When you double crochet increase in the same stitch after chaining, are you counting the chain as one stitch? Also, is the “same stitch”the one the chain is coming out of?

I have done this pattern 3 times and I keep ending up with extra stitches I think… once I get to the fourth round I have an uneven pumpkin due to extra stitches and I’m not sure where I’m adding them
