    In 2005 at the age of 47 my BMI was 20.5. I had good muscle tone. I was diagnosed with 1.5 diabetes.
    I was at the doctors to go over my full panel blood test when my doctor left the room. He came back with another Doctor Who had a white coat down to his calves. They were both smiling. I was told that they’ve never seen blood work like this except in athletes in training. I laughed and said I’m definitely not an athlete and training. I was told my LDL is high (i never asked what the number was) they said they weren’t going to worry about my LDL considering the rest of my blood work. They said to keep doing what I was doing. Not sure how I was eating but I did smoke a half a pack of cigarettes a day and got drunk every 2 to 3 weeks on a Friday or Saturday night. Stop drinking in 2009 and stop smoking in 2013.
    As I moved around and saw different doctors they all wanted to put me on statins.
    Two years ago my LDL was 415, HDL 64, triglycerides 88.
    Last year my LDL went down to 376, with no drugs, my last blood test 3 months ago my LDL was 288, HDL went up to 84 triglycerides are 82.
    My BMI now is 21.5 but not good muscle tone.
    Finally found a functional medicine doctor that’s going to listen to me. Next week I have a blood draw. There are 24 items on this list to be checked.
    I’ve been low carb for years, went lower carb keto then carnivore and back to keto carnivore. One thing I’ve learned is I definitely have to make sure I drink a gallon of water a day and not over stuff when I eat. I’m lactose intolerant, grains make me sick, and I don’t eat fruit.
    According to Western medicine I should’ve been dead years ago.

      i stopped eating grains and just rercently dairy - my migraines completely subsided. whatever you do, dont eat seed oils and artificial sweetener! eat salt if your heart is acting up. I didnt drink milk in years, but what really did it for me was completely cutting out every form of dairy.

      Cholesterol is a bogus test as there are Polynesians with cholesterol in the 400-600 and perfectly healthy( not lab data compliant). They don’t eat all the other stuff that’s processed we shouldn’t. Door do your or research everyone. No one cares more than you. Dr. Gcpanos

      ​​@@Beatrice-nx5ld(I felt the urge to chime in) The problem with resistance training is that most of the people over 50-55 have some kind of minor injuries or diseases that make resistance training difficult. For example I injuried my wrist doing planks. Nothing serious but it never healed. It just happens so easily when you are bit older.

    I recently had an extremely high LDL result... I was eating low carb and definitely lost weight vs prior test... So yeah, I believe Nicks research.

      Having high LDL means efficient fat trasport from liver to peripheral cell. Even if LDL truly cause heart disease, its better than diabetes.

    Yes it did happened to me. Iam a 67 year 0ld 5foot and weigh 63kgs A YEAR AGO. Now I weigh 47kgs by changing my diet. I was also taking Trimedizine for AFIB, Losartan potassium, bisoprolol, sitagliptin, atorvastatin and many others not to mention Levothyroxine due to thyroidectomy. I was a mess my body ached from head to toe and feel tired all the time. I went to my doctors and what they did were to prescribed more drugs for pains and vitamins. Luckily I met a neurologist who told me to change my diet to 50grams of carbohyrates a day. I followed her adviced and go to You Tube and thank you doctor because I learned a lot about Keto, Carnivore and lots of fibers diet from you. I tried to experiment to make meals that have a balance mix of fats, protein and fibers. I also stopped using sugar in my coffee, and teas. The result was amazing in just 10months Ilost 16kgs and now Iam just maintaining my 47kgs weight. By the way for the first time in my life my last blood works are okey except for high LDL.

    This sort of thing does not inspire confidence. It just makes me think we have a lot of confident doctors who are are clueless. Seems like we shouldn’t still be arguing over what’s a good diet in 2024. But here we are.

      ​@ArticulateArena Fact: science changes, it especially changes depending who it's funded by. Nick's research as well as Dave Feldman's is not funded by big Pharma or the sugar industries and other big food. It is all self funded - no conflict of interest.

      @@GameofTrees no, I’m not worried about numbers. What I’m worried about is that if we really understood the process then the result should have been predicted by the medical community at large. The fact that it comes as a surprise means they don’t understand their own test results. We’re getting a lot of fallacious medical advice and lots of conflicted opinions which makes leading a healthy lifestyle difficult.

