Without getting too technical, MIDI is just a standardized format/method of recording and sequencing the input of notes. It doesn't generate sounds. "Digital" (which should really be called PCM, because anything on a computer is "digital") is real audio. And perhaps you even knew this, but I just want to be clear about what we're talking about for everyone's benefit. This is a sequence of notes, like MIDI. And like MIDI, the notes are played through hardware that translates those note values into tones. In this case, you have three chips: the Yamaha OPN2 (aka YM2612 -- produces FM sounds), the PSG (Programmable Sound Generator -- produces square waves and noise), and a PWM (Pulse Width Modulation -- produces PCM for those drum samples). The last one is intended Sega 32X, and the first two are from the Sega Genesis. So it's note sequencing with Genesis/32X hardware.
Game consoles back in the day did sequenced music like this because cartridges did not have the memory necessary to handle real audio. Sequenced music takes up very little room at all on a cartridge.

@@ymtx81z the PSG actually doesn't originate from the Genesis. It was there since the SG-1000, Sega's first console ever. Though it was also utilized in the Master System and Game Gear.

It is impressive how implementing such a song can blast process on the 32X quite nicely.
Don’t underestimate the power of seven dedicated FM channels, four PSG channels, and the PWM pcm channel
actually its 6 fm and 3 psg (3rd one switches from noise)
