  • Join me as I react to scrubs episode by episode and give you a real doctor's take on the series!
    I am really loving this show so far and am excited to continue to watch more episodes!
    Watching this video back, I realized there was something that I really would like to have commented on, but I didn't... The patient who was Spanish speaking, when they first introduced her, Elliot said she had Lupus and came in with shortness of breath. Her plan was starting heparin and getting a VQ scan. This was something that just was natural for me to hear, so I didn't even think to comment on it in the moment! Lupus is a disease that makes you more prone to clotting. Elliot was concerned for a clot in her lungs, which is why she started an anticoagulant (heparin) and got a VQ scan, which is a scan of her lungs to help evaluate for a clot.

    The 1 in 3 death rate thing might be wrong but I do love how they use it to set up your expectations (three patients in peril, one will die) only to subvert them by having all three die to really gut-punch you. This is easily one of the best episodes of Scrubs, but there are sooooo many great ones still to come.

      @@ElvinIsbell It made it an epic episode. if they put the story book ending- it would be forgotten. I've seen that seen thousands of times, and I cry every time.

      I am the type of person that gets swept up in the story to even remember details like that at the beginning. When it came back at the end it was an extra gut punch. I love it.

      @@robertluong3024 I remember the 1 and 3 and wondering "What patient dies?" After all 3 died, I was like "Oh, that's where they're going. I've got to watch more of this. It might be my favorite show ever."

    Scrubs is an awesome show. It's listed as a "comedy", but it's so much more. It does have some good life lessons in it. It often deals with really serious subjects with a touch of humor. There are also some just plain goofy episodes, so not every one will be deep. There are 4 or 5 really deeply emotional episodes. Enjoy watching it! I look forward to seeing your next reactions!

      I just realized that your very accurate description of Scrubs also describes another show listed as "comedy": Bojack Horseman. Beneath the veneer of animated animals doing human things, the scripts often deal with incredibly serious and traumatic events such as addiction, abuse, neglect, and death

      I think it went the route of MASH, it had tons of comedy, but what made it great was the "drama" and the heart of the show.

    My wife has been a family medicine attending for the past 12 years. When she is asked what tv show is most accurate when it comes to her job, it isn’t ER, it isn’t Grey’s Anatomy or any of the other ones, it is hands down SCRUBS. Great show

    Ah! So glad you loved this episode. Scrubs, despite being a comedy, is so good at hitting those emotional moments. When the script for this episode was first written, the network wanted the show to change the ending so that all three patients lived, so the show stayed lighthearted. But the showmakers insisted on doing it this way. It really sets the mood for the rest of the series imo

    Yeah I feel this one, I’m a dialysis machine technician, I’ve seen what some patients have to go through. When a new employee asked me what is the most important thing to learn in the job was? I said to be good at talking to people, not just small talk (even though that’s very important) but real communication, especially about things the patient maybe interested in. For some of my regular clients (I don’t say patients as I am not a doctor or nurse) I even read up on things, like a lady I have to visit quite often I read up on embroidery as she was one of the last hand stitched embroidery craft persons in the area. So I read up on the history of it, it’s so important that we try to make people smile when we can as unfortunately that smile doesn’t last all that long with what they need to go through. Some jobs are just that, jobs, some are careers and others still are callings. After all wouldn’t we all want to be afforded the same consideration when it’s us on the receiving end.

    A couple of interesting things about the Janitor: In season 1 he was originally supposed to be just a figment of JD's imagination. He wasn't supposed to interact with anyone else (although he does in two episodes) The other quite amazing thing is that most of Neil Flynn's lines in later seasons are pure improv. The script would just say like "Janitor says something mean to JD"

    Sometimes, patients have such amazing spirit! My best friend since high school contracted Reactive Arthritis (Reiter's Syndrome) at age 20. By age 50, he had two hip replacements, a knee replacement, and was starting to experience organ failure. He lost two kidneys, got a donor from a nephew but lost that one to infection. He was still working as an electrical engineer and doing peritoneal dialysis every night. In his last phone call with me before entering hospice, he actually said, "You know, Randy, overall I think I've been pretty lucky." I am still in awe of his words and his spirit. Thank you for these reactions. They're great!

