To me, keeping it broad, the guy on the horse represents depression, trying to draw Tyler back in. Tyler’s friends are at the top of the trench (album title) trying to help. Yellow represents hope (tape on the jacket), the flowers dropped by the friends disrupt the trance Tyler was put under enough for him to try and get away, but falls and is drawn back in again. The next two songs in the trilogy (Nico and the Niners, Levitate) build on this. There is a whole lore around this, so much detail and depth.

In an interview, Tyler explained that the jumpsuit is the concept of something that provides you cover and protection when you are feeling exposed. It is both a cry for help (cover me) and admitting you are not enough. The hends and neck line, I interpret as- they will have to destroy Tyler's ability to create to get him to surrender to the depression and overall mental difficulties.

I interpret those “I’ll be right there…” lyrics in the context of a few of their other songs like Ride and House of Gold. The common thread being that it’s one thing to say I’ll be right there or I’d take a bullet for you, or I will make you queen of everything, and it’s a very different thing to actually make another person’s life and happiness a priority above your own agenda.
Basically the "tie me down and the rip off my hands" and the "grab my throat and lift me in the air" are metaphors for the black paint on his hands and neck, and the black paint means blurryface (the guy in the red) basically has control over him, and when all the ppl at the top start to throw yellow flowers, then he starts to break free and run from blurry face (real name is nico) and the reason for this is because the color yellow dampens the bishops (all the ppl in the red robes, nico is the leader) power.

There are different characters, locations, details such as maps and letters and documents. so much world buildings has gone into it. The place even has it's own religion. The story being told continues into the most recent album and will be concluded on the album they are currently wotking on.
