May i ask a question? Does the question characteristic not matter? As an example, first question asking about "negative effect on performance at work", then second question is about "is the air fresh or stale?". Does it not change the severity index?

how about negative items? should I reverse the code? For example, strongly disagree will times 5 instead of strongly agree. is it that way?

This is just the mean divided by 5, so now it has a range from 0.2 to 1. It should still have the same interpretation as the mean. If this formula measures importance, so should the mean. So you can just use the mean instead of RII to get the importance rank.