    I always forget how tense Elliot & Carla's relationship is in the earliest episodes. Really love this episode for showing them clashing but bonding over a difficult case.

    All the respect to people like Mrs. Tanner who realize when it's their time and are ready to go. Dragging life on at any cost will just end with you getting dementia and lying in a bed being treated like a baby again with no idea what's around you any more. That's more horrifying to me than the idea of dying at my full mental capacity.
    This show has some REALLY great serious and emotional moments. I think it even won some awards for how it deals with death.
    No worries about how frequently you get to watch these. You have a lot on your plate right now. Real life comes first. Best of luck to you.

      While I generally agree with your setiment, I disagree in this specific case. I've been doing dialysis for 14 years now, it's not that big of a deal. I think her grandchildren would have profited from her being around for a bit longer.

      I think she should have waited a few weeks after her granddaughter's birthday. Now, the child will associate her birthday with her grandmother's passing.

      Not everyone necessarily gets severe dementia, although it's obviously higher risk factor as you age... It has been over 20 years since this episode aired and I wonder if the expectation of quality of life past 80 was a bit lower back then. 74 does seem pretty young to just say "I'm ready" now with no obvious disability. But maybe she has constant back pain and frequent arthritis flare ups and her memory is less sharp than her personality these days, and she just has been putting on a chipper face until now.

      @@formdusktilldeath I don't think it's fair to compare your case with another person's. Maybe she doesn't want to rack up a burden of medical bills, or maybe she doesn't want to spend the rest of her life reliant on dialysis. From the sound of it, she was a very active woman, and to be reliant on dialysis would probably be awful for her. She wants to go on her terms.

    I've watched Scrubs several times all the way through, and I've seen this episode at least a half-dozen times. It still makes me cry all the time.

      I didn't even realize how much I'd been waiting for this episode specifically. The first real "we're gonna really punch you in the heart today" episode, out of several. Dr. Crystal's already watched "My Lunch" so I guess she knows this is basically the predecessor of that one a few seasons later...its grandmother, if you will :)

    Scrubs went off the air In 2008, and had one of lf not the most perfect series finale of all time.
    But here I sit 14 years later, and the ending of this episode STILL chokes me up.
    Such an underrated show. I'm glad you are sticking with it, and I'm glad I get to watch it again through the eyes of a physician.

    The funny thing about the Janitor (and to pay attention to in the first season) is that he was supposed to be a figment of J.D.'s imagination, which if memory serves me right, is why no one else really interacts with him. But Neil Flynn (the actor who played the Janitor) was so funny with his lines, which were mostly ad libbed that they decided to make him a "real" character in season 2.

      The show creators said that was only going to happen if the show wasn't renewed for a second season. That in the last episode it'd be revealed he was just JD's imagination. But Neil Flynn was changed from a guest star across all of Season 1 to a regular cast member afterwards because of how great he was.

      If you listen to Fake Doctors, Real Friends it's been established that Bil Lawrence did have this idea but it's pretty quickly disproven, he interacts with many other people in S1. It's become a joke that Bill still tries to defend this was the idea. Excellent podcast, I'd recommend it!

      @@jasonlandow9291 At some point I'm going to have to rewatch it and really look out for it. They can be tricky! Like Kelso looking at what JD's being distracted by, and it could've just been "why are you staring at nothing" instead of "why are you staring at the Janitor mopping"...a trick they'd use again for another "imaginary character", of course...

    I was so pleased when I found out that Kathryn Joosten, the actress who played J.D.'s "old lady," went on to co-star in "Desperate Housewives" for several seasons! She really was a great character actress. Spoiler alert: Her character, Karen McCluskey, had another on-screen death in DH, this time even more emotional than the one in "Scrubs" because the actress herself died shortly afterwards.

    Much respect for watching them in order. Not many reactors do this and while the series can be consumed in a non-linear fashion, I think watching them thru is still the best way. It helps define the relationships of the characters, something Scrubs has always been amazing at. Still my favorite tv show to this day. Always watch it when life starts taking that downward turn.

      I agree, and while obviously there are a few "best episodes" like the rabies one, they're definitely all worth watching. Can't help but feel like it's a bit of an insult to the show to just watch those few episodes and ignore all the rest.

      Love the excuse to rewatch early episodes to remember some of those earlier character developments. I always forget how frosty Carla and Elliot were at first; this episode was probably the first real bonding they did if I'm not mistaken.

