There is two versions of the pilot, as the TARDIS scene was recorded twice. One version is on The Hartnell Years, the other was transmitted August Bank Holiday in 1991.
This wasn't just a test run. It was supposed to be the btoadcast version. But for a myriad of reasons (a few of which you addressed) Sidney Newman ordered a remount which was the version aired on 23/11/63. The "thunderclap" heard in the opening titles was supposed to be part of the main theme but was dropped before the remounted version was recorded. Fluffed lines were commonplace on television at that time due to costs and time constraints in reshooting a scene or because a lot programmes were broadcast live (Z Cars for example) and mistakes went out on air. So Dr Who was no exception as far as that went. Newman had dinner with Verity Lambert and Waris Hussein after viewing the 'pilot". He told them the issues he had with it and you know the rest.
I forgot to say, I have had Great Fun Colourizing a number of the Stories. I just finished the Second Doctor's (Patrick Troughton) last story, War Games. It looks great in colour.

They were Ordered to do a Remake because of the mistakes. Here are just two, Susan fluffing her line about the hit parade and Barbara tripping over the door-frame. In the Unaired Version in the Junk Yard right at the start, the TARDIS looked like new. I had great fun finding out all the differences between to two Versions.
There were a few. Not sure I caught them all.
