    Often times patients who are about to undergo cataract surgery are over-sold on the laser option. In this video I try to bring light to the truth. There are potential benefits of laser, but as a patient you have a choice whether you want it done with or without laser. An honest surgeon will give you options, but not put too much pressure on their patient to choose the laser upgrade.
    Within a healthy eye is a clear lens. As we become older, that lens becomes cloudy. The cloudy lens is called a Cataract. In cataract surgery, the cloudy lens is removed and replaced with a clear artificial lens implant. During cataract removal, a laser can help assist the surgeon by performing certain steps of the surgery in a very precise manner. This doesn't necessarily always mean a better visual outcome.

    I opted for the manual surgery and the doctor was amazing in the results he achieved with my vision. The doctor offered the option and since I was doing just the standard procedure with the insurance paid lens it made no sense for me to have the laser assisted surgery.

    As a young cataract surgeon myself, I really wanted to like this technology and used it for certain cases for a while, but over time I realized that it was too expensive for my patients for really no benefit to them. Also I found out that I am more reliable than the machine is.

    Hi,
    The laser system providers claim that laser is more efficient is precision cutting as well capsulotomy. That manua cutting tend to be irregular and circular cutting not properly done resulting in longer duration for healing. Can you clarify if their claim is correct based on your experience. Thanks

      I’ll try to answer your questions with my own personal experience:
      1. The Laser is more precise and consistent than me at creating a round capsulotomy. It’s perfectly circular all the time with laser
      2. I’m more efficient than the laser (laser takes longer to get the job done)
      3. With good technique, healing duration is roughly the same (the difference is not more than a day or two)

    Thank you for this video! What to do if you have dense cataracts, pupil does not dilate well, opt for monofocal and no correction for minor astigmatism? I read both laser or scalpel can handle this situation, but laser MAY cause less inflammation, faster healing and no fragments left in eye. Would you say this is a situation that you would recommend the laser? You never said when you would recommend the laser in the video.

    My surgeon said exactly that. Manual all the way unless you have an inexperienced surgeon who needs the laser bc his technique is lacking.

    My doctor suggested I have the laser assisted surgery because I have a large macular hole and have had 2 failed vitrectomies, so he said since I no longer have any vitreous fluid, my eyeball moves around more than a "normal" eye. He thinks the laser will be a bit safer, but gave me the option. I can't decide. Do you have an opinion ?

      He is either an inexperienced surgeon or trying to recoup his investment on Femto laser.

    Is it possible to have laser-assisted surgery under general anesthesia or should the patient follow the instructions during the procedure ? Thanks.

      It's ideal when the patient is awake enough to follow directions, but it's still possible when the patient is asleep

      @@DoctorKrad Thank you, Doctor, for your response ! May I ask how do you manage patients with ADHD, does IV sedation work well ? I'm afraid of moving or not understanding the instructions in time, however, the idea of being under general anesthesia makes me more anxious...

    What is the risk of the laser malfunctioning when it is being used for cataract surgery?

      The laser device holds the eye in place via suction. Eye stability is very important to ensure precise laser application. There is risk of losing suction during the laser. Good thing is that the laser automatically stops in this situation. The surgeon may decide to either reapply suction and continue, or to perform the rest of the surgery by hand if poor suction is expected to continue. It’s not really a malfunction, but definitely could be an issue in some scenarios. I haven’t heard of a true laser malfunction where a laser starts shooting randomly, incorrectly, or doesn’t stop when it’s supposed to.
