  • This is an educational video about retrieving additional epinephrine from a used EpiPen auto-injector, both new and old style. This video was recorded onsite at Roane State Community College's Center for Health Sciences in Knoxville, Tennessee. The presenter is Dr Thomas Kessler, the videographer Wm Fred Baty.

    Thanks so much for this. I've found myself in situations where I needed more epi (camping, on an airplane etc). Sometimes multiple doses are needed before you can get to an ER. Obviously this is a last resort, and obviously it's not sterile or even very sanitary, but if I had the choice between this and death, I'll take this any day.

    I feel like the detractors in these comments have never gone into Anaphylactic shock. Air injections and bacteria are not a concern when the alternative is death. More harm than good? As opposed to dying? I'm not sure how much more harm you can do beyond death.

    I took mine apart! I was prescribed the brand name EpiPen as shown at 9:40. Those 3 tabs you mentioned, there's 3 on each side. I used a knife and cut those off, pulled the safety and slid the entire assembly out of the top. The white insert that's around the syringe is part of the firing assembly. The spring slides inside of that and around the syringe. On the syringe, at the very top is a small 'ball' detent that gets locked in rather loosely to the white housing. When the safety cap is inserted, it pushes the ball outwards to get a full lock onto the white piece.
    The white piece consists of two parts. The syringe ball locks into both. When the pen is pressed against you, it squeezes the ball outwards, much as a tennis ball would when placed under a car tire.

    This is fantastic! Thanks for this! I spend a lot of time in the outdoors and will hopefully never have to use this. Nonetheless, it's a good education to have.

    Brilliant. Well worth knowing for people far from a hospital, such as ocean sailing and wilderness hiking. One additional thought. It is not necessary to be precise in the dose. Standard epipen is 0.3mg. In hospitals standard dose for a cardiac arrest is 1 mg, direct into a vein, so there seems very little risk of overdose in the field

      True, but it appears that the plunger is limited to a very small range of motion, displacing the volume of the intended first dose. So the largest dose(s) that can be administered is that displaced volume of epinephrine solution. Hence the need to draw in corresponding volumes of air to permit administration of doses beyond the first.

    Sir you are awesome, as a backpacker I hope to never need this information but thank God I saw it. I do wonder though why on earth they put 4-5 doses in a pen but only make it usable once. Seems like a waste and given that they say a dose only lasts 15 minutes that would mean at one dose to last an hour to help you would need 4 pens. Most deep woods or lake scenarios are not 15 minutes from professional medical.

      Aussie Dingo depends on the country imo, an autoinjector here cost about 500k rp, which is around $40

      The problem is that it is much more difficult to come up with a device that meets current regulatory approval criteria *and* permits administration of multiple doses. There was a 'Twin-ject' device that allowed 2 doses to be administered, but the second one was not auto-injected and the device was very complicated to use, as compared to single-use devices.

    Anyone trying to salvage the unused epinephrine from the used pen must do so with the full knowledge of the risks for bacterial contamination from a used pen and the possibility of injecting air IM.

      And then weigh the likelihood and significance of these risks against the risk of not providing further potentially or actually life-saving treatment of a sever allergic reaction. A tiny amount of air injected IM is unlikely to cause sever consequences. Similarly, risk of bacterial contamination exists every time you get cut and bleed. These are minor, almost trivial concerns by comparison.

    BEWARE!! That spring is under a massive amount of force. I work where those spring units are made and that spring is strong enough to propel a unit 25-30 yards, not feet! Definitely wear safety glasses if you're going to try this.

    This is a great video. People need to remember that this procedure is to be used in a life threatening situation when no alternative is available. To often in today's throw away society people think that only the PERFECT solution is acceptable. I am amazed at how the birthing procedure MUST BE PERFECTLY STERILE!!!!. If that is so I wonder how baby's were ever delivered during cave man time. Anyway pardon my digression.
    What I really wanted to say was that by watching another video and watching this video closely, it appears that the black part of the plunger is the same size as the dose needed. That being said you should be able to move the plunger the distance of the black rubber section. This you could do by simply holding your fingernail to the vial at the spot where the front edge of the plunger is and then depressing the plunger until the back edge of the rubber comes to that point.
    Hey It is just a thought! If you don't like the idea you don't have to use it!

      Jon Andriotis you are 100% correct the black plunger on and epipen is the dose size to be pushed out. So all that’s needed to be done is hold the syringe point up and draw the black plunger to the back of the syringe to draw in that amount of air then drip the syringe so the the air moves to the top and push the plunger in another dose will be pre measured because there’s also a stopper that Won’t allow the plunger to be pushed in more than a dose. I’ve done this trick a few times having sever allergies and in Canada an epipen is $125 and I’m not always close to the hospital I wish everyone know this people have died because they needed a second dose and could get to a hospital in time.

    This is Interesting video of how the auto injector pen works, but as far as actually doing this modification to an Epi-Pen and quickly enough in a remote area, there are just too many risk factors in which this idea will do more harm than good. .

      What risk factors? If a dose lasts 15 minutes and it takes 3-5 minutes to get 3 more doses out of this how is there a detractor here? It has a high likelyhood of damaging the needle in the process but don't really see any potential medical issues.

    You could save the hassle and buy Twinjects instead... They are designed to give two doses in one "pen"

      Yeah but a dose only lasts 15 min per medical info so if you're actually in the outdoors your chance of finding medical in 15 minutes is slim to none.

      where are you getting that information? You can administer 3-4 doses and more especially if the patient is dying.

      HERPDEDERP49 thats not true, the smallest amount of air in the bloodstream can cause a pulmonary embolism

      it takes about 100ml of air injected at more than 100ml/s into the venous system to cause an Air embolism.
      that's about the size of a perfume bottle. which is actually a pretty large amount to accidently inject. and also very quickly
      however, 2 mL of air in the cerebral circulation can be fatal, while 0.5 mL of air into a coronary artery can cause cardiac arrest.
      still not huge but that's still alot to accidently inject especially in an area where an epi pen would not be put.

      Everyone here is wrong since to cause a gaseous embolism the air has to be injected into the bloodstream, and the epipen is an intramuscular injection, so you don't even hit a vein.

      You are scared of the air going through the needle.
      What i am scared about is, how fast the needle goes into your body, it just went through a plastic bin which is like 5x tougher then skin. That thing travels like 500fps
