As a person who has had SI joint pain for the past four months and has been struggling to find someone who can adjust the sacrum, this video is jampacked with just all kinds of fantastic information. I found this video three months ago and I’m re-watching it today. It really confirms everything that I have learned and been through in the past four months. I have found a good osteopathic doctor, good chiropractor, and good physical therapist who all understand the SI joints and know how to put the sacrum back into place. My old physical therapist and chiropractor did muscle energy technique and the typical back crack to get the sacrum to move, but none of those movements addressed the position of the sacrum itself and getting it unstuck. My muscles are also chronically tight and pulling on the sacrum, causing compression at all times. We will be addressing the muscles to loosen those up, which will then allow the pain to slow and loosen the other surrounding muscles, allow the ligaments to start to heal, and then strengthen and stabilize the area. I will come back in a little while and provide an update to this comment.

Can you describe current pain patterns you have? How severe and what triggers it? I also struggle with SI for 5 months now but there's no SI specialists I can even google even within 300km radius, I really consider flying to USA to visit dr Hesch, apparently an expert in this, but scared with my pains (sometimes can walk 4000-5000 steps, sometimes 200 and then pain)

For years i had reinjured it badly at least once per year. this talk and all the stretches are the only thing that provided a cure for that pain. believe this guy, follow his lead and you can take control of this . thank you very much sir your presentation is excellent!!!

I was looking for a second opinion before rushing into steroid injections and surgery. This is the video that sent me across the state to see Dr. McRoberts. This is exactly how he is. He is patient, thorough, and explained my MRI to me in a way that I could understand. Never rushed me and didn't encourage the use of steroids or other injections. He gave me exercises to do at home and a Rx for PT. He was so encouraging. And I really appreciate that he takes the more conservative approach of helping the body to heal itself before heading in to more agressive treatments. I can honestly say he relieved my fears of what was happening to me. If I ever have other spinal issues he will be the first doctor I see.

So last week at a year follow up dr appointment I suggested an injection to the SI joint so I'm still looking for answers.

This is a great find. I’ve learned more about my si joint dysfunction than I have in the past 12 years going to doctors, including why burning the nerves didn’t work. The doc tried three times, and then just said “there are a lot of nerves back there.” Pain doctors didn’t help and kept me on painkillers which lost effectiveness so I gave them up. My current primary says “there’s nothing more we can do,” but this shows more options for me to try. I’ll work on the bridges and the stretch, and now know where to go from there.

I also have had sacro-illiac problems since age 27. It was only two years ago at age 66 that I found a physical therapist that started me on the way to healing. First she felt the area and said the joint was twisted. She took her body and physically manipulated the tendon. Then they gave me exercises to begin rehabilitating the area. I also took yoga and pilates. Lately, I have had massage and acupuncture and at the moment I am pain freeafter 41 years of pain.

Sorry to hear this. Did you ever have relief after the surgery? I know that this seems to be the problem with fusions. They eventually cause problems above or below the fused area, and then you have to have another surgery. I was wondering how long of a time one gets relief from pain before another surgery is needed. Really sorry you are having this.

I developed my back and SI joint injury from respective stress at work. I used to work standing in one spot for 8-12 hours a day lifting heavy boxes. The middle of my back would burn and would go numb. My thighs felt like they were on fire. They would feel hot or cold to the touch and get numb. Then the SI joint became an issue. It was painful doing that job. It even messed up my nerves. I quit.
    I learned alot.

Wow. Doc, I just ran across this video by accident. I've been suffering with SI Joint issues for years now. And, it truly IS debilitating. I'm about to have another ablation. Last one I had lasted for quite a while. I'm about to the point of asking my neurosurgeon about fusion. Unfortunately, my whole spine is a mess. I had a multi-level lumbar laminectomy several years back due to extreme stenosis and am now losing some stability there, so looks like a multilevel lumbar fusion is the cards. I also just had a multilevel fusion of about all of my cervical vertebrae. I think I'm truly on the way to becoming a $6 Million man. Anyway, I haven't had it suggested to inject the lubricant into the joint. I believe I will ask about that next. This was a great video, and I really appreciate you posting it.

I wish this dr was on the west coast of Florida & not the east. Im near Tampa & hes in Ft Lauderdale. Also my insurance would have to cover seeing him. But the video didvgive me a lot of information I had not been given by any of the otthopedic drs Ive seen yet. This is a great video. I wonder whether you are able to assist with a couple of questions?
    1) is it possible to jump between stage 2 and 3 without the assistance of an osteopath or alike to realign the SI joint? I find that I can have some awful days unable to take a step or turn without SI pain and others where I can get by with just a dull ache (which feels like it's ready to jump to Stage 3 any second).
    2) I have had Thai massage for 10 years but only just started having the SI joint pain recently, have been told I have anterior pelvic tilt in the past though and I think you're video suggested Asian massage linking to posterior tilt only. Could it still be the cause?
    3) whilst doing glute bridge, I get mild pain/discomfort in my SI area. Is that an indicator that it is helping or should I avoid any exercise that causes more discomfort than sat doing nothing (I often just get dull ache then but glute bridges intensify that).
    Thanks in advance

If it is possible for the SI joint to be out of place, how does a surgeon make sure it is correctly aligned before fusion? Also, the pigeon pose (Yoga) that some people recommend for SI Joint relief would be moving the ilium in the opposite direction it should be going, right? I get some relief by doing the opposite. Forcing my knee behind me, hips as far out in front of my knee as possible. Great explanation of this painful condition. Thanks.

