  • Medications, such as Ozempic, Mounjaro, Wegovy, and more, have become increasingly popular for weight loss. These drugs are glucagon-like peptide 1 (GLP-1) receptor agonists, and they mimic a hormone that, among other things, tells the brain a person is full so they eat less. But are they safe, and do they help improve metabolic health? Dr. Rob Lustig and Dr. Casey Means discuss these weight-loss drugs and their effects on the body, and why such medications likely won’t solve the obesity crisis, for which the food industry is culpable.

I feel EXTREMELY better physically and mentally. I AM better. I was able to wean off 7 prescription drugs. I now take 2. And I haven't even been on Carnivore that long, relatively. Ozempic was always going to be a short term solution for me. I was on this medication for 6 months while my body adjusted to a HFLC diet and intermittent fasting. I am now just doing the LCHF diet and fasting without the medication and still losing weight. It was helpful for me to change my lifestyle but I was concerned about the long term effects of this medication as well.

29:20 and 35:43 - Based upon many comments here this rebound is not absolutely true as he claims. Many have used GLP-1 agonists as a kickstart to a more heathy mode of eating. I did that with Byetta a decade ago. When I stopped treatment after some months I would actually _imagine_ I was still on it and was able to continue the new eating habits. So, it really depends upon the person and the level of motivation and knowledge involved. True, a passive individual will most likely revert. [Edit] A few minutes later he _does_ acknowledge the possible use as kickstart to a healthier way of eating so he _is_ more nuanced but he remains doubtful of success in general.

I live with a partner on ozempic for type 2 diabetes. They had severe vomiting and had to reduce the dose to .5 u . I have noticed the facial changes, and loss of muscle changes to their leggs, i am a retired nurse and am very concerned. I am not diabetic and for 3 months stopped all sugar and eating only real food, and lost 38lbs, no vomiting no muscle loss in fact musle gain. They are also on a cholesterol blocking agent. I worked as a endocrine nurse, and coronary care nurse, medical surgical nurse for 40 years. When cholesterol blocking agents came, i was and other nurses were confused as much of our brain and every cell is of such. I am 67 he is 72. I see the future not so good. I disagree with his medications for his diabetes and his habits. As a young nurse dealing with diabetics, we encouraged low carbohydrate low to no sugar diets, now it seems to use medications and eat what you like. I cringe every day watching the pouring of several daily orange juice and two tablespoons of sugar in his coffee. Ozempic is in my humble opinion a bad drug, as with all cholesterol blocking agents. Statins and ozempic and his habits are killing him slowly. If i say something or try to reason with him, it is not so good, and i get the living with a nurse thing. He is my second partner who just does not get it, my partner of 30 years died in his sleep beside me as i slept of a massive heart attack his 3rd on autopsy the first and second he did nothing i could not get him to see the doctor. 1. The liver makes all the glucose you need. You literally do not need to eat any. Any glucose you eat is "extra".

GLP1s don’t raise insulin in the absence of glucose. GLP1s potentiate glucose-stimulated insulin secretion. If you continue eating high processed carb diet you could actually raise insulin levels and worsen insulin resistance. With a low carb diet, you improve insulin sensitivity due to effects on satiety.

While I was on the low introductory dose I lost 3-5 pounds, and I felt the appetite suppression and very mild nausea and slight constipation. What made me quit, though was that while my appetite was decreased, I sensed some mild depression symptoms - as you said I looked forward to eating as a reward, and it removed that as a motivator for me. I didn't like the feeling, and I read about other side effects, so I quit. My doctors tried to assure me that depression is not a symptom of this drug, but I stuck to my guns and refused to continue.

Big difference in addition to less appetite is food that is real/healthy tastes amazing, and I eat less in general but love the experience of eating. How can you increase protein if you can't eat. Stomach is always full and uncomfortable. I can go the whole day and never feel hungry. When I do try to eat, it is only a couple of mouthful.
How do you increase exercise if just walking is a major challenge? Lifting the legs feel like climbing a mountain. It is insane. Why would Anyone want to live in such a helpless state. I refuse to see my endocrinologist because she has ruined my health with this medication. I am diabetic and she thought I would lose weight but I put on 15 kilos. How can that happen when I eat so little? My Tsh shot up to over 20, B12 nosed dived . Death seems more attractive than living like this. This is not living, just waiting for death😢 This medication is a death pill and destroyer of health.

Muscle loss to a certain extent but 1:1 is not acceptable. And does it really increase insulin sensitivity? No, it makes more insulin. If it was more sensitive, you would need less. Because there is so much insulin, fat isn’t the primary source of fuel burning. The body can’t access the fat. That’s why muscle is being cannibalized.

Sadly the BS here is to actually recommend the right way to take these drugs! I am taking Mounjaro on my keto diet, now I am carnivore and lifting weights heavy and cardio in between. I’ve stalled at 7.5mg going to 10mg soon. I am down 60lbs and have another 70-80 lbs to go. To fix my insulin resistance I need massive zone 2 cardio. I think that’s actually the right therapy for weight loss. Combined with heavy resistance training.

