MyLifeAsLisaa a friend told me its just another part/segment of the fanmeet... so one member dress up as a doctor in each fanmeet... or thats what I get from photos and videos..
It's a segment included in their fansignings for the Guilty promotions, and if I understood correctly from what I read on their fancafe, it was Minhyuk's idea to introduce this segment! For each fansigning, there was a member chosen to be the special counselor. Before the fansigning, the selected member would stand outside the fansigning room holding a box where fans could put their worries and concerns that they wrote and then the fans enter the room. Then after the fansigning, the rest of the members would leave the room but the counselor of the day would stay to answer the questions and after that's done then the fansigning is officially over. I know this from experience since I was lucky enough to get picked to go to one of their fansignings and Hyungwon was the counselor that day! Because they had 2 fansignings each week since the start of their Guilty promotions (except the last week in which they had 4), each member was the counselor at least twice.

MyLifeAsLisaa yup yup, it's a lottery system and they pick 100 fans each time. The more albums you buy the greater chance you have, but there are stories of people who buy a lot and don't get it and people who buy a few and are lucky to get in. So in the end it's just all luck! I failed a few times and only managed to get into one fansign (bought 16 albums), but the total albums I have is 40 haha.
