  • SARS-CoV-2 Variants of Concern in the United States-Challenges and Opportunities
    JAMA, 17th February, 2021
    jamanetwork.co...
    Rochelle P. Walensky, MD, MPH1; Henry T. Walke, MD, MPH1; Anthony S. Fauci, MD2
January 10, 2020, the first genomic sequence isolated in Wuhan
As of February 3, 2021, 468,000 sequences uploaded to public domain
3 specific viral lineages reflecting VOCs, B.1.1.7, B.1.351, and P.1.
    B.1.1.7 lineage
September 2020, first detected, UK, December 2020
    In at least 80 countries
In at least 33 US states
B.1.1.7, potential, reverse the present downward trend in new infections
Highlighted the risk of domestic and international travel
February 3, 2021, B.1.1.7 variant in the US is now approaching 1%, with prevalence in some states exceeding 2%
    B.1.351 lineage
October 2020, South Africa, December 2020
    In at least 41 countries
    South Carolina and Maryland
Demonstrated reduced response to previous ‘wild type’ antibodies
Lymphocyte response may well be the same
    P.1 lineage
    December 2020, travellers from Brazil
    Minnesota
    Zambia
    B.1.351 variant
Early December to 22 of 23 samples (96%) sequenced
December 2020 to early January 2021
    16-fold increase in COVID-19 incidence
The possibility of a similar experience in the US is a real threat
    First
Level of community transmission must be aggressively decreased
    consistent use of face masks,
    physical distancing,
restrictions on high-risk and high-capacity settings,
    frequent hand washing,
    delaying travel,
widespread diagnostic testing and screening to swiftly identify and isolate infectious individuals,
particularly those who are asymptomatic, and quarantine contacts
    Second
    Increased genome sequence surveillance
    Third
CDC’s 750 samples per week, to more than 6000
    Fourth
Accelerate SARS-CoV-2 vaccination nationally and globally
CDC is leading a comprehensive suite of studies to assess the actual effectiveness of these vaccines
    Looking out for breakthrough infections
Safety and Efficacy of the BNT162b2 mRNA Covid-19 Vaccine
    www.nejm.org/d...
    NEJM, (December 31st)
A two-dose regimen of BNT162b2 conferred 94.8% protection against Covid-19 in persons 16 years of age or older
    After 1 dose, efficacy = 52.4%
    NEJM, (17th February)
But if you take data after 2 weeks ….
    www.nejm.org/d...
One Pfizer dose, vaccine efficacy of 92.6% (based on FDA data)
One Moderna dose, vaccine efficacy of 92.1% (based on FDA data)
Uncertainty about duration of protection with a single dose
Administration of a second dose within 1 month, little added benefit in the short term
Leaving high risk groups completely unprotected
Postponement of the second dose is a matter of national security that,
if ignored, will certainly result in thousands of Covid-19-related hospitalizations and deaths this winter in the United States
hospitalizations and deaths that would have been prevented with a first dose of vaccine.

Why are countries still allowing international travel? No better than at the beginning when China allowed citizens to travel to other countries. Frustrating and disappointing. How about isolation and genomic testing when foreign travelers or citizens arrive in our country?

In Ontario, most places are opening up slowly, save for Toronto and peel region. The Public Health Units (and a couple more) have requested to stay on “ stay at home” orders for two extra weeks. Variant cases in Toronto are rising while the overall cases are going down. For the non informed person, staying closed is going to be hard to understand.

Our news outlets have been asking Dr. Fauci about this over and over and he says that the first dose is like a primer and there isn't enough data to show that the first shot will last long enough for protection to wait the 3 months for the second dose so he doesn't think we should do it, but are reviewing the data coming from other countries...its a bit frustrating because we could be getting more shots in and there seems to be A LOT of data coming in from other countries. It so confusing and frustrating because he is a very cautious man. Again im sure there is more to it and Im no scientist, but I feel like we are missing something here in the states compared to the other countries that are just doing the 1 shots. The problem with this analysis is that these variants aren't traveling through a virgin population. The US has recorded almost 30 million covid cases. Many experts estimate the true number of infections to be as high as 5 times the official number so possibly 150 million have some level of immunity. Add to that, 64 million Americans have received at least one vaccination. There is some overlap to those numbers but the number of vulnerable Americans is a fraction of what it was a year ago.
Finally, despite the scary headlines about vaccine shortages, most of my friends and family have gotten a shot or are scheduled for a shot. I've gotten both (Pfizer). With estimates of the J&J production, the US will be awash in vaccines.

