The pilot chose the fly through a low mountainous area with heavy fog/clouds, seems pretty obvious to me what the most likely cause would be. Also the aircraft was bit in the 80's so it is very easy to see a possible maintenance issue. Being Iran i can imagine they would blame the US for it

Equipment matters. I have flown on helicopters with radar tuned well enough they could pick up larger waves & surf. So things like mountains or shorelines really ought not be a problem unless... [receiver] sensitivity was out to lunch. To high, you are blind for the clutter, too low and you're blind because the screen shows nearly nothing. It is possible the pilot screwed up his own settings. Occasionally flexing sensitivity up and down is sometimes necessary to grasp a picture of what is returned in low visibility situations.

Fog limits visibility and sometimes patches of fog are thicker than the rest of the fog. Fog is no joke, I’ve driven though it before, it was hard to see. The fog absorbs so much of the light you just can’t see in front of you. A lot of people have died because of fog and zero visibility. It is sad when people perish like that because it’s messed up it’s still an issue.

Of course it was an accident. Look at videos of the crash scene and look at the thick, dense fog in the area. No way a person flying a 40 yr old helicopter could see anything.
