In statistics sigma is "standard deviation" and is a measure of how many items in a lot are out of spec. Six sigma means four parts out of a lot of one million. So in a six sigma system, only four bad ones out of a million is acceptable. Any higher count of bad ones per million calls for corrective action.

Good video, although it was implied that Bob Hartman came up with the philosophy. He first used the term in an article in 1988, but John Krafcik coined it (maybe) , but a majority of the philosophy was built by Taiichi Ohno and this is accepted by many schools of thought. If you are giving credit to someone, I believe it is proper that credit is given to Ohno. But it's difficult, as Deming was in Japan as well at the time, and TPS was inspired by his principles as well.
The bottom line is, that Lean, Six Sigma are out there - and have been inspired and enhanced by multiple sources for all to use.
