    Excellent content on a very important genetic risk factor that fly's under the radar.
    I'd add that until we have the genetic therapeutics available it's important to minimize all other CVD risk factors, including reducing ApoB.
    PCSK9 inhibitors are very effective at reducing ApoB and they also reduce Lp(a) by 20-30%.
    Another risk from elevated Lp(a) is thromboembolic events. There is some data supporting baby aspirin for high Lp(a). However, this should be discussed with one's doctor due to the increased risk of bleeding from aspirin therapy.

    One more point, everyone should get their LP(a) tested once. It's a very inexpensive test at only $20-$30 through direct labs such as Ulta Labs. Your PCP will likely add it to your lipid panel on an annual checkup too, if asked.

    Dr K. Please comment, there are those that say LDL’s are not the problem. It’s the VLDL that get converted to Small LDL’s that are the problem. They are able to gain access to the artery lining and cause atherosclerosis plaque. Does your labeling’A’ and ‘B’ also follow this small LDL explanation?

      Exactly! VLDL's are the particles that come out of the liver and transport triglycerides and cholesterol esters to the periphery. Once the triglycerides are distributed, the particle becomes IDL and LDL, both of which have a higher relative cholesterol concentration. The smaller, denser LDL particles seem to lodge in the walls of the arteries causing atherosclerotic disease which may be related to the absolute ratio of triglycerides to cholesterol in the original VLDL particle (higher the original triglyceride content, the smaller and denser the LDL particle)

    So hdl and ldl aren't cholesterol, why keep calling thrm good and bad cholesterol?

      Awesome question! it’s a little deceiving. Cholesterol is cholesterol. On your laboratory report, you’ll notice that HDL is labeled HDL with a little c an LDL, LDL with a little c. That means this cholesterol is associated with each of those particles.
      The problem is that number can be deceptive. What’s more important is the number of LDL and HDL particles. That’s why we’re getting away from reporting just cholesterol, HDL and LDL and now moving more towards measuring apoA1 and apoB which are actual markers for both HDL and LDL particles, respectively.
      I do intend to include this in my part two lecture on lipid labs. 🤛🏼