    You know... I'm just SICK of all the conflicting so-called 'information' coming out about health and nutrition recently. I think a lot of it is for the purpose of selling you something. So I've shut down now about all this crap and am not paying any attention to it anymore. I'll probably turn out to be much healthier for it.     ‘Bad’ Cholesterol Not As Bad As People Think, Shows Texas A&M Study - Texas A&M Today
    Steve Riechman, a researcher in the Department of Health and Kinesiology, says the study reveals that LDL is not the evil Darth Vader of health it has been made out to be in recent years and that new attitudes need to be adopted in regards to the substance. His work, with help from colleagues from the University of Pittsburgh, Kent State University, the Johns Hopkins Weight Management Center and the Northern Ontario School of Medicine, is published in the Journal of Gerontology.
    Riechman and colleagues examined 52 adults from ages to 60 to 69 who were in generally good health but not physically active, and none of them were participating in a training program. The study showed that after fairly vigorous workouts, participants who had gained the most muscle mass also had the highest levels of LDL (bad) cholesterol, “a very unexpected result and one that surprised us.

      I've known it was the muscle building lipoprotein for 5 years and I'm housekeeping. I bet the nurses at my retirement facility don't even know. Scared to ask and fight about stupid crap when I got crap to clean. Quite literally a lot of the time to be honest.

    I am on a strict keto diet but had to re-introduce more carbs intermittently and this works great. I don’t believe humans were never meant to eat carbs in quantities occasionally. Full on keto is fantastic for health and weight management. Great video again! 🎉

      We got honey and fruit when we could if we could otherwise we ate the end trails where the animal digested the sugars.
      So yeah, like it's not the end of the world. Unless carbs cause flare ups or bad habits.

      I would think we’d eat nuts, fruits, berries and vegetables if they were available. It makes no sense that we were strictly meat eaters and nothing else.

    I am 72 years old and three months ago I had triple bypass surgery for blockages. I am 6 foot tall and I weigh 165 pounds with a BMI I have a BMI I have a bout 22. My LDL is through the roof. I was on the ketogenic diet for four months and my LDL kept going up which seems to fit with what you’re saying about this last study. My cardiologist has me on a Staten Crestor but I’m having a lot of side effectsShould I stay on the Staten

      No, I would get off that as soon as possible. Look up Dr Ken Berry and Dr Anthony Chaffee on statins. They have a lot of good info about why you should never take it.

    Dr Ali Nadir also did a great talk on obese people and the importance to NOT eat a high fat diet until the fat cells were able to store lipids again, as opposed to an inflamed lipid leaking state. Until the body restores its ability to safely store fat, ..as its designed to do, he recommends lean meat and low carb. The body will eat its own fat and adding extra fat in a dysfunctional metabolic state could be harmful. The body is already overwhelmed with lipids. Adding more fat just gums up the works. For severely obese people with a deranged lipid metabolism, lean meats must be used for up to 6 months..until the body resets.

    Interesting, when I went healthy low carb ketogenic diet my total cholesterol went up to 280, my triglycerides lowered to 50, HDL up to 70. I'm lean athletic, muscular always had total cholesterol of 180 for 50 years with developing health issues, and cholesterol rose up when I went low carb but all other markers improved. I'm a LMHR and have tremendous health improvements on ketogenic diet. Its helped me over come some health concerns as my total cholesterol rose, triglycerides lowered, HDL rose inflammation lowered I feel better and better.

    I saw this exact presentation at the Metabolic Health Symposium in January 2024. It's fascinating and so interesting all the information about LDL levels. Been Carnivore since September 2023 doing wonderful, LOL but LDL has gone up

    I think it's important to mention something that maybe isn't mentioned enough as a cautionary note. Hypothroidism raises your cholesterol. Given how many people, especially women, have low thyroid, it needs to be talked about a bit more. What it means, functionally, is that your lipid metabolism will not be as efficient as someone with a healthy thyroid. You might have a lack of stomach acid or bile, your digestion will be slow, you might have assorted gut problems, your liver might be down regulated, you might have tissue hypothyroidism in multiple organs. All this can add up to a big problem on a high fat diet. If your insulin is low, and hypothyroidism has already damaged receptors in your liver, high triglycerides are kind of inevitable. You can't store excess fat (no insulin) and your clean-up system is sluggish and dysfunctional at best.
    I don't see this talked about enough. I've been high fat carnivore for over a year and my latest lipid panel was pretty terrifying (Trigs: 328.) So, must be said, for people like me, lots of saturated fat isn't a good idea.

