They were originally going to show the head in the box but then decided not to show it. They wanted to add to the suspense of the scene. The head was even made and was used years later in the movie Contagion.
One thing I learned about watching reactions to this movie was not everyone was saying "shoot him". I just assumed everyone thought the same as me during this scene.

Kevin Spacey was on the top of his game in this movie.. A masterpiece in acting.

I'm just wondering: In order for Mills to "become wrath", was John Doe expecting David to off HIMSELF, after shooting HIM?
