  • This video shows a gentleman with Hyperreflexia, a Positive Hoffman's test, Clonus and incoordination. If you see this during your evaluation, you need to be suspicious of a central cervical cord lesion(stenosis), head injury and occasionally MS to name a few. Let me know what you think of this video. Also feel free to thank these folks for showing the real signs and symptoms that often times you only read about in school.

    I started watching your videos because since 2013 my arms and legs have been getting progressively weaker. I finally found a doctor to take me seriously and she sent me for an MRI which showed spinal cord compression at C7 to 4mm. I had that herniated disc removed on August 15, 2022, but I am still having the same weakness in my arms and legs. Along with the weakness I have shortness of breath. What is the likelihood that the spinal cord issue was a coincidence and something else isn't going on?

    Hi Paul, Thanks for the video. How were you sure that it was a central cervical stenosis and not another pathology such as MS ? I see you referred to the neurosurgeon here and that you considered treatment based on PT methods.

      Follow up MRI showed significant c-spine stenosis.

    Can I ask what about a positive c7 makes you want to also check the lower extremities?

    Babinski should have been extensor response with other positive signs of upper motor neuron lesion. Is there any reason for plantar response?

    Is hyperreflexia serious ? Reason I’m asking is I’m 4plus in my lower extremities
    Have a large herniated disk at t8t9 but neurosurgeon said the surgery is to risky

      Lots of people have hyperefflexia. It's when you have it both in the UE's and LE's along with a positive Babinski, and clonus you should be concerned. Also, look out for LE incoordination and even bowel and bladder issues.

      Thanks for information they said hyperreflexia is coming from the herniated disk in thoracic spine as it become a new symptom in last month as herniated disk has grown
      I enjoyed watching your videos as it is been very educational to help me understand what’s happening with my degenerative spinal disease and also understand what doctors are saying about my condition

      Clonus has arrived was referred to a different neurosurgeon at university of Florida for decompressing my cord in the tspine

    I saw a positive Hoffman's on this man's right hand but not his left? Maybe it was the cam angle.

      You are correct. It is the camera angle. He was positive on both sides.

    Can u present some cases having neck pain without disc...having facet joint problem or some local cause which can be diagnosed clinically

    Hoffman's +ve and absent Babinski??? How?

      these are 2 completely independent signs

      Babinski indicates lower motor neuron lesion. The hoffmans sign can be from many things everything from simple anxiety disorder, severe kidney disease, multiple sclerosis and ALS

      I think Babinski is in favor of upper motor lesions
