I’m gonna comment to hope I can help someone. In my case I cured 90% of my sciatic symptoms with the combination of both a lower lumbar support pad and coccyx cushion pad. Now I can drive without hip pain. Changed my life. Hope this helps someone.
I recommend you a pillow. It's from Kebado. I purchased it as a gift for my wife. She spends at least two hours on the road, driving to work and back. She told me that this pillow has made a big difference in her comfort level. Her back is a lot better on the long drives and she loves it. I tried it a bit and it's ok from my point of view... I'm not spending too much sitting.

I'm fairly tall and my knees are way higher than my hips when sitting in my car and I get back pain all the time from this. The saloon car I drive is too limited in terms of seat adjustment to counteract this. I tried a cushion on the seat which helps a bit, but my knees are still way higher than my hips. The only solution I can think of is to put an extra couple of cushions on the seat to build the seat height up by at least another 4-6 inches. It seems most car manufacturers are not addressing this problem sufficiently. Of course, they'll tell you that the seat is easily adjustable and the back of it tilts up and down, but why is it that my knees are always way above my hips?? I know you mention moving the seat forward but this will make my knees even higher than they are at the moment.

The horizontal seat is also for thigh support, but when it comes to your low back, it help tilt your pelvis forward, reducing pressure on your low back. You will also find that this position will reduce your hamstring tension
This is great and what I have been searching for... EXCEPT... there are not any cars out there anymore where you can set the lower part of the seat flat (let alone tilted a little down in the front). Dr. Sabharwal, that Lexus you are in will not go flat as I have tried every Lexus. Every seat made these days will always put your lower back in a hole and put all of your upper body weight on you lower back. Audi A8 with the Q2J comfort seat option from 04-10 have a huge amount of adjustment. They allow you to also move the front of the seat edge towards your knees so the weight is spread out more onto your legs than just your butt. The pressure difference from extended to retracted is a lot. Try one out sometime. If you're looking online look for the seats with more switches.

I'm having to relax sitting back a bit. Also wondered if there was any advice on seat bladder. It feels best when I have the bladder off but just wanted to ask. Spinal fusion L2-L-5. Unfortunately what your describing here, would require the most expensive model car. That's the only way we get power seats, that would give us these seat adjustments. Otherwise we can adjust seat back, and possibly seat height. Move the seat back and forth. I bought a lumbar pillow, that's about all I could do with a base model car.
I find if my legs are parallel to the floor or closer, that increases leg and sciatic pain. What if you move the entire seat pan and back at the same time? Which does not decrease the hip angle. Thanks

I don't get it. Look at the angle of your neck vertebrae to the rest of your spine. It's ridiculous. Imagine walking around like that. If I drive at that level of recline I get terrible headaches. My chiro agrees with me. I use an inversion table every day and a sit-stand desk so I know my spine and how to take care of it. Modern car and aeroplane seats are the antithesis of ergonomic design. If I set my seat back angle more appropriately (I don't mean vertical) then the headrests in most cars force my neck further forward and down. When I fly I take a lumber bolster (as I do driving). I don't recline the seat at all even on an 8-hour flight. I feel far more refreshed at the other end than if I sit in the seat the way the airlines would have you do.
Modern car seats are simply designed to set a crash dummy's rear in a hole so it won't move when they do the crash tests, and therefore the rating is good. I'm not arguing your point about the neck, but sitting your bottom in a hole puts all of your weight on your lower back and causes back pain for many people. Do an quick Internet search on "wedge cushions" that are designed to help get the seat back to a more neutral position. Sales of these have gone crazy the last few years with the latest horrific design of the car seats. I have driven several new cars recently and everyone gives me sciatic nerve pain... I get back into my 2011 GMC Acadia that sits flat and my back is fine. Trouble is I need a newer vehicle because the GMC is slowly falling apart.

I would place a small couch pillow on the seat, this eliminate the dip in your car seat and take pressure off your hamstrings.

Sucks honestly. You can't achieve a good angle without lifting the seat to a certain degree.

The horizontal seat is also for thigh support, but when it comes to your low back, it help tilt your pelvis forward, reducing pressure on your low back. You will also find that this position will reduce your hamstring tension
