  • Ugh! Synthetic progesterone is soooo confusing! You probably know women who used it to get pregnant. And you know women who used it for induction of labor. You might know someone who used it for irregular periods, or even contraception. And now we’re talking about using it for menopause. In this tutorial, I’ll present the different forms of progesterone that you can use for menopause. And you’ll probably be surprised and relieved to learn about some of them.

    Welcome Back Home (in Houston), Dr. Barbie! I can't believe you made a lifestyle change/move for all of us-- to provide the best, most-up-to-date video productions, props and info. How lucky are we?! I love your new Menopause Barbie office, like your china hutch, its beautiful and fun, just like you. Your passion for what you are doing really shows. I look so forward to these videos, share your website with my post-menopausal friends and am learning so much. Today, I learned about other options for progesterone like IUDs, suppository or vaginal gel options. (and the funny dentist story! I come from a dental family, they'll get a chuckle too!). And I look forward to your "combo options" video since that is what I use now. I really can't thank-you enough for all that you have sacrificed to make these videos for all of us! I feel like I know you as one of the best-ever doctors I could ever have! Bless You, Annie

    Hello Dr Barbie. I hope you remember me. I had a consultation with you in October last year due to post menopausal bledding on HRT bleeding. I did not want to come off HRT. The bleeding stopped after a month or so. It was suggested by the gynaecologist here to combine Utrogestan with Mirena to counter the bleeding with additional progestogen support. But due to long queue in the NHS my turn came only very recent. As my bleeding had stopped and been free of bleeding for 5 months, I was debating and researching if I should go on to Mirena at all. Part of me said why rock the boat. Also as I basically started taking HRT because of lack of sleep, I thought of continuing with Utrogestan. Lastly I was worried about the Mirena getting expelled. But your table of comparison in the other video is fabulous. Had I known that, I may have decided to get the Mirena after all. Superb video Dr. Barbie. Thank you so very much for educating us.

      Absolutely Barbie! Thank you for teaching us to read ;-) You are so right, my gyne asked me how I knew that estradiol was bio-identical, and nearly fell off his chair when I asked specifically for a vaginal bio-identical progestorone.....(I have VERY lax joints and so you will understand why I did not want to take progesterone orally!). As I am now able to have a much more informed discussion, I have decided to discuss the possibility of having an elective hysterectomy....put simply, as (the 'chameleon') progesterone is required to protect the endometrium from thickening and possibly cancer, and as I will not be pregnant again, then it would be entirely appropriate to have my uterus removed. So look forward to your videos, with a thousand thanks :-)

    • @MenopauseTaylor
      @MenopauseTaylor  Год назад

      They are two completely different chemical compounds.
      But the key is to know that no two bodies are the same. So some bodies will prefer one and other bodies will prefer another. This is that case with all options for menopause management.
      That's why I always teach that women are not robots. Nothing is right for all women.
      "Bio-identical" has become a marketing buzz word. The only opinion that matters is YOUR BODY'S. And your body doesn't care about marketing.

    Thank you Dr Taylor for all the wonderfully helpful advice you so generously share with all of us trying to navigate our way through menopause, it's very much appreciated.
    I struggle with progesterone HRT side effects (bioidentical and Mirena IUD) as well as Duavive/Duavee and would like to try Prometrium vaginally but my gynecologist said NO! When I challenged her, she reached out to the top gynecologists in our big city and this was their collective response to using Prometrium vaginally -
    "Vaginal prometrium is definitely not adequate. There is no guideline in the world or proper RCT (randomized control trial) that supports this."
    I have been unsuccessful in providing my doctor any scientific studies that prove that Prometrium inserted vaginally is sufficient for uterine protection. Is there an official study (online or anywhere) that I could access that proves this? Vaginal insertion is not a suggested option in the Prometrium paper instructions provided with the box of pills.
    So... if I must take Prometrium orally, do you think 12 days every 3-4 months is adequate to thin the uterine lining? (This was suggested in Dr Mache Seibel's book THE ESTROGEN WINDOW p.44). He does not specify any delivery method or brand of progesterone other than to suggest that he uses bioidentical in this instance.
    BTW, I use Estradot patch 50 and replace it every 4 days.
    Thank you.