    I think the actual statistic is that 1 in 3 patients admitted to the hospital will eventually die at the hospital, not necessarily during that specific admission.
    Also I've seen Scrubs many many times, but I just started working full time in long-term care and this was the first time this episode has made me cry. I understand the bond that healthcare workers make with their patients so much more than I did when I first saw it.

    It's funny how much of what you say are things they address later in the show. Especially how Elliot calls out Dr Cox later about having never really helped her, but Cox isn't the one who had her back all the time... it was someone else.

      Personally I prefer to wait until later in the show and say "funny how you said XYZ about this earlier in the show", rather than the other way around.

    This one always gets me too, especially once "Hallelujah" starts to play in the background, just hits you right in the feels!!

    Jimmy Valvano said that you should laugh, think, and cry every day, and if you do that- you've had a good day. One episode of Scrubs (especially this one) hits all three.

    Scrubs does drama better than any other medical show and it's because they hide it in the comedy. You can watch 10 episodes in a row of comedy and the land mines of drama episodes really hit hard.

    My wife's grandfather did about 1 month of dialysis before he said nope, and prepared himself to die. My grandma died when she was 83, and for several years before that had said she was ready, husband had died long ago, friends had passed, both kids and her grandkids and grown up. Sometimes you just know. A Nurse literally saved my dads life, a young DR, not an intern, but still very young, had decided he just had a virus and was going to send him home w/meds. When he left the room the nurse said your dad isn't going anywhere I'm still on shift after he leaves and i'll get the next Dr. to admit him; he was in the ICU the next morning and in a coma the next afternoon.

    I think Cox was aware of Elliot's hard time and would be there if she needed it and if she went to him again but he knows that she will make the right decision at the end of the day and it's his schtick of being the "tough love" doc

    I read for the first season of scrubs they just had doctors come in and tell funny stories and wrote episodes around them. Season 1 is definitely more realistic than the rest of the show. It shifts to a character driven drama with the hospital just there to give the characters something to do. On a related note the show House was written off cases from hospital morgues where doctors failed to diagnose an illness which is why all the cases are so over the top.

    0:50 "one of of every three patients that's admitted to this place"
    He's referring to the ICU. They could have been more clear about it, but throughout Scrubs, although they do rotations to other floors for other things, they're mostly in the ICU.
    1/3 sounds pretty accurate for an ICU
    But it's funny how much is supposed to be ICU but looks way more lax

    I've been in hospital more times than I care to remember and most of that was before I was even 17 and as a patient you HAVE to have a sense of humour or being in there is going to destroy you. I always crack jokes even in the worst of situations (in fact, that's when I crack the most jokes) and try to make my doctors laugh. Most of them do but I've met one or two that had the bedside manner of a brick. My first surgery that I had as (what I consider) an adult, it was my release day and I had Queen "I Want To Break Free" playing on my phone on repeat. Maybe not the exact right time to do it but the other patients in the room didn't seem to mind.

    This show doesn't go for cheap "gut punches" but it isn't afraid to lay into you either. It will go from laugh out loud funny to OMG faster than anything and it's the authenticity of the characters that brings it out. This episode was definitely the biggest "oh this is different" moment of the first season and it's perfect.

    when i got thyroid cancer my doctor/surgon who was the same and she was blunt as hell when she gave me my news, and she was fast aswell in saying this is what we are gonna do. but as i thought about it and there is really no good way to say that you have cancer and i am quite the blunt man myself, so i ended up liking her alot. and every time i saw her after that first meet she was the best and funniest, warmhearted and caring doctor ive ever had to this day. i even told her that and that i wished that i could have her as my personal physician. we even have contact once a year now to check up even if the period of tests to see if the cancer come back is done.

    One thing I love about Scrubs is that it's a love letter of sorts to patients, showing the ways they affect the people working in the hospital, the wisdom and weirdness and simple LIFE that each patient brings with them and sometimes leaves in the hospital.

    i really hope you continue to watch this, i'm excited to continue this journey with you because this is one of my favorite shows of all time. i'm not even really into medical shows, but this one i watched when i was a little kid and still love it so much to this day. great reaction, am loving these!!

