Counting Respirations Nursing Skill Assessment | Respiratory Rate CNA Skill

How to check respiratory rate nursing assessment skill (and CNA skill): Counting respirations during a physical nursing assessment seems like a simple nursing skill, but it can be tricky in some cases.
For example, it can be difficult to count the respiratory rate on some patients who don't have a lot of chest or shoulder movement while breathing. Also, you'll want to know how to count respirations without the patient knowing that you are doing so, because patients will often alter their respiratory rate if they know you are assessing it.
When assessing the respiratory rate, one rise and one fall of the chest equals one breath.
In this video, I give some tips on how to count the respiratory rate in patients without them knowing. I also give general tips on counting respirations during your respiratory exam assessment.

Another trick I've found is to hold the patient's wrist while it's resting on their chest (like have them put it across the chest at sternum). Obviously only if they are comfortable with doing so. Then I can feel rise/fall if they're someone who it isn't obvious enough to see.

When I worked at a major children's hospital, we usually checked their pulse with a stethoscope on their chest since their radial pulse could be hard to feel. That also made it easy to take their respirations the same way. If time was short, I'd double-task, counting the heart rate to myself and training myself to count (without thinking) their respirations with my fingers. Down with my fingers got me a count of five, up to a count of ten.
Also when I worked with teen girls as a guy, I went with radial pulses. Plopping a stethoscope on their chest unnecessarily didn't seem to be a good idea. And for respiration, I pretended to still be counting their pulse while noting their respiration while looking to the side. No staring. Managing to make vital signs non-intrusive made the rest of their care go well.
One final technique. I cared for a teen boy in a coma from a motorcycle accident. At times he would get upset. He would tense up, his respirations would be in the 50s and his heart rate approaching 200. I could count the respirations but there was no way to count a heart rate of over three times a second. My reflexes simply aren't that fast. A nurse I was working with offered a trick. Start counting very fast in your head. Speed up or slow down that counting until it synchronizes with the heart rate. That doesn't require fast reflexes just meshing two beats. Then without stopping start your counting at one and keep going for some time period. It works.

Great video. New nurse here. May I ask if I’m watching for the up and down and my eyes are focused on the patients breathing how am I suppose to know when it has been 30 seconds or a full min? Once while in nursing school during clinical I used my timer on my phone and right when the timer went off my instructor walked into the patients room and seen my phone on the bedside table and told me that was a big no. After I would always have a classmate with me to check the time while I watched my patient. But I really need to know how to do it on my own. Thank you

My CNA state test is tomorrow. I’m nervous that I might get respiration as one of the clinical skills I’ll have to perform in front of the NAE because I’m still not great at it. I have a hard time seeing the chest rise and fall and I know I’m not supposed to just stare at the patient when counting but it’s so hard to do it without making it so obvious. I don’t want to fail just because I have a hard time seeing someone breath.

If they let you use a stethoscope, go with that. It is much easier to HEAR breathing sounds than to SEE them, particularly when patients are lying down and breathing with their diaphragm.