Fantastic!! Been watching various utube vids on low back pain and SI pain from Chiropractors. But this explains it so much better. Having rodeoed professionally, ive used DCs for many years. Most really helped various pains. But, old age has resulted in a very painful hip that i just feel is from lower back or hip muscles rather than actual hip problem. This was a great explanation of how things work together. Also wathed your utube on lower back probs. I wish you could demonstrate on yiur table what not to do and what to do when sleeping. Same for the hamstring stretch. The language is a bit confusing ie, "throwing leg over", "sleeping on side with knees not together", etc. And for hammy stretch, "back in extension" does that mean concave or convex?

I’m asking how will I know if it is the si joint if I’m still not having pain except for when I get in and out of car, going up stairs and standing too long. Again excellent! Thank you. What if you have pain in the area you are describing and sciatica. 24/7 . And, for 3 years non-stop. I have also been diagnosed with “severe facet degeneration of mid and lower spine” now. Two years ago mild. Haven’t been able to exercise due to this constant pain in left butt. 6 months PT- did not help. Certain exercise made butt pain to intense so stop trying to strengthen butt muscles. Over 60, and last year I can hardly walk when I get up in morning. Legs so hard to move. Gets better after 2 hours but pain in butt is so bad I have to sit on ice and heating pad 24/7. Go grocery shopping and very limited errandS. Can no longer do my 2 mile walks that I use to do everyday. It came on gradually after I prevented my mother from falling by holding her up when she passed out. Not sure if that caused it. Been to spinal surgeon. 3 years 3 MRI, XRAYS. Last MRI a 3 months ago ruled out stenosis, but have mild disc disease, last Xrays 3 months ago showed that Facet degeneration had gone from mild to severe over last 2 years. Please can you tell me what you think the non-stop pain in left butt in area you describe is? In this video, sacroiliac joint pain comes and goes when you move it out of place. Right? Not, constant. I had 3 epidurals 2 1/2 years ago: L3/4; L5 S1; Pimaformis. None worked. I just finished 4 weeks PT - did not help, and because I can’tdo exercises to tighten/ strengthen butt muscles they said I need to go back to the doctor. I am in the process of seeing another spinal surgeon. Before I could see him I had to have XRays, MRI, PT. -- should I insist on CT scan or is that necessary? And, do you think I have a problem with sacroiliac? Thank you kindly. I am scared I will end up like your mother. My physical condition is going down hill each year the last 3 and still haven’t been diagnosed as far as the left buttocks pain and sciatica left side. Would facet degeneration cause this. Usually, the pain in my lower back from it is more of a burning pain. Your opinion would be greatly appreciated. I wish I lived closer to you. I would come to you. I live on West coast. You are so good! So kind of you to do these videos. I have watched others of yours.

Excellent video. a Quick question. after years with lower back pain i had surgery 3 months ago to install a device from Premia Spine called the TOPS device at L4 L5. . This device is a newer, in the US, alternative to a spinal fusion. Some of the pain was relieved but the on going chronic back pain is still mostly there. On my last visit with the Doc, he tried pushing the points you mentioned in the hip. Some pain when he pressed the right side but left side was off the chart. Now he wants to do injections, therapy etc. In addition to the pain, something is causing a buzzing sensation. It happens ~3 days out of 5 and always when I wake up in the morning. Its either a buzzing or non buzzing day. My question is, is it possible the injections will turn off the buzzing sensation; even temporarily ?
    Thanks Glenn, SC

Dr McRoberts Thank you for taking the time to explain everything we needed to know about the S-I joint!! Do you know a doctor that you could recommend in the San Antonio or Austin area that has a practice like yours?. I have osteoarthritis & bone spurs in my S-I joints & getting the injection you described Synsec (sp)? or rooster juice is much more appealing than the fusion I have been recommended. Thanks again.😊

Try Dr. Kancherla: He is an interventional physiatrist specializing in the non-operative management of low back and neck pain, focusing on individualized treatment plans and therapy in Austin. I've heard good things, but don't know personally.

Pelvic instability is so bad that I'm using a wheelchair. It buckles with the smallest movement.
It feels like I'm on a boat just sitting.
Car accidents (tboned 2x), falls. It buckles and I fall. Usually on my butt. That's why I use a wheelchair.
Stabbing pain used to cause my legs to buckle making me fall. I decided to use a wheelchair to avoid the pain and falls.
I've done a year of PT, pain shots, imaging. And the doctor who was going to do ifuse just left the hospital.
So I'm waiting for a different neurosurgeon to evaluate and schedule.
It's like my upper half and lower half work like normal independently - but they don't feel connected in the hips.

Ive been suffering from this horrible pain for 14 years. All my Mris and X rays come out clean. I just want this godawful pain to go away. It's robbed me of my 20's, a mans prime. My injury happened after lifting something. Life is too short for me to live it this way. Its 3am and cant sleep 😔. My mental strenght to go hold on is chipping away.

Have you been to an Orthopaedic Doctor? I go in July to ck out my options. I TOTALLY UNDERSTAND about quality of life. I've been living like that for over 15 yrs. Gets old!!! I had to pursue things myself and totally bypass referrals.