    I am empathic and I love that Bill Lawrence can make me laugh, cry then laugh again all in the span of just a few minutes.
    I have re-watched this show so many times from start to finish (excluding season 9) and I never get sick of it. I honestly think I re-watch it almost once a year. It really is a great show.
    Thank you for reminding us how it feels to watch it with fresh eyes.
    By the way, pay attention to the music choices. Bill Lawrence is very careful with what songs he uses in his shows.

    I hope you react to the whole series. there are some seriously heart tugging moments throughout the show, or at least through season 3 which has my favorite episode lol. i remember getting exited to watch it on tv back in the day after school. and as a kid i watched it very different than i did when i went back as an adult to watch it again.

    Im absolutely enjoying your reaction videos to scrubs! Out of all the medical dramas I've watched, its mix of comedy and attempt at medical accuracy has always kept it at a special place in my heart. I recently started watching Chicago Med, and I have been absolutely hooked! After watching a bunch of shows like House and The Good Doctor that have a bunch of 1 million to one diagnosis, its nice to have another more grounded medical show. I'd definitely recommend giving it a shot if you're interested! Its a lot more serious than Scrubs but still carries a nice energy.

    She doesn't speak Spanish, but later on, you find out that Elliot speaks fluent German, so it's not like she doesn't speak another foreign language.

    Scrubs had a perfect blend where they spend 95% of the time just having fun but as a result they get to spend 5% of the time absolutely punching you in the gut.

    1) About people calling out doctors that look and are young... I just can't, for the life of me, figure out why people would think it's appropriate to do so.
    On my honeymoon a bunch of years ago, as one of the pilots shows up to board the plane, he sits near us for a bit. Dude looked like a straight up teenager to the point that my wife and I noticed but didn't say anything. Some... guy sitting around us, possibly drunk, felt the need to ask him if he was our pilot and then tell him how young he looks. The pilot wasn't too thrilled based by the look on his face.
    2) About Spanish. I every now and again tell people that we as Americans should know at least a little bit of Spanish. Of course there's a certain type of person who really doesn't always appreciate that. I'm talking about red in the face levels of irate, lol. But I interact with hundreds of people a day, and Spanish 100% helps me out. It just makes interactions easier.

    There is really nothing I can say about this show that basically wasnt already said .. so I just repeat it :). It's really just amazing. Also watched it several times, years ago .. it's light but meaningful. It's funny, it's goofy, serious, you can laugh , you can cry, the cast is amazing the stories are great .. the soundtrack is spot on .. they just talk about certain things in such a way, it feels like as if these episodes somehow always "know what to say and how to say it". It's just a really cool show :). Looking forward to the next episodes. I really enjoy your videos and medical input.

    When I was admitted to Walter Reed hospital we could sign ourselves out for a few hours especially on the weekends because nothing medical would happen until Monday. Come to think about it my stay at a German hospital was the same.

    This was the episode that solidified this show as an all time great for me. What kind of comedy has the guts to kill 3 people in a bittersweet way in just the 4th episode ever?
    I said it on one of your other videos, but this is why I'm glad you're watching in order so you can get the context of not just where the show is, but where these currently young doctors are in their medical journey. It's interesting to see how they grow as time goes on.

    I ve watched Scrubs trough and trough about 4 times. One of the best sitcoms ever. Be warned, you are down for an «emotional... ride of some sort»

    I'm a year late. But I have to say I really like you and reactions. I've watched a lot of Scrubs reactions and while a didn't dislike any of them then yours is definitely my favourite. Happy to follow along.

    Scrubs is one of the shows I will always go back to watch, this episode alone was nominated for awards on its own merit. The episode that gets me the most is when Dr Cox loses 3 people and goes off the deep end a bit.

    Still one of my absolute favourite shows.
    The script actually comes from a diary of a medicine student until resident, who the producer knew (they were roommates in college). He didn't even change the names (the "real" Dr. Turk is a white guy by the way - which I find very funny).
    And it wasn't filmed in a studio. It was done in an old hospital, so the shots aren't those typical wide-screen, perfectly lit frames, but everything is a little more cramped, which makes it feel less fake.
    And of course the more realistic part of patients actually dying (quite a lot of characters die during this show, from sidecharacters to main ones) and not them just being "the heroes in white" that save everyone.

