- There were 2 main fails during the flight - open carabineer and flying without a helmet.
Carabineer wasn't old according to its specification. The reason why it bended is that, carabiner was not closed before take off. Pilot checked carabiner visually, unfortunately it was frozen, it looked sealed but it was open.
1st Mistake: Preflight check should be done more carefully.
2nd Mistake: Pilot should have noticed bent carabineer and should have stopped making maneuvers.
3rd Mistake: Pilot wasn't supposed to risk his life and should have worn a helmet.
We admit our hard mistake and share this video to you for experience.
Always make preflight check carefully!
Always fly with a helmet!

Possibly company uploaded video for analysis and not for commercial uses. By the way, cases when carabineer stays opened because it is frozen, isn't rear. But there isn't any company or pilot who uploads video like this, admit and publish there mistake.
- Yes, pilot was supposed to make preflight check more carefully. It was his mistake.
Before you get pulled, please, take in account that company didn't hide it's pilot's mistake because they wanted to share this experience.

How important is a preflight check when your pilot is blind during the flight? Common dude, be fair, the problem was during the whole flight in front of his nose. This video shows only that this pilot should use his license as toilet paper.

Flying without reserve also NOT OK!
Flying solo without one is plain stupid. Flying tandem without one should be straight to jail. Especially if you cannot do pre-flight check properly like that cookie in the video.
Agreed! Good thing that karabiner performed so well and didn't break since, amongst his other bad choices, this bad pilot also chose to fly tandem without a reserve parachute - so he and his passenger had no backup whatsoever. FFS! They are very, very lucky to be alive, despite this pilot's very bad airmanship.

There are much more mistakes than you mentioned :
- Chest clip of passenger harness is absent, he could get out of it from above;
- Passenger helmet is too loose, effectively not clipped;
- Pilot hands-off brakes right after take-off, too close to ground;
- Passenger never gets back-installed, then compromising his abs, that he will probably need for running at landing, as he isn't in skis (!); Moreover his weight is heavily supported by legs clips, during spirals;
- Pilot makes long series of maneuvers without asking passenger if all going good, and judging by his face, it wasn't;
- If he would have seen the bent carabiner in flight, it is not just to stop maneuvers, throwing reserve when still high is a better bet, because a final cut of the carabiner at low altitude would have been catastrophic; What is most, lots of snow in the ground !
- He did not visually check as you said, please look at 0:09 it's already open at single gaze.
Finally, and personally, I don't like the idea of relying only in passenger power for take-off, why pilot is in skis when passenger isn't !?
Still, can't believe it : @ 0:27 he checks carabiners in flight (glider + passenger) and did not see it !
What is on the very top error : from 2:10 and later on, we see he has not even a reserve parachute !!
@@LoucassOo Great input - tandem pilots, please take note! Being a tandem pilot is a hugely different thing to flying solo, requiring far more skill and attention. Being a great tandem pilot is all about taking care of your passenger, making sure they have a great time, from start to finish; it's not about showing off what a great pilot you think you are. The best tandem pilot of all is the one with the happiest passengers and best safety record.
@@LoucassOo it seems at times during the flight that the trimmer flapping around might have obstructed the view of the carabiner?
@@quoth_raven maybe, but it doesn't matter, it is not a justification and in any case it must be checked in preflight.

Translate to English: "The pilot made the decision never to fly on a commercial tandem flight after this failure." Most probably the right decision!
Frustrating though this video is to watch -- due to the extremely poor airmanship and general attitude demonstrated by this irresponsible tandem pilot, which reflects poorly on the whole freeflight community -- I thank you for sharing this incident so that others can learn from these careless mistakes, and hopefully avoid them. As with most -- almost all -- incidents like this, the real failure is with the pilot, not the equipment.

The problem here is not the karabiner - which performed amazingly well under the circumstances - it's the pilot, 100%.

Personally, I very much value the pilot's decision as in the description. Nevertheless, private tandem flights - and even single flights! - should be considered as well! Anyway, my respect for posting and decision!

No, they are sharing this highly embarrassing video so that others can learn from this pilots mistakes. Kudos to them for that, I think. Much better than trying to hide it, as some might have done, and others not learning from it. Or hearing some false rumour about why this happened e.g. blaming the equipment.

You have totally the wrong end of the stick here. The pilot is 100% to blame for this, not the equipment - which performed admirably under the circumstances. Considering the pilot first failed to properly close the karabiner before launch, then failed to notice the karabiner was open at any point during the flight, even when it was wide open and near breaking, and performed some wingovers - greatly increasing the strain on the open karabiner - the karabiner performed extremely well.

Agreed, it also seems the passenger isn't having much fun at all on this flight! He has his eyes closed or just stares at the camera to not look around most of the time. He really didn't need the pilot to start doing wingovers and spirals! Another sign of a bad tandem pilot.

As far as I know, pilot noticed bended carabineer only after landing. Possibly he wanted this video for analysis and for giving us an example of how important the preflight check is.

He didn’t know until after the video stopped. He pulled some more G moves after 4-57. Can’t see a reserve? Lucky fellas 😬

A great advertisement of gross negligence. Obviously, no carabiner check during pre-flight. Also, pilot never noticed the almost-broken carabiner or he would not have been pulling oscillations on set up for final. Don't hire this company if you value your life.
