And the only transformation he's used in the past offscreen at some point was his second form. It's never explained what caused him to go to his second form. I suppose it could be argued that Cold was aware of Freeza's true power, given it's presumably one of the reasons he handed down his empire to his son.

This guy here is probably another person from Malaka's race and looks like a communication officer or computer specialist, not a medical doctor because this room is a central communication center of the empire, not a hospital.

Between this and Zarbon's scouter, it seems theirs some sort of glitch in the way their tech works which makes it explode when it should just not pick up anything understandable, if not overheat a little bit and shut down.
Little does Goku know that he killed about a hundred or more of Frieza’s goons by simply overloading their computer with his power level to the point it exploded killing everyone in the room even when they tried to escape. I doubt any of those guys survived the resulting explosion.