    I love Scrubs, and so much is silly and fun. But when they pull out the emotional stuff, I don't think any show does it better; definitely no comedy I have ever seen. My Old Lady is definitely a top 10 episode in the series.
    Trust me, you will grow to love this show more and more as you start to really get to know these characters. Dr. Cox is definitely a complicated character you grow to really love as the show goes on. You realize he truly does care a lot. My favorite episode in Season 1 is My Fifteen Minutes... it is fantastic.

      I didn't want to say too much when she criticized Cox for being very intimidating/unapproachable as an attending, especially Elliot, since that's a developing thread. My mind was instinctively trying to defend him or explain how he's really great. And then realized, you know what, just let it play out. The characters are complex enough not to need a quick answer or simple judgement.

    Scrubs is just writing Genius.
    Say what you will about Zachs later work, he was brilliant as JD.
    What I adore most about this show, is how well they use licensed music when you compare it to how some other shows and movies squander good songs.
    Just look at Hallelujah here, and compare it to the absolute disgrace of having it used in Watchmen.

      Hm, I wonder what version of the music she'll be getting in some of the other episodes. I still think it's criminal not to at least notify that music has been replaced when series like this are moved to different platforms/formats...
      I admit, I forgot Watchmen even used Hallelujah, since, yeah, that scene was probably the worst of the movie. Personally I think most of the other big musical moments were unusually great, but it's certainly a divisive movie :)

    If I recall correctly, this was one of the first times that the show veered away from comedy. Ultimately, it led to the show having some more serious moments in later seasons, which I think it handles beautifully.

    I remember watching this when it first aired on NBC. And at first, I thought it was a pretty funny sit-com. It was silly, semi-brainless, entertainment.
    And then this episode hit... And I realized I was watching something far better than I could ever had expected. This show gonna get super real.
    (And then... "My Lunch.")

    OK, so, like, other doctors have made the same complaint about inaccuracy and again I have to point this out. The hospital on Scrubs is horribly underfunded, mostly because medicine in America is a for-profit enterprise which means some places are a lot worse than others. That particular hospital is poor and cant get the best people nor can it get all the people it needs and they also dont have the best equipment or labs or surgical suites. They make this point many times during the series. In fact at one point Bob Kelso even admits he hates being a jerk but he has to look tough so as to keep the terrible place running smoothly.
    Maybe 1 in 3 is a tad unrealistic, but the truth is since America values money more than life, there are some real hospitals that just dont do a great job. That is very sad but apparently not sad enough to get 300 million people angry and marching on Washington to have it changed.

      Worse - 300 million people in this country (or near enough) are angry about it, but those 300-ish million don't have enough money to motivate enough politicians to change how our healthcare system operates.

    I think it is important to differentiate between fair and unfair bias.
    Stereotypes often arise from justified bias, but are turned to the extreme.
    For example a fair bias would be to say that men are generally better at sports.
    Unfair bias or stereotypes are to either say every woman is bad at sports, or even to say "every man is better at sports than women".
    I think the surgeons thing was a fair bias back in the day, when everything in hospitals were a bit more competative, but has become largely a stereotype.
    Although I wouldn't be shocked if a study was done and they find that surgeons bond less with their patients than other docs.

    I really don't know where the 1 in 3 patients die statistic comes from. Even in the 90's there was a mortality rate of 8.5%. Even in emergency admissions it only was 13.5% max. The only thing I can imagine is that the show means that: [...]more than 1.5 million adults are admitted for community-acquired pneumonia (CAP) in winter, with 1 in 3 dying during their hospital stay

    I forget if they explain it in this episode, but they do make a point in some episode that most of their patients are older adults who are in and out all the time. So if somebody might be admitted 20 times over a few years, 33% doesn't sound like much.

    Regarding speeking spanish in the US. My best friend in high school took French & I took Spanish. As luck would have it, he ended up marrying a latina gal. He was wise...it is a blessing _not_ to understand ever little thing your spouse says!

    idk if you can/will answer this BUT:
    what is it like to witness your first death and was your first expirience "in the job"?
    gladly when my great-grandmother died i was too little and grandmas/pas are still alive so i never ever witnessed someone dying.
    i try but cant imagine what it is like....especially if its part of your dayli life so to speak....
