Drugs given through subcutaneous route are absorbed through capillaries or lymphatic vessels. So, it's supposed to be much slower than when you give it through an IV. Massaging the injection site can increase the distribution of the drugs. Say for example you're giving a long acting insulin that will work in about 1-2 hours, if you massage it, it can work immediately, lowering your patient's sugar rapidly. Also, some drugs can damage the tissue around the injection site if your massage it. Like if you're giving LMWH, massaging can increase the risk for hematoma
Supposed to draw back on syringe a bit after inserting needle to look for blood return to.make sure not in a blood vessel. Swabbing with alcohol prior to giving insulin injections went out of favour very long ago before this video due to it causing thickening/ scarring of skin. Applying pressure like that post subcut injections not usually done as causes more bruising. Hard and prolonged pressure like that is for IM injections. Massaging area is only for certain IM injections need to know your medications. Otherwise will cause more bruising or tissue damage.

Use 45 degree angle if pt is very thin or emaciated with very little fat tissue