      Do people have thyroid issues because they eat too much (not fasting enough) and don’t exercise? Is high cholesterol really bad for you? I question everything we’ve been taught about nutrition at this point and practice intermittent fasting along with a weekly 40 hour fast. I used to eat all day every day and as long as I was working out I was fine but things change as you get older. I just started carnivore but it’s more high protein than high fat. We’ll see how that goes.

      @@shelfcloud487 The majority of thyroid problems, for women, are Hashimoto's. It's an autoimmune condition where the body attacks the thyroid. It's not an actual problem with the thyroid gland, it's the body's response to it. Primary hypothyroidism is not an autoimmune condition, it's either a problem directly with the thyroid gland (like cancer) or with the pituitary.
      I've fasted for years and it's made no difference to my hypothyroidism, except maybe make it worse. I'm technically underweight and because of the thyroid I'm not absorbing nutrients well from my food. I was hoping that the carnivore diet might help my thyroid, but after over a year zero carb, my active thyroid hormone is even lower than when I started and my cholesterol (and triglycerides) are sky high. I think Paul Saladino is probably right - some of us just need insulin. My hba1c and fasting insulin have both been rock bottom for over a year, I've been eating the most anti-inflammatory diet that exists and fasting 20+ hours a day. It just hasn't worked for me, so I'm reintroducing carbs and cutting out the fasting. You gotta work with what you've got.

      @@islaadele1212 But how did you grow up? Did that lead to irreversible damage? Was the damage already done before you started fasting? That’s my question. I wish I’d known about intermittent fasting when I was younger and the benefits of fasting in general. I had my gallbladder removed when I was around 40. It was probably from all the crappy food and lack of fasting. Now I regret it.

      I think anorexia in my teens damaged my thyroid. Too much fasting, you could say. The body slowed everything down to help me survive. Unfortunately in later life that's led to chronic malabsorption. I ate meat and liver and eggs for over a year and my iron levels, without IV supplementation, are still third world territory.

      That is NOT what is represented. When your BMI gets to 22, 21, or lower and LDH is high then you can say you are a LMHR. Till then just stop worrying

      I think what we all need to know is if we have pre existing damage to the arterial walls. Then and only then can we know what risk ldl can pose.

    For me I view the body as ALWAYS TRYING to keep us OPTIMALLY ALIVE no matter what WE throw at it. So if we are GROSSLY overweight and we do something to lower our weight and LDL goes DOWN then for me the high LDL was trying to keep us OPTIMALLY ALIVE in our OBESE state. If however we are at a more normal weight and we do something to improve our health and LDL goes up then it is trying to OPTIMIZE us even further towards OPTIMAL HEALTH for our current condition. So we really NEED to FULLY understand what LDL is doing rather than ASSUMING high levels are bad and low levels are good etc.

    I reduced my carb intake almost to Ketosis and together with my BMI going well into the normal range, LDL-C went from 125 to 70. My HDL went from 50 to 69 and Triglycerides from 150 to 50. Comments?

      What was your starting BMI? [and end BMI ?]
      If you were over BMI 35 it should go down, then as you get leaner it starts climbing

    Which LDL, what size particle? The worry would be small damaged LDL wouldn't it. I'm 62, practice Keto, OMAD and fasting, Lift weights and HIIT, 6'1' 209 Lbs 9-10% body fat. Absolute faith in what I do.

    I only worry about my triglyceride it pretty high 300.. it damaging my kidney.. thats why i go carnivore diet..

    There is High Density LDL, then there is also Low Density LDL Large fluffy LDL as apposed to high density LDL. One is a healthy LDL, and the other not so much. Yes, there is alot missing from this study. Most likely what was increasing was the good LDL. No other explanation makes more sense in this case. Having an elevated cholesterol level may not be a negative health issue. The more important negative effects of health is having high Triglycerides, more so than high Cholesterol. Having high triglycerides is mostly caused from too much sugar-glucose, or carbs in the diet. Lowering triglycerides will automatically lower Cholesterol levels to a degree as well. Having low carbs in the diet is not enough for good health. There is no itemized list of what all was included in the diet besides having 130g of carbs (vague statement). Most likely the levels of LDL increased due to a high fat content diet. A high fat diet can also result in high HD LDL levels. Increasing a diet rich in Omega 3 oils will raise the good HD LDL levels. A high fructose/carb diet generally results with high LD LDL levels. Cholesterol levels are a marker of health... no more, no less. However, it is an important marker all the same.

