Left and Right AIC Pattern Confusion: When You're Not Sure What Leg You Are Standing On.

  • One of the more difficult concepts to understand is the difference between the Left AIC and Right AIC patterns. Many people think they are a Right AIC pattern because they see their pelvis is oriented to the left and/or they feel their weight is on the left leg. In reality however, they are simply in "fake" left stance.
    Many people, including me, learned to use our left leg really well, but in complete compensation.
    Since our pelvis never truly moved into the correct position for left stance, we mastered standing on our left leg in compensatory extension patterns, overusing our hip flexors, quadriceps, and back extensors for compensatory stability and movement.
    We never used our hamstrings, adductors, and abductors appropriately. We just never realized it. And why would we? You don't learn patterned biomechanics in school.

    This guy is saving my life... Sorry to be rude. But 99% of doctors and therapists are totally unaware and ignorant it buggles my mind. I have red and been told you cant heal scoliosis and I should learn to live with that.
    I intuitively knew it was not a muscle-scheletal problem, no strenghtening or stretching would'nt help, my instinct knew better than all those incompetent doctors. Finally after 5 years of pain and depression I came across this guy's channel. Thank you ❤️

      ...when one finally gets an X-ray of the knee booked...with difficulty stumbles there, and gets told to stand with the knee pointing straight, not 45 degrees outward. "Ah yes, that's the problem for which I'm here..." "Just try to point it forward for the picture." "Oh, the scan was taken when I didn't put any weight on the knee." "We can only do it once." Scroll down to "normal" knee X-ray and the customary beep to get a flu jab. Obsolete health care invested in obsolete practices as stubbornly set in their own pattern of rigid stilt gait.

      I assure you you are completely normal, and the Postural Restoration Institute exists because of what you are experiencing. No one understood what was going on with me either, and then I found PRI, and they put my entire life into perspective.

    I was SO confused about this exact thing when I first started watching your videos. I could feel that my pelvis was oriented to the right (my left hip was way forward compared to my right), but at the same time I favored my left leg (my right leg hardly bore any weight) and my left shoulder was lower than my right, making me wonder if I was somehow a right AIC. But the adduction drop test immediately confirmed that you're right. I was already mostly convinced I was a left AIC based on what you've said about the relative size of the left and right diaphragms and their pull on the spine, but it was nice to have confirmation.

      Same here man. Still a little confused but getting there. Left quad is big and tend to lean on it. So i guess i am left AIC with some more compensations i believe

      The drop test failed for me, I was able to drop my knee on both sides. How are you treating your overactive left side? I can't get my left side to relax and my right side to activate for the life of me. It's distracting me constantly!

    I have a tilt to my left, and my left hip is higher and remains there whenever I do any of the illustrations in your videos. After watching and understanding your whole concept way of thinking towards lateral pelvic tilt I want to strongly say thank you. This has been a reoccurring issue for years now, and I'm mad I didn't find you sooner. Although I'm just discovering this information now, I am excited to know I'm at least discovering the right information. I have stretched in years prior, but never held any form of credibility in them for long term stability and relief solely. I'm gonna get started now, and PRI seems like a great place to begin.     This is very helpful for me to conceptualize why someone with more weight on the left leg is actually still patterned as a L AIC above in the pelvis. Bodies are fascinating. They can find their way to the most efficient and pain-free way of standing and moving as they negotiate all of the injuries and patterns they've found themselves in. Even, as you said, with a pelvis tilted forward but rotated in a direction we wouldn't expect.

    This is very helpful. I am guessing this is why my left leg is always hyperextended/knee locked, and my right leg is toe pointing outward/knee quivering all day long.

    Thanks Neal for the amazing video! I am new at P.R.I. I have 4 months that i have completed the Myokin (homestudy) and that was one of my biggest questions. My pelvic had left orientetion so i was very comfused.

    Love these videos that go over the basics!
    (Btw, would love to see videos on what “good posture” is while sitting, standing, and maybe even laying down. I always doubt how straight my legs should be when standing, or what ribs should look like when standing as a PRI neutral person. PRI seems to really contradict classic “good posture” like “chest up, shoulders back” and the like- which sounds like extension!)

