I did something similar , where my alchemist made the "ultimate male enhancement pill" that actually made your genitals fall off, and the other teams bard bard fell for it.

Wait I'm confused. The Druid was part of group one, the group who made the fake head. I'm assuming the DM was running both groups at separate times (except for when both groups would PvP) in order to make sure one group weren't privy to the traps of the other group. How in the hell did the druid not know about the trap if the player would have been there at the table with his fellow group members? Either the player was there but the character wasn't privy to the plan, so the player was chill with committing to the lols and playing out his druid doing something stupid (which props to the druids player for being chill enough to do that), but if BOTH the player and the character were not privy to the plan, that would be impossible unless the Druid missed a session and the DM just chose not to catch the player up (which would be a dick move). Funny story, but that is the one thing that confuses me.

So noone, thought that placing an entire head of a lich would be a bad idea? Like, hand and an eye are one thing, bu how would you control the head when your own brain is currently rolling around in the dirt?

The best thing I got away with was the PC were trying to find who was behind one of the mystery in the kingdom. (People were being transformed by fey influence and they were trying to figure out who was controlling it.). Turns out anything they said that I could use was causing a change.
I wouldn't suggest anyone but a veteran dungeon master try this it's way too easy to make the ruse convincing from the DM's position as in unfalsifiable levels of convincing.
I mean, it WAS kind if the DMs fault too allow that, but the players also didn't care for the druid, so they are also very bad people. A shitty table that did a bad think and we all laughed at the absurdity of it all
That was the whole theme of the game my man, it's also a story from previous editions where character death was far more common and accepted, which seems incredibly likely here.
