  • One unexpected and unwanted outcome from modernization of society is the increasing prevalence of diabetes mellitus due to changes in lifestyles. Diabetes is a lifelong disease which has no cure, causing terrible suffering among patients and tremendous strain on healthcare systems. Watch this episode of Medicurio to learn more about the cause, symptoms, and treatments available for diabetes.
    Rarer forms of Diabetes:
    Gestational Diabetes: Similar to Type 2 diabetes as some insulin resistance occurs, but only occurs during pregnancy. In most cases, gestational diabetes disappears once the baby is delivered, but some women progress into Type 2 diabetes even after birth. Likely due to various hormones released during pregnancy that interfere with the insulin signalling pathway, although the true cause is still being researched. Babies born to mothers with untreated gestational diabetes exhibit signs of Type 2 diabetes from birth.
    MODY (Maturity Onset Diabetes in Youth): Diabetes due to mutations in a MODY gene: a gene associated with either beta cell maturation, glucose storage and release, or insulin receptors. Specific symptoms and treatments vary based on the mutation, but all exhibit high blood glucose. There are currently 11 MODY genes known, but there are likely more that are undiscovered or too rare.

    Trivia:
    The first mention of diabetes occurred in Ancient Egypt over 3000 years ago, which described a sickness that caused excessive urination. This was likely Type 1 diabetes, as Type 2 diabetes only became more common when society and lifestyles became more modernized.
    One of the first ways to diagnose diabetes was to urinate on the ground and see if ants were attracted to the urine (which they would if it contained sugar). This was discovered in Ancient India.
    Insulin was discovered in Canada by Dr. Frederick Banting and his student Charles Best at the University of Toronto in 1920. They isolated the hormone from beagles and later injected diabetic beagles with their isolated substance, realizing that the hormone lowered blood glucose. This saved an innumerable amount of Type 1 diabetics' lives, who rarely survived past childhood. Banting got the 1923 Nobel Prize in Medicine and Physiology that was shared with his supervisor, Dr. John Macleod. This decision infuriated Banting, who thought that Best should have shared the prize with him, and shared his cash award with Best.
    Initially, insulin was isolated from pig pancreas and then administered to patients. Over time, scientists managed to insert the gene for insulin into bacteria and yeast, which produced the hormone on a massive scale and allowed for more patients to be treated. Now, many different types of insulin are available. By slightly modifying the gene inserted into bacteria/yeast, and therefore modifying the insulin protein, scientists can control how fast insulin is broken down in the body. Fast-acting insulin can be administered right after a meal, while slow-acting insulin is administered before sleeping so that blood glucose levels can be maintained overnight.

    7:40 Insulin should not be injected directly into the blood! This is only done by doctors in the case of intensive medical emergencies. Insulin is administered subcutaneously.

    I thoroughly enjoyed your explanation, liked and subbed (and rung the little bell). However, If I can offer some critique of a disproportionately minuscule thing for how long this response is... In the last 20 seconds of your demonstration you suggest that diabetes is part of societal agency factors. Diet and exercise. Specifically, individuals that need to be held accountable for their own 'healthy lifestyle' (which is a paradox- worthy of self-investigation). Inadvertently, you make no mention of the structural factors of society that are intertwined with the epidemiology. For example. Not mentioning that there is also a free-market over-saturation of sugar and fat consumables available to people that makes these products cheaper in comparison to healthier alternatives; which affects individuals in lower socioeconomic positions more than others. Which exponentially worsens their situation.
    Companies that produce products and Governments that protect the companies, that essentially poison one tenth of the population should be held more accountable than the individual. Now to clarify what I mean by "Companies... Governments...should be held more accountable..." is that our collective focus can not solely rest on the individual, as the structural parameters that perpetuate poor health exists and we should challenge the status quo. Tobacco smoking, which is a similar product still exist under similar yet heavily reduced circumstances. Rates of smoking are decreasing in places around the world, and it is through education of the agency and the structural factors that this is possible.
    Also if you made it this far congratulations. But to squash any sugar lobbyist lurking about with the argument of "they chose to consume our products" I would say - You have knowingly profited off the ill health and suffering of may individuals and you have come to the rescue of your own disintegrating moral complex and capital self-interests, Bravo.

      Great point! If I had more time I would have wanted to mention the socioeconomic factors as well, since they play such a huge role in health and disease.

    Educating me while in High School would have been very helpful for me to understand what Type 2 diabetes is hand how to avoid it in later life. Too late now.

    • @trevor7234
      @trevor7234 5 лет назад

      it's not too late. You can reverse Type 2 by lowering or cutting out carbs completely. Aim for 50g max per day and you will be cured (barring any other rare medical issues that may impact this).

      High school is already very late, though, as it’s very difficult to change your habits by that age. Better to start off early.

    • @trevor7234
      @trevor7234 5 лет назад +6

      @@PeriodicallyRational of course diabetics can have chocolate. Type 1 diabetes means we are "carbohydrate intolerant." We can eat all the sugar we want, we just need to take as much insulin as necessary and at the correct time to match the insulin with the carbohydrate impact on our blood sugars. I, like many diabetics, suck at this, so I use a low-carb diet to make things super easy in attaining stable daily normal blood sugars. =)

  • @ant7936
    @ant7936 4 года назад +1

    So, the solution to everything is medication?
    I think not.
    A "healthy diet" to prevent diabetes T2 is a diet low in sugars.
    Fat in the diet does not cause T2.

    Explicit explaination. I have some doubts regarding weight gain in type 2 diabetes. I know that obesity(mainly abdominal) induce insulin resistance because they release adipokines .My question is why there's no significant muscle wasting and weight loss in these patients as they also aren't able to use glucose as primary fuel and would eventually end up using stored fat and proteins, so they should become lean. I appreciate if you can clear my doubt.

    • @Medicurio
      @Medicurio  6 лет назад +1

      Undiagnosed Type 2 diabetes does cause weight loss and muscle wasting in some patients due to the reasons you mentioned, as well as dehydration from excessive urination (due to high blood sugar). In fact, one of the symptoms of diabetes (mainly Type 1 and sometimes Type 2) is sudden weight loss. Perhaps this is not significant or does not occur in all patients is because some patients may not be fully insulin resistant, so they simply eat even more to compensate for less glucose entering cells, which maintains their weight and lessens the need for muscle breakdown (but further exacerbates insulin resistance). Once the pancreas shuts down and insulin is no longer produced during the later stages of uncontrolled Type 2 diabetes, the weight loss is accelerated if there is no medical intervention. Hope that helps.

    Great video. I did not know that some / most? T2 medicine is reducing the amount of glucose in the body when you could just reduce the consumption of carbohydrates.. But I guess that wouldn't earn the companies that produce those drugs any money...

    Question: I could be getting a Pancreas transplant in the near future. The doctors say this will essentially cure my type 1 diabetes. If type one is caused by my immune system attacking my beta cells, wouldn't my immune system just do it all over again?

    • @Medicurio
      @Medicurio  5 лет назад +1

      That's a great question. Usually transplants involve immunosuppressive drugs as well to decrease the chance that the immune system will attack the new transplanted organs/cells.