    I've watched the video multipe times, and read many of the comments below. I'm confused why your telling people that taking progesterone will feel icky, and you only want to take it to protect against cancer. Obv I want to do that, but I thought in Perimenopause developing a progesterone deficiency (or at least too little progesterone compared to estrogen) causes all kind of symptoms, and that taking progesterone may help calm them down (insomnia/anxiety/irritability). My question was orig whether Mirena could still help with those symptoms (insomnia/anxiety/irritability) too or not since it stayed in the vagina, or if either the oral micronized progesterone vs birth control progesterone would do a better job. But reading your other comment responses below it sounds like you don't think progesterone helps with anything but uterine cancer / regulating period?

    • @MenopauseTaylor
      @MenopauseTaylor  Год назад

      Progesterone ONLY purpose during your reproductive life was to support a pregnancy.
      "Pro" = in support of
      "Gest" = gestation ([pregnancy)
      "One" = hormone
      Progesterone's ONLY purpose during your post-menopausal life is to prevent uterine cancer.
      This has nothing to do with "what I think." It's hard science.

    Hi Dr. Taylor, why is is so hard to figure out whether or not HRT is good or bad? I have been watching your tutorial videos and am all set to take the plunge and see my doctor about starting HRT. Then there is a RUclips video that someone makes that suggests that there are all kinds of horrible side-effects of not getting the balance of progesterone and estrogen right - itching, bloating, bleeding....the list goes on and on. Does that mean that every woman has to endure a minimum of 3 months of potentially the wrong combination to ensure she has given the hormone therapy enough time to ‘work’ before she can tell her doctor what the symptoms are so that an adjustment can be made to ‘correct’ the balance of the two hormones? And, is a blood test to determine the woman’s current ‘hormone panel’ the best way to start the initial determination of estrogen/progesterone dosage? To be honest, the whole process scares me. I feel that I need to do something now that I am 58 and have been post menopausal for 5 years now even though I don’t actually have any ‘bad’ symptoms of menopause. What do you suggest? Thanks.

      Janice,
      This is precisely the kind of thing for which I do one-on-one consultations. You definitely need one. No two women are alike, and addressing your situation requires tailoring all the facts specifically to YOU. I can make the decision-making process very easy for you.
      I'm in the business of educating you about everything you need to know and do in order to get exactly what you want for managing your menopause your way. So, here's how a consultation works:
      1) You would schedule a one-on-one consultation at my website, MenopauseTaylor.ME, choosing a time that is convenient for you. I do consultations on Skype, FaceTime. Facebook Messenger, What’s App, and the phone.
      2) I would send you an email to confirm that you are scheduled for the stated time, and invite you to send me as much information about yourself as you please. You can make it brimming with details and tell me about your reproductive life, any gynecologic problems you've had, your symptoms of peri-menopause, and your symptoms of post-menopause. You can include all your fears, concerns, and goals for your menopause management. You may also send me any labs, mammogram results, bone density results, etc. that you would like me to include in designing your protocol. For instance, if you want to adhere to a certain category of management options (herbs or diet & lifestyle, or whatever), you should tell me that. I will give you a 5-day deadline for sending all materials, and will not accept anything after that date.
      3) I'll get busy creating a document that gives you all the education you need to understand absolutely everything, points out the pros and cons of each option, and tailors it all to YOU, using the information you have provided. The document will be loooooong. Most are over 30 pages.
      4) I will send you the document by email 72 hours before your consultation. You will benefit greatly by reading it in advance. Light bulbs will go off like crazy, and if you're like most women, you'll feel that you've already gotten your money's worth.
      5) During your consultation, we will review the document and address any issues you want in order to fine tune everything and determine your best options. I'll tell you exactly how to find the right kind of professional, how to have the conversation in order to get what you want, and the nuances of things like guidelines and insurance. I leave nothing unaddressed.
      6) You will go to a professional of your choosing in order to get the actual hormones or whatever you choose to use.
      7) After the first consultation, I'll hold your hand (forever if you want me to). In other words, you'll be able to schedule mini-consultations (15-minute or 30-minute, in addition to full consultations) whenever you need to. I'll keep records of your situation. Most women schedule mini-consultation whenever they go to the doctor and don't understand what he or she said, or why. I translate. I make sense of everything. Most schedule mini-consultation whenever they have a new test and they want me to explain the results. And all schedule mini-consultation whenever their situation changes and they need to re-weigh and re-balance their options.
      The time you spend with me will make everything about working with any other professional so easy. And they will love the fact that you have an actual education rather than a bunch of misconceptions that you got from the Internet. If your initial hormone regimen is not right, you'll want to consult with me before going to your doctor. I can prepare you for what to expect and what to request.
      So, that's my role. And nobody else on earth does this kind of thing but me. So, I hope you schedule a consultation and let me hold your hand (figuratively).

