I had the same problem, I've search for a while, but I didn't found the last one Hunter Journal, but in this video I realized that I didn't find the goam. Have you found the "fossilised goam" yet? It is found in a hidden area to the right of the Fungal Wastes entrance. Requires the Desolate Dive spell to access.