    This is great! Semantics! It’s the verbiage and criteria used by PRI vs our feelings and what we see our pelvis doing. An animation that shows the difference between Rotation of pelvis vs The Pelvis Being FORWARD.

    When I’m standing my right leg is slightly forward than my left leg, I could clearly feel my pelvis rotated to the left side when I’m standing up. I know u said even though it feels like that left AIC pattern is the underlying issue of it all. So my question is; Is it possible to have a just left oriented pelvis without really having a left AIC or is it only happens as a compensation due to the AIC????

    Welp, according to the side lying test I’m in the right AIC pattern. My right knee definitely feels stuck when lined up with my hip. Oddly my right shoulder sits lower than my left.

    Something I used to notice before I realized my pattern and discovered PRI is that while laying in bed I would have the urge to raise my right arm above my head and stay in that position. It felt almost like relieving an itch! I now believe that is because my body was trying to get air into the right chest wall. Figured you’d find this interesting and wondering if u had any comment....thanks

      Oh, definitely! It probably relieves tension in the upper right chest, which is constantly being smothered by the right arm being pulled down by the overactive right lat. Great observation.

    So what is weight on left side, right psoas tight AF, right glute sleepy, tight right intercostals and obliques tight and sore, left leg will abduct, right won’t, but right leg has very poor internal rotation.?What do you call that? Because I’ve subscribed and watched hours of your content and can’t figure out how to address my right tight psoas and QL, and weak right glute with forward tilt of pelvis on right side. I’m sure left AIC pattern is the norm but what about those that have something different?

    Im getting WAY Too much caught upin all tgis hoopla! I got a total hip replacement in my Right Hip at age 38 in 2013.
    I git to get ready for work.
    When i get an extea $185 I'll see you.
    Until then. Im going to forget everything you said & fn live my damn life & im going to dance when abd how i want & walk how i want. And I'll continue to place more weight on my lrft leg sonce it HURTS to place more weight on my right leg.

    What should I do If I don't have a PRI center in my area? are you not taking on any new clients? I need help asap, I keep throwing out my back during my MMA workouts every few months and it sucks. I am stuck in left AIC, it seems that both my legs (femur) are externally rotated, my right foot is supinated and my left foot is slightly supinated as well but not to much. My left TFL is getting sore all the time, and I have a shorter right leg it seems from the rotated pelvis when i walk I feel the difference, and I stuck on my right side. I have been doing your exercises and have all the tools but I think I am having trouble letting go of my right side, I need help.

      actually, I found a PRI clinic in Seattle Alpine Physical Therapy. It's 140 a visit, but it's certified and highly reviewed I am so glad I stumbled on this content now so I can finally fix this issue, and it will set me up for a future of good health..

    Hey Neal. Have you looked in to the Carrick institute? I have been recovering from a neurological lyme disease with assistance from their vestibular rehab programs, and have seen great results. All of their neuro rehabilitation exercises are extremely similar to what you have created in your programs, and could potentially be a source of continued learning in regards to the neurological piece (perticularly the ocular piece.) I was missing the dental piece, and didn't have a descriptive word for the right torsion) I think that will be a huge help to get through my healing plateau. Thank you for all of the wonderful information.

    I am naturally always on my left leg, like you are in the thumbnail. The right side of my body is atrophied and weak compared to my left side, which does all the work I don’t consciously force the right side to work. Does that mean I am right AIC pattern?

    Hi Neal, can u please make a video about posterior pelvic tilt, I dont understand some things, can i be left aic and posterior tilt at the same time. Pec pattern is anterior pelvic tilt, so do i have strong or weak left hamstrings. I dont see any video on your channel about posterior pelvic tilt but a lot of them about anterior so could u please cover that i would be very grateful. Even now i am grateful cause now i am slowly realising what is going on with me, but i am still confused. Thanks for helping people