For example, should I request a bone-density test; blood work from my doctor and send that information to you to make an assessment or are there other test results that you require to make this assessment for me specifically? Thanks for any additional information you can provide.

    Hi Dr. Barbie !!
    I’m so thankful for all the priceless info you provide ! I’ve been binge watching all your videos in order as you asks us 😁. I’m 46, I have my uterus, my mother had uterus cancer and has osteoporosis. I’ve been having 20 of the 22 symptoms of menopause while taking birth control. I finally got my doctor to prescribe me estradiol 1mg. (He did blood work that showed low estrogen) how much progesterone do I need ? He prescribed me prometrium 200 mg and told me I could take 1 or 2 pills at night. What dose in your opinion is better to balance with the 1 mg estradiol ?

    • @MenopauseTaylor
      @MenopauseTaylor  6 лет назад +4

      It all depends on which "regimen" you use. If you use the continuous regimen & take both the estradiol AND the progesterone every single day you need 100 mg of progesterone every day.
      If you use the cyclic regimen and only take progesterone on certain days, you need 200 mg of progesterone on the days you take it.
      2 tablets of Prometrium 200 mg (400 mg total) will probably make you feel terrible! You'll have side effects like acne weight gain, bloating, severe drowsiness, and depression.
      Here are the dosages for estrogen and progesterone that meet the goals of preventing long-term disease associated with menopause.
      Conjugated Estrogen 0.625 mg / day
      Estradiol 1.0 mg / day
      Estradiol Patches 0.5 mg / day
      Estropipate 1.25 mg / day Esterified Estrogen 0.625 mg / day
      Ethinyl Estradiol 0.01 - 0.015 mg / day depending on the source of the estrogen
      Compounded Bi-Est or Tri-Est 2.5 mg / day
      There are standard dosages of progesterone to balance these dosages of estrogen, depending on whether you take the progesterone every day (continuous) or just certain days (cyclic).
      Here are the standard dosages:
      Medroxyprogesterne Acetate 2.5 mg continuous or 5 mg cyclical
      Norethindrone Acetate 2.5 mg continuous or 5 mg cyclical
      Norethindrone 0.35 mg continuous or 0.7 mg cyclical
      Micronized Progesterone 100 mg continuous or 200 mg cyclical
      Progesterone Gel Twice weekly continuous or every other day for 12 days cyclical
      Oh, and the younger you are, the higher the dosage you'll need. If you're just peri-menopausal, you probably WILL NOT be relieved of your symptoms with dosages designed for women much older than you are. That's why birth control options are so popular for women during peri-menopause. Birth control dosages of estrogen and progesterone are much higher than the ones in HRT.

    • @Cristina-qn2in
      @Cristina-qn2in 6 лет назад +3

      Thank you so much for getting back with me Dr. Barbie !! So for sure I don't need to be taking more than 200 mg. of the prometrium! i don't need any more issues! ;)
      Just two months ago (before I knew you) I was still taking Viorele birth control pills (I stopped taking them because being misinformed I figured on my own they were causing melasma on my face, which at this point i don't care anymore! but also i figured they were "evil" so i decided to stop taking them ! i think i went crazy since then.... that's when i found you !! and i watched all your videos in one week !!!
      This is what the Viorele birth control pills contain:
      Desogestrel and Ethinyl Estradiol Tablets UPS,0.15 mg/0.02 mg and Ethinyl Estradiol Tablets USP, 0.01 mg.)
      I read on the description paper they give you at the pharmacy that Desogestrel is some form of progesterone that is used to treat menopausal symptoms so I am guessing that's why my OB prescribed this particular BC pill? But I thought 1mg. of estradiol was more than 0.15 mg. than the birth control pills contain??
      I am very sorry if I'm asking stupid questions Dr. Barbie ! Should i stay on the birth control pills till I turn 50 or 55 then ? Will staying on BC pills help me better with the symptoms of menopause? more than the 1mg. estradiol/200 mg. prometrium at this point?
      Thank you sooo much for your time !!!

