I called UT southwestern Medical center in Dallas TX and they stated they don't work with individuals over the phone that are from out of state.

Currently insurance doesn’t cover it I cost 4,500 dollars.
It depends. Some insurances have covered it, but some providers don't accept insurance. It differs from place to place.
