This is because 'should' is also used by itself to make a suggestion - using your example, "You should ask your teacher". If you put both words in a present or future sentence it is like saying the same thing twice. "I suggested that you should ask your teacher" - in the past would be correct because you are reporting the suggestion that was previously made, not repeating the same thing. It is also fine to leave out 'should' altogether in a past statement - "I suggested that you ask your teacher". With American English they don't tend to use 'should' in this way.