It is my understanding that one ready is COVID testing in the US is privatized. Every time I get tested, which is five times now, I set an appt at a private lab, and go give what we’ve affectionately been calling a nasal lobotomy, or the blood test and wait for the results. Some lab has to collect my snot/brain tissue and blood samples that are left over and transport them to a lab that does genomic sequencing. The lab doing the sequencing would either be doing so for research purposes, or would be paid to do so for public health. My guess is they pool those samples before they sequence. We will get to more genomic info on these variants when some group funds research for universities to take this up, or the government contracts private labs.

Air travel must stop, or a 2 week mandatory isolation in specially suited hotels.
I believe some countries including Canada are doing that now.
They should've done that in March 2020. If I was in charge, I would've restricted travel in and out of the country, with a proper isolation program in place.
We could've nib this in the bud at the start and irradiate the virus, like we did with Polio.
The sad part is; this will happen again and again every decade.

If international passenger traffic had been stopped in January, or even February, 2020, there would be no pandemic, only an outbreak in China. Workers on cargo transport could have stayed aboard at ports, then quarantined upon return home. A 2012 study found that there was no link between low vitamin D levels and high cholesterol . . . in the short-term. But then, Cholesterol and athersclerotic plaque build-up is a slow-burn thing, I'm told. Best regime is exercise in the short-term, to keep cholesterol, in the words of Eric Morecambe, "Well-down", and an extra touch of vit D in winter, when exercise isn't always so accessible.
Further, its got to be remembered that a decline in the bodily production of vitamin D in winter has been with the human race as long as its been on the planet. As the rotational oscillations of the planet process during each year, the hemispheres alternately tilt towards the sun in summer and tilt away in winter, changing both the length of the sunlit day and the intensity of the sun's radiation with consequent effects on Vitamin D production - and that's if your out in the open, all the time. As the human race has developed its spent progressively more time under cover, in-doors, so the contribution of sun radiation produced Vit D has declined.
I presume that the difference/deficiency in Vitamin D production throughout the year that's exclusively attributable to dwelling in-doors is the amount compensated for in the HMG recommended daily dose of Vit D supplementation i.e. 10ug (400iu).

The problem with Americas rollout is how it is being done. If it was a first come first serve vaccination schedule heard immunity would be reached quickly. Trying to control the roll out to health care providers, and those most at risk is actually slowing the roll out in most places. The amount of bureaucracy and logistical complexity is a real problem. So it really becomes a matter of control not herd immunity. Do you focus on preventing death in those already most at risk and closest to doing from anything and everything and slow the machine down to do it or do you push ahead full speed and get the nation to herd immunity ASAP? I am trying to wait for 2nd generation vaccines myself since I have already had SARS-CoV-2.This roll out is insanely slow it is painful to watch. This is why we have 99% of vaccination handled by peoples GP's and commercial Pharmacies aka Chemists because it is faster! The government has never been good at rolling out these sorts of programs with any sort of speed! The more they try to control who has access to it the slower it get's it is almost painful to watch!

Feeling quite tragic as I lay ill and wait for the vaccine that is set aside for my mother, whom declines it. She's my best friend in the world. My son is in denial, he thinks the Covid is a hoax. I'm in medical school... I understand the science. My saving grace; I have died before, it's quite nice. I'll keep calm, & crack on.
    Half a million dead here---
    devastating.
Stay Well Sir John Campbell 🙏

Thank you for the information. I certainly hope the teams in the USA are paying attention to what is happening around the world. Unfortunately, they have been most successful at confusing the general public rather than inform. I hope they give someone other than Dr. Fauci the job of explaining why there is reason to be considered because he's not well respected anymore. So far I'm weathering the storm as South Dakota is seeing the worst of it in the "big" cities, where I am not.

If the vaccine contains COVID-19 any later testing of vaccinated residents would/could show positive. Possibly this could explain the outbreak?

    • @deborahhebblethwaite1865
      @deborahhebblethwaite1865 3 года назад +2

@@rongablue impossible the vaccine only contains the genetic markers for the S protein that attaches to the ace 2 receptor. No vaccines so far contain the whole genetic sequence of the virus. That is why the experts get concerned about these variants. They are affecting that area of the virus giving better attaching ability. I am less concerned about people truly sickened by the virus and recovering like myself. While yes i could again be infected i have been exposed to the whole virus....this latter point is my own idea not the experts. I have been studying viruses with Vincent Rancaniello from TWIV, youtube and it is free

In the US the official advice is to get it when it becomes available to you, even if you’ve had Covid. Realistically you have immunity but if you get a vaccine research shows that you _will_ get an immunity boost after the first shot but not much more from the second. Also younger cohorts are at relatively low risk and the vaccine supply is expanding, so if you need vaccination to travel for your own sense of safety or it’s required where you want to go it would be, IMO, very reasonable and proper to accept vaccinations when it becomes available to you.