    I found that my LDL did go up on low carb about 40 points to 280. However my HDL went to its highest level ever to 71 which is also my age at 71 and my BMI is less than 25.

    I'm the poster boy for this I have a bmi of 22-23 and my ldl cholesterol jumped 2.00 full points after starting carnivore. What I find shocking is there is no discussion of if the ldl is creating plague in the arteries. I think it's correct to say If there is no plague there's no issue. So why isn't any one talking and more importantly studying this aspect. I mean who cares what the ldl level is the question to be answered is " is it harming the arteries". If not we can probably stop talking about it.
    I'm getting an ct angiogram ( I'm in Canada so it's hard to find) once I have a clean artery scan I can relax imo. Would love to hear Dr. Westman respond to this and cover it in a video.

    As someone who has controlled my weight by controlling carb intake for 40 YEARS (!), I am fascinated by this study. 5'4" 128 lbs BP 110/70 Trig 101 HDL 78 LDL 228. Not in their cutoff numbers, but close enough for me. :) Been refusing statins for the last 12 yrs. Tired of the arrogant doctor attitude, being labeled "noncompliant", yah yah yah...

    My cardiologist gave me ezetimibe have to say ldl went down and had more fat pass through me making things a lot more comfortable and digestion seems to be better.
    Mainly ketovore diet.

    They refer to BMI, so what about ”skinnyfat” I.e. TOFI persons? Would this have an impact on the study?

    Unfortunately I have FH and without a statin, my cholesterol went so high that the test couldn’t register it (>700) and LDL went up to 587 on keto in just 3 months. My BMI isn’t quite in the normal range, but it’s closing in.
    I’m sticking with keto because I know what happens to my overall health when I go off it.
    I just wish there were affordable alternatives that would reign in cholesterol to reasonable levels without side effects. 😕

      Or perhaps cholesterol isn’t the cause without the glucose damage to the arterial wall and the glycation of the LDL particles. How does the arterial wall become porous? Glucose damage - we know it kills cells - diabetics have to amputate body parts because they killed them with high glucose. Avoid sugar spikes and you avoid arterial damage (hopefully arterial wall can be repaired with fasting and carnivore - we’re all counting on it).

      Cholesterol and lipoproteins are under the control of your genes, whatever your level is that’s exactly what it’s supposed to be. There’s nothing to show that cholesterol, lipoproteins or any subfraction of it are an issue.

    The research is not blaming the increased fat in the diet, it’s blaming excessive exercise (the 6 pack) which is depleting the glycogen too rapidly creating stress to LDL production so higher carbs are required if you exercise excessively!

    I brought my BMI down from 35 to 21. I used to be 230 lb 10 years ago and weighed as much as 245.
    Now I weigh 145 lb first thing in the morning.
    My LDL went up, but was more than compensated by drop in TRG to 67 from 130, rise in HDL from 35 to 74.
    My Omeg-3 Index is 11% through supplementation alone.
    My Fasting Insulin is now 6 (ideal is 3-5 in American Units)
    My fasting glucose is 90, for a HOMA-IR of 1.3 (Ideal is 0.7 to 1.0)
    Yes, one has to look at the bigger picture.
    Want me to give up Keto and IF? Convince me; what is YOUR Fasting Insulin?

    Having a BMI under 25 doesn't mean you are "lean" though. Not in the sense that you'd necessarily have a visible six pack or large amounts of muscle mass. It just means you aren't overweight in relation to the rest of the population. When my BMI is 24 I know I'm carrying at least 7lbs extra fat than I should be.
    LDL alone doesn't matter. This is why instead of looking at numbers on a test result we should be looking at individuals as a whole and not chucking pills like statins at anyone over the age of 40, like we do in the UK.

    Too much white coat emphasis on LDL, and cholesterol altogether. They generally do not even do a detailed breakdown on of cholesterol. They are just preaching from an out of date hymn sheet.

