While there are some elements of a “This class counters this.” Balancing system, it’s not as clear cut as that. Heavy with his slow speed is more or less easy pickings for Spies and Snipers. More so Snipers as slower classes are easier to get headshots on. However, most other classes are only as good as the people playing them. A really good Scout counters everything but a Sentry gun, but a kinda decent Scout could probably be easily killed by a moderately competent Sniper. So when it comes to Class counters, think of it more like this. Classes with a more Generalist style of gameplay will typically do better against more Specialist styles of gameplay. But Specialists will usually do better against Generalists if the Specialist is more skilled.
For the engineer, he has four different buildings he can make, each one with different levels. The engineer uses 'metal' to upgrade and repair them, which is automatically picked up with ammo packs. Sentries: Sentry Guns are autonomous turrets that will fire at enemies that enter their range, usually beeping as a sort of 'I see you' thing. The level one is a squat single barrel that fires 2-3 rounds a second with a bit of knockback. Level 2 the sentry extends two double gatling guns which have a higher rate of fire and thus DPS. Level 3 adds a rocket launcher giving the turret a bit of splash damage and extra oomph. Dispenser: Dispensers are essentially movable health and ammo suppliers, which can be handy since your team just needs to hop around a corner, wait a few seconds as the dispenser heals/rearms them, and then their back in the fight. Aside from cosmetic changes, upgrading the dispenser increases the amount of health and ammo the supply. Teleporters: These ones come in two parts, the entrance and the exit. Simply put, the engineer drops the entrance near a spawn then drops the exit near the point/fight. Once both are deployed the teleporter begins operating and can, well, teleport the team. Upgrading the teleporter decreases the amount of time it needs to teleport another person, higher level = faster recharge. To balance this, Engineers can only place one building at a time, have a moderate and finite amount of metal, and these machines don't really pose a threat to experienced players by themselves. Great at supporting the team or buying them time, but don't depend on them to win matches by themselves.

About the water combat, Yes you can fight underwater. the only weapons that don't work under water is flareguns and flamethrowers (they blow bubbles underwater.) although pyro has a melee weapon that is almost exclusively made for underwater scenarios, said melee weapon electrocutes wet players and deals a Critical hit. when a player leaves water, they will still be dripping wet for a few seconds which gives Pyro another chance to kill with this thing.
Basically, yes, each class can counter and get countered by each other. Scout counters Sniper and Spy Heavy counters Scout and Pyro Soldier counters Engineer, Scout and Sniper Sniper counters Heavy, Engineer and Demoman Pyro counters Soldier, Demoman and Spy Engineer counters Pyro, Heavy and Scout Demoman counters Heavy and Engineer Spy counters Sniper, Heavy and Demoman Medic gets countered by everyone due to being mostly a support class, not made for fighting.

Not just classes that counter certain class but there weapons involved to counter like the Sniper's Razorback to counter Spy one hit backstab, Hue-Longheater is Heavy's other minigun to counter Spy on close range with the ring of fire but the Spies can avoid it in many ways, Medic's Vaccinator are counter for Sniper from their one fully charge headshot kill and any classes have stronger bonus damages (expect melee weapons), so yeah that just one part of the weapons counter for the classes

Its very rock paper sciccors. When you encounter a certain character you instantly know what they have and what they can do or counter. Well, most Heavy main are use Sandvich or Second Banana since Sandvich help to keep your medic alive and second banana use for solo heavy but still help to extinguish fire from burning teammate. Don't ask me why food can extinguish fire on teammates, I just want to keep medic alive.

heavy actually isn't that great against demoman or soldier: both of these can abuse splash damage and shott before taking cover, while the heavy needs a sustained line of sight. He is however a great counter to the pyro (since he aslo excels at close range damage, but is more durable, hits harder, and has no projectile the pyro could reflect) and otherwise an easy to avoid but terrifying to face head on sack of health points. yes the engineer can build mini sentries, but these are replacements to the normal one, not an addition (basically, it cant be repaired not upgraded, but is cheaper to deploy, build fast and picking it makes the engineer have some more health... basically if the normal sentry is a sturdy defense one needs to babysit, the mini is disposable and is more an annoyance to place down anywhere to deal free damage than a real line of defense). Also out of the mini sentry case, all engineer buildings have 3 levels: the sentry gains firepower, the dispenser healnd and resupplies faster, and the teleporter recharges faster. Higher level buildings are also more durable (especially the sentry). Higher levels are reached by spending enough metal on the building (an engineer only resource akin to ammunition that he uses both to repair, build and upgrade) so its not like the engineer can just place down max level things: he has to spend time and ressources during the match to do it. Also yes you can snatch the sandwich: once throw, the first player (ally or enemy) that touches it without being full health will use it Overall heavy is not really played in competitive or by high skill players: thats because he has a low skill ceiling and as such a pro heavy wont be able to do anythign as crazy as what a pro soldier or demoman could do; and with only 6 players on each team, his place is more often than not better taken by another class. He is just a slow slab of health that needs line of sight and becomes even slower when shooting, and has no movement trick like rocket jumping to fix his mobility issues.

Rock paper scissors format is not a thing in TF2. You can beat these with game sense.

Overall heavy is not really played in competitive or by high skill players: thats because he has a low skill ceiling and as such a pro heavy wont be able to do anythign as crazy as what a pro soldier or demoman could do; and with only 6 players on each team, his place is more often than not better taken by another class. He is just a slow slab of health that needs line of sight and becomes even slower when shooting, and has no movement trick like rocket jumping to fix his mobility issues.
