    FAILURE MODE AND EFFECT ANALYSIS (FMEA)
    Failure Mode and Effect Analysis (FMEA) is an analytical technique (a paper test) that combines the technology and experience of people in identifying foreseeable failure modes of a product or process and planning for its elimination. FMEA is a "before-the-event" action requiring a team effort to easily and inexpensively alleviate changes in design and production.
    FMEA is used for risk management under lean six sigma, project management, total quality management, production planning and control, service quality, and financial planning in the automobile industry, aviation industry, banking, hotel, and many more areas where the risk is very high.
    OBJECTIVES OF FMEA
    • Recognize and evaluate the potential failure of a product or process and its effects.
    • Identify actions that could eliminate or reduce the chance of potential failures.
    • Document the process.
    TYPES OF FMEA
    There are several types of FMEA: design FMEA, process FMEA, equipment FMEA, maintenance FMEA, concept FMEA, service FMEA, system FMEA, environmental FMEA, and others.
    However, for all intents and purposes, all can be broadly categorized under either design FMEA or process FMEA. For instance, equipment, service, and environmental FMEA are just slightly modified versions of the process FMEA, and system FMEA is a combination of design and process FMEA.
    DESIGN FMEA
    Design FMEA aids in the design process by identifying known and foreseeable failure modes and then ranking failures according to their relative impact on the product.
    PROCESS FMEA
    Process FMEA is used to identify potential process failure modes by ranking failures and helping to establish priorities according to the relative impact on the internal or external customer.
    STAGES OF FMEA
    There are four stages of FMEA
    1) Specifying Possibilities
    a. Functions
    b. Possible Failure Modes
    c. Root Causes
    d. Effects
    e. Detection/Prevention
    2) Quantifying Risk
    a. Probability of Cause
    b. Severity of Effect
    c. Effectiveness of Control to Prevent Cause
    d. Risk Priority Number
    3) Correcting High-Risk Causes
    a. Prioritizing Work
    b. Detailing Action
    c. Assigning Action Responsibility
    d. Check Points on Completion
    4) Re-evaluation of Risk
    a. Recalculation of Risk Priority Number

    Thank you sir for uploading the video. In the video at last RPN value was reduced value, in that how can we select manually S O D factors? Just prior to selection of S O D factors in second time you have mentioned the remedial action to improve. Action not at implemented, if suppose implemented again we need to conduct the re-examination. I didn't get clarity.

      Once you have calculated the RPN value, you need to prepare a corrective or preventive action plan, after implementation of the corrective and preventive action you need to again check whether the RPN is reduced or not. What remains for the clarification?

    can AIAG sev,occ,det rating guidlines can be used for HR,PURCHASE AND STORE IF not then what to do

      Take the example of Absenteeism in HR function, prepare the severity, occurrence, and detection table for the HR functions related to the absenteeism, find the severity of absence of the person, occurrence ( how often it is happening or what is the cause of absenteeism) and detection ( what are the current control for the absenteeism). Calculate the RPN value for absenteeism, take necessary corrective action and find the new value of RPN. Check whether it reduced substantially? Severity may vary according to the department.

      For severity, occurrence, and detection, there is the requirement of a separate table for each, but according to function it depends upon the severity of the functional loss in the process due to any kind of failure