      I know you are being tongue in cheek, but to be certain, Nick specifically urges noone do the 'oreos to lower LDL'. That said, it's not the Oreo, it's carbs in general, I believe he has previously done the same thing with starch (potato? ) but Oreo is more attention grabbing.

      It was a glycogen dump into the liver while the subject already had a low BMI [and the body was already running on lipids]
      The Oreo experiment would have different results if he had a BMI over 35

    In 3 months of eating Ketovore I finally have a cholesterol rating LOL. It went from being so high they couldn't give a rating to 4.13. Not perfect, but expect better in 3 months.

    Been on keto for three months. BMI now 22, weight 68kg from 74kg, LDL increase 2.5 times to 418 mg/dl while HDL is 80. Doctor wants me to 1. stop keto 2. take statin plus ezetimibe combination. WOW! Confusing

    Its not raising my cholesterol. Despite diet my LDL is fluctuating from 90 to 109 tryglicerydes around 50 - 70 ... total cholesterol is 160- 190 ... It changes but not drastically. I'm on keto about 5 years and on carnivore 1 year . Just my tryglicerydes changes . When I was on " No diet"(eating like hoover everything) my tryglicerydes where 145 - 160.Im 49yo

      Lean mass hyper responder. Leanness predicts higher increases in ldl on keto/carnivore.
      The evidence so far suggests this isn’t causing any plaque development

      Unless you’re eating massive amounts of protein you should be at least somewhat in ketosis. Are you basing this off of lack of ketones via urine strips or something?

    Hi. Don't know if you'll see this but here goes. I'm 51, male and have been obese more than half my life. The only times I've been able to maintain a decent weight was while eating low carb. My cholesterol is "flipped", and has been that way since I was 14. I've tried statins, they do terrible things to my ability to think properly. I'm about to start taking Repatha. I'm currently 385 pounds and my health is starting to suffer for the weight. I wanted to start a keto diet again but my doctor says this will not work with the Repatha medication. I'm sick, and tired and don't know who to trust anymore.
    Suggestions?

      Go full keto and exercise every day-start slow and ramp up as you lose weight. Ditch the meds.
      EDIT: Also do intermittent fasting. Eat between noon and 7pm only-2 or 3 meals whichever fits your schedule best. Good luck and hoping you see great progress

    Dr Westmqn, I have been on a keto carnivore diet for nearly 2 years, with some failures along the way. I find the information in books and videos by Dr Greger promoting veganism quite persuasive, but when I try it I get cravings and put on weight. There seems to be a conflict in the information space. Could you shed light on this please.

    As VLDL traffic in blood is going up, packed with high load of triglycerides along to the concept, why is it that we see a lowering of triglycerides ? Any thoughts welcome

      Because the triglycerides are being burned as fuel. So trigs are low, and LDL containers remain.

      my experice: i cut out dairy, grains, and seed oils. i am becoming thin, have a thin face again, and although i didnt train or anything, i am breathing way more throgh my nose when jogging the odd 200 meters - i was never able to breath through my nose, it was always the mouth. skin inflamattion is way better.

      There is a strong link between PCOS and too much insulin. It's the carbs that stimulate insulin not the meat or the fat. A ketogenic diet helps resolving this issue

      Eating red meat and saturated fats are NOT a requirement of being "on KETO". ESA (eggs, salmon, and avocados,) for instance, are low in saturated fats and are low in carbs. I eat a lot of vegetables and ESA and little red meat and saturated fat.

    I cut carbs by greatly reducing bread, rice pasta, and other high carb foods. I don't eat cereal anymore. And most of all I cut down on sugar as much as possible and I mean cut down to almost zero. My LDL went down significantly and I feel great and I have no digestive problems and some of my other symptoms have gone away. I would love to discuss all this with you. how much would it cost?

    BMI is a terrible parameter. My ldl and total went up by 150 each . My bmi is 26.

    Lower carb, but not keto. No processed foods or sugar. Bmi 20, Total cholesterol 278, Triglycerides 33, HDL 104, VLDL 4, LDL 170. T Chol/hdl 2.7. These numbers still concern me.

    Nick is hard to follow and understand. If he wants the average persons to understand his research, he needs to be able bring the science to people so, they can understand and speak in a slow, clear, and concise manner. And unfortunately, you are not doing a great job explaining his theory.
