I would not rule out poor/dirty and thin casting causing early failure, without the help of steam leaks via bad vents/lines bad piping.
most everything took a huge downturn starting in the mid 1990's and went awry from there(bean counters cutting costs/recycled material, imports, etc.)
The whole back of the boiler is deteriorating. It’s crystallize and falling apart time for new Boiler.

Mikey, Why did it Fail? On Start up, was not clean purged? To get rid of the Manufacturer's oils.?
Constant fresh water
Why the constant fresh water? A single bad radiator vent isn’t enough to cause that.
"parking two trucks", park one and have someone sit in the other double parked with flashers on/engine running, assuming the road is wide enough for a single lane of traffic to still pass.
worst case fire truck/ambulance comes and they take off out of the way or police show up and you just say loading/unloading materials/tools (lol) worst case they have to drive around the block now and then( likely cheaper than parking tickets/being towed/etc.

Burnham has a pretty lousy track record. 10-15 years is normal. Weil Mclain, Peerless, Williamson will easily have double the life span

My Burnham steam boiler is 30 years old and still working. I hardly did any maintenance on it. The ignitor is still original. I don't know how much life it has left??
