cerebral edema is an osmolality issue which will result in free water shift. Why is it important to correct potassium aggresively as it will help to provide some osmole but also help to correct hyponatremia which will counter drop in glucose. Rate of correction is still the key - Rapid hyponatremia correction (not every rapid correction will is CPM but there is always a chance). my suggestion will be ---------------- Watch for osmolality and rate of correction, watch for hypokalemia and sodium correction, if concern change balance crystaloid to saline , watch for urine output --- dont forget to correct hypophosphatemia which is usually overlooked these guys are very high risk of refeeding syndrom.