    Ok so I’m trying my best to understand this… i mean i def know what your talking about I’ve lived with it my whole life and i cant believe how many times I’ve tried to explain (not correctly) but still i try to tell so many chiropractors that i think my hips go this way but then with my muscle development in my upper body that it must go this way abut then why does the pain in my neck on this side ? Lol and they just look at me like I’m a crazy person. So i just stoop trying to explain cause there not even picking up on what i mean…..so THANK YOU for all this knowledge i felt relieved just when i came across a video of your first time and I didn’t understand at first what exactly you were describing but I could just see your movement of the body and i was like this guy definitely know what’s going on with me… lol so in regards to your video here… I’m wont argue with you if your saying I’m a left AIC but….. my only question is that when i abduct my legs on each side (figure 4?) lol um i can do it with little discomfort but when the right leg goes over the left (right ankle on top of left knee) it usually needs the help of my arms and is very uncomfortable and can not be sustained. What yea think is going on? I’m pretty sure I’m a unique case. I’m short with a long torso. A chiropractor once told me that i had 2 extra vertebrae at them bottom of my spine . I also broke my coxics? When i was 20… mahalo for all you help greatly appreciated

      The inability of the right leg to go into abduction/external rotation (which is what I think you are describing) is a completely normal limitation due to the left AIC pattern positioned pelvis. The position of the pelvis prevents the right leg from ERing

    Sir, exact my problem, I have to lock my right knee to turn left and also realize my right glute is not engaged but my right quadricep ( front thigh ). So what's my problem sir and how can fix it? I always have shin splints on right leg and developing bunion on right big toe as well.

    this is complicated. all I know is that my right leg seems shorter, my right glute is weak, and I have difficulties activating it, my right shoulder seems to be a lit higher than the left one. any help people?

    This still doesn't match the symptoms I have. As well as being turned to the left, I have an extreme sense of the right hip being constantly in flexion, where the left is always in extension. When I look at myself in the mirror I have significant anterior tilt on the right side and nothing unusual at all on the left. My right pelvis constantly juts out forward on the right side when I walk and I'm constantly having to try to keep it back.
    I only just realised how much my left side is locked into the opposite extension pattern too. I'd been generally trying to do a lot of extension exercises, but I think my left side was already locked into that. By concentrating on hip flexion on the left side (regardless of which side I'm performing movements on), it's actively helping the right pelvis tip back and starting to balance things, so it can bear weight properly.

    The only thing I'd add is that by engaging much more active hip flexion on the left side, it can at first put even more sense of the weight being down on that side, whether sitting or standing. However, the reason this then allows me to connect properly though the right leg, is specifically because it helps relieve the far more extreme anterior tilt of the right side. It's literally by pulling my left side more forwards that it becomes possible to connect to my right side. The opposite action is what I'd already been locked into and have to stop myself from doing.

    After multiple major injuries on my right particularly right pelvic floor issue which chronically causes a muscle spasm limiting my right internal rotation I swearrr I am a right I am on my left leg, my pelvis is rotated to left, my right pelvis gets stuck anteriorly tilted yet I can’t get right foot to ground and my upper back is rotated back to compensate which limits my right neck rotation, Honestly I can’t work it out. Maybe PRI isn’t the core issue for some of us eg women who have had trauma to pelvic floor altering relationship of pelvic floor and diaphragm and or right hip internal rotation issues?

    Right AIC here, adduction drop test left side is greater then the right. Pelvis is orientated to the Left, front of right hip bone is more forward then left. I'm screwed since the average PRI therapist doesn't believe this is possible.

      You are doing this by yourself? Or with a therapist. The right neck/subclavius or inferior right glute max can result in this situation.

      ​@@NealHallinan i tried with PRI therapist but no good, i try myself now. You are dead on with these generalizations, my right shoulder is slightly higher then left and head is leaning to right slightly, The right leg is in varus abducted femur (but similtaneously twisted IR/ER femur/tibia), I try many exercise until i land on doing a split stance sink squat(left foot forward/right foot back ever so slightly ( half a foot length)) weighted more into right side back foot heel so the hips shift out laterally to the right, I use ball squeeze for adduction activation without the inward translation of right knee ( causes knee pain with squeeze but no ball). It take me a very long time to come to this specific exercise execution that i come away feeling better. Have you ever seen helped someone with similar situation successfully? It might take me equally a long time to figure out how to progress this.

