I think they are similar to Kiba's clan. Kiba and his family have pronounced canine teeth, and heightened sense of smell.
The Hoshigake clan are likely more "evolved" versions of that. I do not think it is a sage transformation due to the fact Kisame is weaker than Itachi.
Human physiology isnt consistent in Naruto. We have people that sword fight with their spines, people that have hollow voids in their body for swarms insects to live in. It isnt too extreme for there to be a grey skinned dude with gills.

This is an example of how great kishimoto's lore and storytelling can be when he isn't forced into a generic shounen plot by editors. Itachi and Kisame weren't omnipotent gods here and were almost taken out by farmers who planned very well. That's how Naruto should've been the whole time.

One thing you missed however is that obito put kisame on itachi team, because he was the perfect counter, water style user, can take chakra and make himself stronger, reports directly to obito. has a knack for killing his teammates with no person ever being able to sway him. kisame might be one of the few people who could not be talked no jutsu. kisame questions him the whole time in part one on why they dont just fight the other ninja, itachi is trying to prevent a massacre. in fact kisame probably looking down on tobi in the after life.

Only hole in this is that the sword don’t utilize chakra, it takes it away, so it might not even be able to disrupt due to not knowing how to… by utilize I mean he can’t use chakra he can only expand due to the volume of chakra being taken.

The Dangerous Duo, and definitely two characters in my top ten favorite of the franchise. Their level of devotion and loyalty is something to aspire to, in my opinion.
Kisame, being sad after finding out Itachi died, especially due to it being against Sasuke; although he likely expected that from that encounter of theirs. He was tasked, and agreed, with no hesitation, to stop anyone else from tagging along with Sasuke to that fight.
Then when Itachi is reanimated in the 4th Great Ninja War, only to see Bee wielding Samehada now; realizing that meant Kisame was gone too. The sad, melancholic look on Itachi's face after seeing that and realizing his old best bro in arms, has passed on too. 😔 They definitely had been through a lot alongside each other, and bonded significantly due to that. 💯

Man, idk why because I already knew. But for some reason after watching this I wish Edo Itachi and Kisame would’ve gotten an opportunity to talk to each other and or at least acknowledge each other as friends like someone special to each other. I think that would’ve been a pretty cool send off especially for Kisame who wasn’t evil per’se he was kinda a victim of circumstances honestly. Just like most of the Akatsuki. They were all for the most part taken advantage of from something major affecting their lives. Even Obito! Kisames loyalty is something of a breath of fresh air, per all the betrayals/ lack of restraint and ulterior motives that usually stem from characters with unruly tendencies. Especially after the amount of betrayal he had faced in his own village. He wasn't my favorite Akatsuki, mines actually kakuzu, itachi being a close second, but my respect for Kisame is definitely higher then most characters for that very reason. He was also up there as far as power goes, I'd say he is probably physically stronger then Itachi and had much more in the tank as far as chakra reserves is concerned.

Big problem with sharks using nature energies, is they have to sit still. which they literally cant, most sharks have to constantly be swimming. Also sage mode can only be done by mixing sage energy with chakra. also have to stay still. His sword takes chakra, in fact you could make a case that if the sword took sage chakra that it, it self could be turned to stone. furthermore the sword uses chakra it gets, to change it shape or from kisame himself, without a wielder the sword would be practically useless.

my theory on why kisame looks like a shark is that he naturally absorbs nature energy for some reason. we know that if you are unskilled at handling nature energy you will start to adopt features of animals and we also know that people with more chakra are better suited to handling nature energy without turning to stone. so my theory is that kisame naturally absorbs it for some reason possibly a genetic mutation however because of his massive chakra pool he does not turn to stone despite not being trained in how to handle it at all.

Itachi to me at least comes off as someone who still feels and can care about people, it makes since he would care somewhat about a good combat partner who was comparable to himself and trusted him to the extent Kisame did. I'm also still supporting he(Kisame) had some involvement with sage/nature chakra based on his ability and chakra reserves, his appearance shark-like from the start similar to how jugo alters, while kisame having better natural ability in chakra control. His appearance similar to his nature form as Jiraiya was due to the imperfect mix and again his control level being lesser making him much more shark-like vs the better control shown by the legendary sannin. The lesser control going with his attacks being more powerful/chakra heavy thus needing more and more nature chakra and bringing about the shark form. Maybe he didn't even know it but I believe he was sage similar to kabuto and that whole ordeal, he was just a natural example who didn't particularly train at mastering sage things as directly as other examples.

A good theory i have about Kiri swords, Samehada and the Hoshigaki, is that Byakuren (the 1th Mizukage) was the one who forged the swords, and used the scale of a giant dead shark that once terrorized Kiri to craft the weapon. The first Hoshigaki could have been someone that had another scale from that shark implented into his body (like what Ame did to Hanzo with the dead Salamander poison sack) , ence why Kisame could fuse with it when Bee and Fuguki doesnt seemed to be able to

Of all of the Akatsuki, Kisame’s death hit me the most for sone reason. Dude is first absolutely loyal to his village - has to kill a girl that’s potentially interested in him and finds out it was for naught since he was betrayed by his superior.
So he kills said superior, takes Samehada and joins the akatsuki since, they wanna control the earth with fear? At least there won’t be that much war after it anymore.
Kinda forms a friendly relationship with itachi. Not Naruto-Sasuke like, but i’d argue they didn’t hate each other.
Then Itachi dies and Kisame is somewhat alone again.
Then Samehada betrays him for some Octussy.
Then he gets captured and, rather than ratting out his “Team” aka the Akatsuki, he commits suicide.
Dude was called a tailless tailed beast, got used and thrown away, tried to find his own path and was loyal to his comrades.
Like, if you rewatch part one naruto, that’s basically jiraiyas definition of a good “ninja” - his death was just super sad for me

When you say that he’s frothing at the mouth to fight kakashi, I felt you didn’t contextualize that enough. He has such an intense desire to fight kakashi because kakashi defeated a fellow member of the 7 swordsmen, zabuza. And with the idea that maybe Kisame might have been in on the coup for the hidden mist adds another layer of loyalty to that situation, just another moment to prove how fiercely loyal kisame is.

My head cannon is that kisame is just a shark that gained chakra and is just using transformation jutsu to appear more human. Thats why his transfromed state is more shark like and why he gets water sharks when using the clone seal in his shark jutsus
