    I don't understand, if a patient has jaundice, and has a palpable gallbladder,it's unlikely a gallstone. But the exception is that it is a gallstone? Like I don't understand how it's so contradictory

      The exceptions are that if the stone is in the CBD then there’s no fibrosis and contraction of the gallbladder so then the gallbladder can dilate.
      Another way is if there’s two stones- one in the gallbladder and the other in the CBD, the one in gallbladder when blocked in the Hartman’s pouch cause an inflammation and swelling which cause the gallbladder to be palpable and the one in the CBD cause the ducts to dilate.
      And lastly a condition called mirizzi syndrome, where the cystic duct is short therefore a stone in the cystic duct cause the gallbladder to dilate and the ducts to dilate.
      In general, if it’s non palpable it’s a gallstone but these are a few exceptions depending on where the stone is and the anatomical differences of the duct system.