      @@mikekeen1767 A higher right shoulder may indicate a subclavius restriction or “something” that is pulling the neck down on the right, moreso than it is actually pulling the right shoulder up. Either way, the neck is overactive, but why the neck is overactive is the science of PRI: figuring out why this individual’s brain is over-relying on the neck for “something”. If anyone were to be a right AIC, it would be me because so much of my testing used to come out contrary to expectations, yet I could still adduct my right leg easier than my left, even though I felt my left stance muscles more than my right. Ultimately, once I worked through my visual and oral cavity compensations, it wasn’t until I cut my insoles into 3/4 lengths, so my left toes were more in contact with the harder inside of the shoe, that I started to feel my right stance muscles as easily as the left. It’s been a long, strange trip full of experimentation. Without seeing you I can’t really provide any real recommendations beyond something abnormal is going on: could be sensory, could be structural.

      ​@@NealHallinan its crazy how so many little things can impact our posture, I think it is maybe perhaps I grew up playing baseball pitching right handed as a youth, many thousands of pitches where i stepped down the mount planted my left foot and pivoted all my body weight from that to throw the ball. The only real noticeable structural changes would be the extra bone growth into the left side Illium outwards & pincer bone growth around the left femur. As well as the twists in the knees where the Left knee is ER / IR femur/Tibia, and the Right knee is IR / ER femur Tibia. So due to the above factors my left leg is limited in abduction, and my right leg is limited into adduction. The act of pitching is pushing off the rubber with the right leg, and then landing recieving with the left leg repeatedly maybe it is possible to create a right AIC through pure brute force of the high forces of pitching that overrides our natural tendency? I guess the only way to reverse such a thing would be to do the opposite? maybe i can subsitute a med ball throw for a baseball landing on right leg and pivoting with rightward trunk rotation. Although maybe it is a special case where its is because in youth we are still growing and things are more malleable and now it is not so maybe i'm screwed now.

      @@mikekeen1767 I was a pitcher also, but had to stop pitching at 13 because of severe shoulder pain. The left AIC pattern simply describes an asymmetric diaphragm and brain activity that creates natural right dominance in humans. What you may have done is created pathology through your lower right back (iliolumbar ligaments) that created lumbar instability and a pelvis that begins to “orient” towards the left (even though the left ilium is still forward compared to the right). I was the same way and I became a master of using my left leg…completely in compensation. Lower spine instability will undoubtedly lead to overuse of your right QL, subclavius and neck.

    When I lie on my back, right side of my pelvis is slightly higher, as if pelvis rotated toward left. Is that rare? I take it that it is compatible with the common AIC pattern.

    Can you please help? I have a structurally longer right leg and an x-ray that clearly shows my lumbar spine rotated to the left, resulting in my S curve scoliosis. I have always naturally put more weight on my shorter left leg and my right leg is far weaker. Therefore, do I truly have the rarer right AIC pattern? This is key to understanding how I can begin to address my pelvic rotation! I looove your videos, I just need to know if I'm right AIC or not! Thanks 🙏

    Hi, really looking for some understanding in my case. I am a professional ice skater doing shows all around the world. I have had to pospone one contract after the other because of my problem. Everything started with an injury of my right foot last summer (multiple stuck bones and inflammed ligaments. Causing the tendon running from the big toe to the calf to inflame while having to work way harder on stabilizing my foot).
    As a result of the pain, i have started limping for 4 months without going away from doing shows for financial reasons.
    After the end of my contract, in november, i have taken some time resting in order to finally heal this pain. It did, but the limping is getting worse day after day while i don’t have pain on my foot anymore. My left hip flexor started to hurt and i really felt like i couldn’t pit proper load on my right leg. When i walk, i notice my right hip moving forward along with my right leg , although i never seem to be able to have a good heel strike followed with a nice roll to the toes. As well, My right leg doesn’t want to extend through the knee for the propulsion phase. In the meantime, my left hip never gets to move forward during the entire gait cycle.
    A few days ago, i read one of your articles that talks about the left AIC pattern with the right BC and right TMCC.
    I have started to feel some big neck/ trapezius muscle pain on the left side along with the feel that my shoulder cannot rest down anymore.
    My PT only told me I lacked internal rotation on my right hip.
    I do know i have a big hip asymmetry but still have trouble figuring if it is a right or left AIC. In your article, you associated the right side of the neck being painful and overactive along with a left AIC. It did throw me off a little while i feel pain on the left side of my neck, along with my left hip stuck behind me.
    I feel completely insane and like i cannot walk nor stand still anymore. Please tell me what you think.