Thank you very much for the great discussion. Doctor Metabolical is a hero to me and the government needs to start listening to him and others like him.
My doctor treated my T2D with Ozempic until I quit using it after eight months. For me, the side-affects were absolutely horrible. Out of a week in between injections, I could count on not only two solid days of horrible nausea each week, but also frequent painful constipation. It was as though my digestive tract had come to a screeching halt. I knew it would slow down digestion, but it was simply too extreme for me to handle. Additionally, all of these symptoms would occasionally cause me to have panic attacks, usually at least one every three weeks or so. I transitioned from Ozempic treatment to a strict carnivore diet with intermittent fasting and continued to lose weight while swearing off all things sugar and starch (still sugar). My A1C is at its lowest it has ever been in a few years, (5.4). I felt it was the best solution to prevent rebound weight-gain, while also reducing bodily insulin production. I do not recommend using Ozempic at all. I recommend avoiding it at all costs. It's awful!

I have purchased two of your books: Metabolical and Fat Chance.
In this video, you have touched upon the topic of children and their food intake. This is ideally the hot topic of today. The change can occur on an individual level only as the Market comes before Man for today's governments.

He hasn’t used these because I guess the 5 other meds are keeping things in line but he doesn’t look great, he’s had muscle loss & lots of loose skin on his legs. But when I suggest diet changes as he goes on a Rita’s custard run, he says I’m not a dr.

      Sorry, I had to laugh.
But, on a more serious note, he really needs to make simple (and probably) minor changes to his diet. Over the past 20 years, I have gone from 305 lbs to 168 lbs as of this morning. I have maintain a weight in the range of 170 lbs -175 lbs now for about 4 years. All of this was accomplished by changing my diet in dramatic fashion (cutting 90% of sugar , eliminating fast food, eliminating processed food, eating and cooking REAL food).
In 2016, my lipid specialist did put me on Victoza for a year to attack my insulin resistance. These BEST and most permanent solution is to change to eat real food.

This is absolutely not what we see in clinical practice at our clinic regarding fat versus skeletal muscle loss. We also utilize hormone replacement therapy in combination with GLP1s. We also advocate for a proper human diet, this is critical. We have seen many patients with minimal skeletal muscle loss if this is approached correctly. Also, this is not about DIETS. This is about helping people with a tool to fix the real issues for those that struggle with obesity and that is a substance abuse problem related to chronic excessive processed / refined carbohydrate consumption. Then for most all roads lead back to not having a healthy emotional management system.

Have you ever just wanted to help someone, but knew you couldn't? That was how I felt about her. It breaks my heart to see a beautiful young woman destroying her body.
Your recent video on her popped up. Although you were brutally honest about her situation (some people would say "mean" LOL!), I still liked you because you provided some valuable information that everyone with a weight problem needs to hear. I really appreciate the Dr. Lustig link in this video.
I'm height/weight proportionate. It's taken me decades to figure out what works for me (I'm 68) , after years of chasing after the magic weight loss bullet, only to find out there isn't one. I didn't have surgery, and won't bore you my own plan, but I will say it's possible to lose 100 pounds and keep it off for more than five minutes.
Keep up the good work, and thank you for all you do. ❤

Type 1 or type 2? Generally type 2 diabetes can be reversed with diet and intermittent fasting. Keto for a short time can be helpful but it's not necessary as long as you get rid of the processed food and sugar. It makes it easier though.
Intermittent fasting seems to be key. When we allow our insulin levels to drop naturally we can access the fat stored on our bodies. Insulin inhibits lipolysis, the presence of insulin prevents us from accessing stored fat and burning it. When we fast we do not enter starvation mode, fasting is muscle sparing. But when we eat a low calorie diet we do burn muscle. All of this is explained at length in Dr Fung's books and Dr Lustig also explains it.
Eating real food and getting rid of processed food, added sugars, starches and processed vegetable and seed oils is very important.
Dr Jason Fung has written two books on the topic of diabetes and obesity, both are life-changing. If you haven't heard of these books please take the time to find them and read them; The Obesity Code and The Diabetes Code.
This is good general information, I don't know your medical history but I do know the way to lose weight and keep it off and become healthy is by eating real food, intermittent fasting, getting reasonable exercise, good sleep and stress reduction. Exercise and movement are not for weight loss, they are for overall health.

