. I think if the banlist came out at the same time as the Starter Deck this wouldn't be the case and the 1st edition text would've been used
