I’m taking this idea as a way that THIS is why he came out of Mikey’s portal unharmed. I mean, he’s the medic. With this theory in mind, he probably found a way to patch himself up and he healed while he was there. There was probably nothing else to do other than let his body heal from those injuries. Well, except avoiding getting new ones, which he did a few times. It’s why he was a little banged up when he was taken out of the portal. He kept getting into fights over the course of the year, just the fight that they saved him from as they opened the portal wasn’t THE fight. It was a petty fight. And this conversation happens a few days after they get him back

I wonder if sometimes, now that he’s back with his brothers, he forgets how to be the version of him that they remember. Like he tries his best to be that person, which isn’t hard since I doubt his coping skills changed all that much, but sometimes I can see him getting way too serious and everyone looking at him surprised before saying something stupid to counteract it and hoping he was able to play it off. And maybe that’s what tipped Donnie off that something was wrong and got him analyzing Leo more. The identity crisis is real

A year worth of sound passing in 5-10 minutes. It would’ve probably sounded like white noise. So realistically, I think Leo might’ve been in there for probably an hour or two is PD time. Idk, I’m stupid so I probably don’t make any sense.
Plus pain makes seconds feel like hours that and the perception of time could have changed drastically within as short as a minute. So there could be a possibility that every other second in the PD could’ve been equivalent to a second on Earth. But the perception could suddenly change and a day in the PD would equal a minute on Earth. Idk if there is a canonical time difference from Earth and the PD. If there is, tell me so I will know if I look stupid or not.