With bodybuilders and fitness models they increase their testosterone injections, HGH injections and other PED injections while increasing their lifting so they actually lose fat and gain muscle at the same time, but of course, at what cost to the body. They don't care. 100's have told me - " if I can take injections, win Mr. Olympia two or three years in a row, then would be ok with dying". Fact. Crazy. Blind ambition. I am already auto immune and have a big autoimmune cluster. Once the software is broken and the body makes antibodies to a particular tissue or hormone that’s it. Then you have declining organ function and a over worked or stressed immune system as sited by the CoVID research group in Europe - the folks having hardest time overcoming CoVID had really high levels of existing Antibodies. I can't be sure but it sounds like it may have happened to you, I'm so sorry. And lol I'm glad it didn't come to that! Our brain chemicals influence our behavior in such strange ways, but you bet every criminal would say the pills did it lol

Thank you! Dr Lustig is amazing. He really understands obesity. I am 65, type 2 diabetic on insulin. I went from overweight to morbidly obese on insulin even when cutting out carbs and eating less. I have always felt that type 2 shouldn't be on insulin because the body is fighting the insulin with insulin resistance. Maybe it's all doctors can do to keep us alive at present. I was interested in Ozempic for diabetes but certainly don't think I want to lose muscle mass. Too expensive anyway. I have been diagnosed diabetic for 35 years. So much has changed and been understood since then. I am so glad you are looking at it holistically. Type 2 diabetics need to be trained and educated like you say!

I was struggling with increase in blood sugar and promised my obesity specialist that I would try Trulicity. I got Covid and 3 weeks later I took Trulicity for 2 months. I was selectively nauseous to meat which led to binging on carbs. When I dropped it, I was left with blood sugars about 300 and gained 20 pounds. I've fasted for 5 days to bring my blood sugar down and I'm starting over with a low carb diet and IF. This screwed me up when I've controlled my diabetes with no drugs for the last 5 years.

You know there is exceptions to everything. Everyone is different. I was going to take it but I’m over 65 and cannot afford to lose muscle. MY daughter is on it and doing beautifully, no side effects. I am hoping these medications will improve and costs will come down.

Good perspectives. I am doing resistance training 3-4 x per week and eating minimum 100g of protein/day. Should I expect to lose muscle as Dr describes? I lost ~35 lbs prior to starting GLP-1 med. using above methods and eating in caloric deficit. Note: I am preT2D, A1C 5.7-5.8, hypertension and hyperlipidemia controlled with meds + I'm on 850 mg metformin BID. So far, minimal side effects (mild nausea controlled with ginger ale or non drowsy dramamine, abating as I go). I look at this as a stop gap to lose my final 15-20 lbs of weight and get to healthier BMI. I

As a nurse I’m glad you questioned how this works because I still don’t quite understand, I wonder why people don’t suffer hypoglycemia if they aren’t a diabetic and does it improve metabolic health. The people who I see that take it still eat poorly.

I had eaten a TON of sugary foods (giant cookie, pizza, Starbucks drink) an hour prior and my blood sugar was only 77 😳 I stopped taking it but I did like the weight loss Im not gonna lie lol

It is a radical treatment perhaps warranted by severe metabolic issues like high BMI, high H1A etc. I was put on it at .25mg and instructed to eat --- if I can -- starting with protein and then non startchy veggies and no white or starchy foods. Definitely no sugary things. I am 5' 8" and started at 212 lbs and now am 153 lbs. My face is emaciated looking, I've had to get physical therapy to rebuild muscle needed for balance and motion and my vocal cords have been stripped down so my voice is often a hoarse raspy whisper. My gut struggles to eject waste almost getting me into the ER several times. Yes; It worked at killing my urges to eat sugary things, but frankly what saved me was the warning to change what I eat!!!!! Stop the sugar!!! But now I have at age 80 the task of rebuilding from the loss of muscle and the loss of vocal cord tissue. Will my gut return to normal? I have trouble speaking; it's a good thing I learned to touch type! I could have done this in a slow measured less damaging way by adopting a low-carb (not necessarily high-fat) regimen. The way to lose fat is to burn it; not muscle.

I have heard about vagal nerve stimulation, I don't know if insurance covers it but maybe that would help? The vagus nerve controls a lot of the activity of the vocal chords. And I would try collagen for constipation, I am on a medication that also slows down the gut, but fiber left me gassy and bloated and didn't help but two scoops of beef collagen with some sugar free flavoring in a bottle of water (I get mine at Aldi) and poop there it is. If it doesn't work try more, it won't hurt you. And this may sound weird but maybe try carnivore? Dr. Kiltz is a great resource, he's a lil crazy but I got the collagen tip from him and he often has live YT chats where anyone can ask questions no matter what they eat.

I had great success with Wegovy because the extra weight I was carrying kept me from running as much as I wanted.
My Achilles would hurt so much and my heart rate got so high out of nowhere.
I lost 23 pounds on Wegovy and those problems went away.
I stopped wegovy a month ago and I've lost a couple more pounds since then =)
Wegovy is great but only if you're willing to transition into a healthier lifestyle afterwards.

This drug reminds me of SSRI when it first came out it was to help people get out of depression enough to start working on their root cause of their depression. Now 1/3 of the US takes it on a pretty continual basis. Never really have worked on the root cause of their issues or maybe they have become reliant on it (I know this is an oversimplification, but so was the discussion of Ozempic).
