What if we have two nouns, how do we know which one is the head?? As in "the elders' sufferings"

"the elders' sufferings" sufferings is the head. First noun is premodifier which is doing the job of possessive.

I think the elders is the head since its answer the question who suffers. Suffering just modifues the elders
