It happened before the game, in blurbs GW released that were just several paragraphs long. However obviously we could get flashbacks that expand on it. Anyways, if you're interested here's what I recall from the blurbs if you don't mind a minor potential spoiler. Basically the Inquisitor that took him, put him in status between torture sessions. Over 100 years past before that Inquisitor fell to Chaos. Another Inquisitor found Titus a dozen of other captured and apparently honorable Space Marines that Inquisitor was torturing too. The new Inquisitor who found them put all those Space Marines into Death Watch, letting them be of service again but kinda letting them think they brought dishonor to their chapters, including Titus. They all took new names during this period. Anyways, they were sent to stop Tyranids, and a Carnifax kinda killed most of them. Titus was badly injured and by this point nearing the last survivor. When Ultramarine reinforcements arrived in system and this included Chief Librarian Tigurius who had meet Titus, obviously, before and sensed him. So they Tigurius redirected the Ultramarines to save Titus. Titus was the last survivor when they arrived and near death. Tigurius cleared him of being a heritage as he was previously charged as and to save his life they upgraded him to a Primaris. Thankfully it worked, Tigurius's words made sure he was accepted back but as you saw in the trailers not every marine was so accepting and some still had reservations. Tigurius wasn't able to figure out why Titus was nearly immune to Chaos and the Warp, only that he's not influenced by it or a dormant psyker. Hence the reservations. Also after Titus was taken Cato Sicarius became the new 2nd company Captain. Though Roboute Guilliman has since promoted him to being one of his personal guards. The new 2nd company Captain is Sevastus Acheran, which happened after Cato himself was lost in the Warp for over 5 years.