    • @MenopauseTaylor
      @MenopauseTaylor  6 лет назад +3

      You can't compare the estrogen in your birth control directly with the estrogen in your HRT. ETHINYL estradiol is not the same as plain estradiol. Your birth control pills have a higher dosage of estrogen than your HRT does. They will definitely do a better job at alleviating your symptoms of menopause, both short-term and long-term.
      You are YOUNG. Your body requires more than it would if you were t55.
      I think your doctor was on the right track in putting on the birth control pill. Should you decide to stay on it, it will take care of ALL our symptoms of peri-menopause, menopause, and prevent the diseases associated with menopause. If you were happy on it, why change anything? I always say, "don't dissect a rose."

    • @MenopauseTaylor
      @MenopauseTaylor  2 года назад +1

      No, my dear. That is precisely what progesterone is supposed to do to you!
      Pro = in support of
      Gest = gestation (pregnancy)
      One = hormone
      Progesterone is the hormone in support of pregnancy. Once you're pregnant, you are nothing but an incubator (according to Mother Nature, anyway). Progesterone makes you fell like you don't want to do anything by lie around and do nothing.
      Insomnia, though is a symptom of estrogen deficiency. Progesterone makes you drowsy.

      5) During your consultation, we will review the document and address any issues you want to fine tune everything and determine your best options. I'll tell you exactly how to find the right kind of professional, how to have the conversation in order to get what you want, and the nuances of things like guidelines and insurance.
Thank you for your videos! Life altering info for me ❤- Is the only difference in the vaginal Prometrium vs. oral is that it doesn’t tax the liver, or contribute to as much bloating? I am taking .1 mg of estradiol patch twice weekly change out/ and 100 mg of Progesterone capsule (for only 12 days- cyclic) I would definitely prefer to stay away from pills and protect my liver at all costs, but my Gyno could only suggest the gel which my insurance won’t cover 😞

    • @MenopauseTaylor
      @MenopauseTaylor  8 месяцев назад

      There is only one difference between oral versus non-oral hormones (all of them):
      Oral estrogen, progesterone, and testosterone travel through your digestive tract (part of which is your live) in order to get into your blood stream.
      Non-oral hormones do not.
      But, beyond that. the key is to understand that the only consequence of traveling through your liver is a slightly higher risk of a blood clot.
      And the key is to focus on whether or not YOU are at higher risk of a blood clot in the first place.
      The risks for a blood clot are as follows:
      1. Pregnancy
      2. A genetic mutation for blood clots
      3. A history of a previous blood clot, especially a pulmonary embolus
      4. Morbid obesity
      5. Immobility
      6. Smoking
      But most women don't worry about a blood clot when they should. PREGNANCY is when you should worry about a blood clot!
      Worry about those risks that pertain to YOU, personally, not about what you're read of heard. This is why I urge you to have a consultation with me. Otherwise, you focus on all the wrong things and make all the wrong decisions for risks that you don't even have.

    • @MenopauseTaylor
      @MenopauseTaylor  7 лет назад +2

      I think they can. Estring only provides estrogen to your vagina, not your whole body. It benefits nothing but your vagina (not your bones, brain, or heart). Mirena provides birth control and progesterone. So have to see if you still have symptoms of menopause or want to take anything to provide the long-term benefits of estrogen, including prevention of heart attack, osteoporosis, and Alzheimer's Disease.

    Hi Dr Taylor, I have a question but am embarrassed to ask. But I’ll try… I (just) started peri-menopausal HRT with my OBGYN starting with Estradiol 1 mg capsules and Progesterone 200 mg capsules prescribed, with my insurance company asking if my Dr can prescribe Medroxyprogestetone Acetate instead of the Progesterone.
    I’m working my way through the earlier videos, so this glitch came as something out of my understanding realm so far. I trust my Dr will do the right thing, and my insurance will cover it, too. But if you have the time, can you specify any basic differences between these two? My brother, a pharmacist, says “they do the same thing,” but I have a feeling there may be differences, and possibly important ones.
    Thank you for your videos being in order. They are going to end up being life savers for me🦋🌸🦋!!

Do progesterone suppositories have the same messy discharge issues as the gel? If you are using intravaginal estrogen 2x per week, could you also use progesterone suppositories on the off days? Shout out to vaginal estrogen: Woot-woot! The Lulu and everything else down there is very happy since I started it. Lastly, how long do you need to wait to have sex after inserting either progesterone or estrogen vaginal suppositories?

