You explained the different stages and symptoms very well. My 11-year old Chihauhua was rated with a 5 murmur, Stage B2. I'm realizing now that when the Vet kept asking if Luke had been coughing and I kept saying "no", when I should have said, "yes", because Luke was hacking (I obviously wasn't putting 2 & 2 together). Looking back now, I'm realizing other subtle signs/symptoms that are more meaningful now. Yesterday, I made Dr. Judy's heart healty dogfood recepe and Luke prefers it cooked rather than raw and he likes it. Five days ago, Luke was started on Vetmedin Chew 1.25mg 1x every 12 hours (he loves it). Still waiting for final ecocardio results (testing was done close to 2 weeks ago at the tune of $2K). Yesterday, he was definitely having nasal/breathing issues, then today was a huge improvement. Slight restlessness for about 20 minutes, a little more nasal issue for a bit, and now he's fast asleep and he slept real well all night long, last night. Just trying to do my best for my little love.

Excellent video explaining heart disease symptoms! 👏 We use dandelion tinctures (5 Leaf Pet) as a diuretic and thiamine pyrophosphate drops (Metabolics) to reduce heart enlargement in our little guy.

Dr. Judy, it is my understanding that there would be a lot of range in the grading of a heart murmur. In other words, depending who is listening, one veterinarian's stage 1 heart murmur might be another veterinarian's stage 3. Am I right about this or is the grading more scientific and precise than I understand? I know dogs with higher grade murmurs who are clinically fine and advanced in age and their veterinarian never recommended a cardiologist to the owner and then there is my dog who had developed what was called a grade one and the veterinarian recommended I take her to a cardiologist prior to getting her teeth cleaned. I declined of course because I found that to be over the top and because it is about $850 just to walk in the door of the local specialty clinic. Interestingly, her grade one disappeared completely after her dental cleaning (no teeth were found to be diseased) she was 14 years old (miniature poodle).
My current dog is a miniature poodle who was grade one at age 9 and then grade 3 about 9 months later. Not sure if this means his heart disease is advancing rapidly or if there is a lot of range in grading murmurs. He has always been on a species appropriate balanced commercial raw in combination with balanced freeze dried raw and balanced low temperature cooked high meat with low veg and very little grain or other high glycemic non-meat things. None of the vets at our practice suggested he needs to go to the cardiologist which I would do if they suggested. I now supplement him with ubiquinol and "heart health" from Bark and Whiskers. He has always gotten fish oil (high quality Nordic Naturals brand for pets in capsules). The heart health supplement contains Hawthorn, carnitine, taurine, D-ribose. His diet, however, should supply sufficient carnitine and taurine. He gets 40 to 60 minute walks about 5 times a week. I take a much less is more approach to vaccination and do titers instead and do not use any pharmaceutical flea/tick meds or heartworm meds as we live in an area that is safe from heartworm.

What you listed plus elevated resting respiratory rate. They should be asleep and you count the breaths they take (chest rise) and it shouldn't be anymore than 30 breaths per minute. You should do several measurement's a day and keep a list and then average them at the end of the day and weekly.
