Dr Marios face being obscured is interesting, i do have a theory behind this, but this is just speculation, if they are the same character in this au: from our point of view, we see no face, but now my point is what if from Marios pov he sees himself..... as he was once a doctor himself if Dr Mario and Mario are the same. This is speculation though, so take my theory with a grain of salt🤔
Yes, they are the same. In Mario's past, he used to be a doctor, but decided to be a plumber instead with his brother. In this video, Mario is basically talking to his past self in a dream.

They're talking about an incident that happened in the "Flicker Prologue" video, where Luigi was attacked and pushed off of a cliff by Mimi. The reason there are two Luigis at the end of the video is because Mario and Luigi went into one of Luigi's memories in Flicker Part 3, and they saw Luigi hurt because of what happened in that memory.