    My name is Ruth and I'm from Argentina LOL! Sos realmente encantadora, inteligente y divertida!!!
    I have a question for you: I'm already 50 (!?@), perimenopause since 44. My mother had a very serious breast cancer en her 50's. I always was afraid it could happen to me too... Anyways, I do want to prevent osteoporosis and heart attack. So I was thinking about taking herbal estrogens and progesterone. I wondering if the following products could be a good option for me: Natural Factors, Womensense, Vitex Chasteberry Extract (containing 80mg chasteberry) and Planetary Herbals, Full Spectrum Soy 1000, 1000mg.
    Do you think these doses can cover the necessary daily needs without being dangerous?

      You are very sweet. Most women who have been on birth control pills until that age of 55 switch to HRT when they stop taking the birth control. The HRT is lower in dosage than birth control; pills, and they've usually gotten to the point where they're well into post-menopause and do very well on the HRT.
      If you take the necessary dosage of HRT, you'll prevent any symptoms of post-menopause and prevent the diseases associated with menopause.

      I hope to meet you soon!

      The word, "safe" is not appropriate Everything, absolutely everything, has both pros and cons. The key to whether or not anything is "safe" is the woman using it. So, I would need to tailor everything specifically to your friend in order to answer this question.

    Thank you Menopause Barbie for this series of videos. I am learning so much!! I had a question regarding Prometrium vaginal suppository. Does it also protect against uterine cancer? I am interested in Prometrium vaginal suppository because it doesn't list side effects of depression or increase chance of blood clots and stroke

    Hi! I took your advice and watched some of the other videos. Thanks It did help. What are your thoughts on taking Provera pills x 10 days for endometrial hyperplasia with polyps after being into menopause approx 7 years? Biopsy was negative for cancer. So GYN wants to try that first. And will it cause the excess lining to shed?? He does not want to do a hysterectomy unless necessary. I would really appreciate your input. Thanks !

    Firstly, thank you so very much for all your very important information that you give freely. The question I have is about the type of progesterone put in a patch with oestrogen. I currently have a patch witches by a company called a Estalis 50/140 The progesterone component is called norethisterone. Wondering what studies if any has been done on the long-term use of this type of progesterone thank you in advance

    • @MenopauseTaylor

      What matters is how a given progestin works FOR YOU personally. If a study didn't study you, it doesn't matter. But all the progestins are equivalent for long-term usage.

      Qlaira is an oral contraceptive.
      It's perfect for you because you are so young. The younger you are, the higher the dosages of hormones you need. And birth control pills have higher dosages than HRT for menopause.
      Stay on your birth control pills for as long as you possibly can, hopefully until you're 55. You should watch video 41, 42, and 43. They will help you really understand all this.

    1st time watching your channel wonderful information on the Mirena I had one put in at the age of 45 due to my periods where too heavy and lasted from 15 day to 20 and start again it was such a relief to have the Mirena over time my periods became light and nonexistent by the time I turned 48 1/2 when I turned 50 I had the Mirena taken out and 6 months later my period came back with a vengeance the doc said the lining of the uterus was thick decided a biopsy should be done to rule out cancer no cancer so they said to try the Medroxyprogrestrone 10 mg for 10 days the 2nd day the period stop 3rd period came back heavy again not as bad but still heavy. Should I go back on the Mirena?

    You look awesome in your pink!I read and heard that progestin is very dangerous. (the Climara Pro patch)
    I am now interested in the Progesterone vaginal gel after hearing this video which I never heard fo before. I think the Progesterone cream I am on is giving me diarrhea. Yes I take a high quality probiotic every day and I had a colonoscopy. Plus I drink Kavita probiotic drink every day.
    I am just super sensitive, This gel sounds great! Does it help with frequent yeast infections?
    Me and my hubby are watching together now and he said he is learning a lot too!
    Do you haver an opinion on the Climara pro patch? I thought Progestin was dangerous and not real Progesterone? I am on the regular Climara patch, and using compounded Progesterone cream.
    Thank you for your hard work and dedication!

      You probably heard that "progestin is dangerous from someone in the alternative and complementary community. That's because they sell progesterone. So, by telling you that progestin is dangerous, they sell more progesterone.
      This is marketing (and fibbing). It's not factual.
      Here are the facts:
      The one and only reason any woman needs progesterone is to protect her uterus from uterine cancer. For this purpose, progestins are much better.
      Progesterone is better for your heart and breast, but not your uterus.
      You see, there are advantages AND disadvantages to absolutely everything. I tell you the truth. I don't give you hype, scare tactics, or marketing.