    I've got eustachian tube dysfunction in my right ear, the right side of my throat feels stuck when I swallow hard, my right scapula always feel tight, my right hip flexor feels tight and my right ACL snapped but I had surgery to fix it. What's going on with me and how do I fix it? Please!

    I have far less right ankle dorsi flexion (due to break above ankle if fibula and tibia). My belly button naturally orients to the left and I feel I stand on my left leg easier. Thanks for your thoughts on this and help

    OK. I understand. I have Left AIC despite my pelvis is oriented to the left. The question : How can i perform 90-90 hip lift ? Which hamstring should i activate ? (LEft - increase the rotation of pelvis or RIGHT ?)

    Speaking of the adduction test, you have an older video showing it. Is there any way to emulate it at home by yourself?

      You can try but it's every easy to "cheat". If the left hip is "patho", meaning it's "loose" because ligaments have loosened, you may think the leg adducted when in reality it's just so disconnected from the hip that it moves way more easily than a non-patho leg.

    Hi Neal,
    I have been really digging into all your info about the AIC pattern and there's one thing that keeps confusing me. For a lack of knowing all terminology I hope this makes sense but it's the following: In your videos it looks like one needs to have the right ASIS in front of the left ASIS (pelvis oriented to the left) when the left leg is forward (initial contact). For example in this video at 5:42, you demonstrate that you need to push yourself to the left with your right glute. And in that position you end up with the left leg in front of you and the left side of the pelvis behind the right side (left ASIS behind right ASIS, pelvis oriented to the left). This really looks very twisted and unnatural to have the pelvis oriented toward the side of the front leg. When you look for pelvic rotation in gait, in every document you always see that the pelvis orients to the side of the back leg. (e.g. at initial contact of the left foot, the pelvis orients to the right, which is opposite of what you demonstrate here).
    I clearly must misunderstand something, so I was wondering if there is a video that demonstrates the full correct motion of the pelvis during gait. Not just stepping in places, but really moving forward?

    Hi neal , I think that I have left aic pattern but not sure means my left side of the whole body are imbalanced means all the muscles in upper body and lower body are weak as compared to the right. My right side muscles in the upper as well as lower are extremely tight what that means ?

    My right short adductor is nearly non existent and is pretty much detached from my pubic bone according to an MRI. My right hip is higher than my left hip too but no leg length discrepancy. What does this all mean?

    But what if when My left hemi pelvic is higher and my right one is lower and more forward but my whole pelvic is still oriented to the right?
    Ps: a year ago I lost sensation through a lot of areas on my right side.. peripheral and central due to nothing, I was suffering from depression. Moderate depression.

    I've just found your channel and I'm really interested in PRI right now. I'm suffering from kinda pain you describe for over 2 years. It started from left piriformis but I didn't think it can be so complex... Then I had extraction of two teeth. And then right suboccipital and midback pain started. I've been to manual therapist, PT and nothing... I'm also a student of PT and active sportsman so it's really annoying to have this pain all the time and can't get rid of it with techniques I know...
    I've started doing 90/90 and breathing exercises this week.
    There's my question - can pain get worse after exercises? And is it possible to workout and do those exercises at the same time? Can all my problems result from extraction of teeth? And what if I cannot feel my left ribcage during all 4 breathing?

    I get what you’re saying Neal, but if I do all left focus PRI techniques (where most of my body weight already is) I end up with injury to left knee and left foot becomes supe tense trying to not end up in excessive supination (biggest toe and smallest toe faaaar apart...). So it’s like a mistery puzzle: what to focus on if you start with left oriented pelvis and already burdened left side. Doing only left sided activities seems off. Maybe right hamstring with adductor would be as important?

      You can try a 90-90 for your right hamstring. I don’t think hip shifting to the right is a good idea for most people though, unless they really know why they are doing it and guided by a professional.

      I'm trying to figure out what you mean by holding the left hip back in order to correctly shift into left AFIR. Should it be accompanied by a feeling that that hip bone should "revolve" in a circular fashion to the left, away from the left side of the body i.e. laterally. And simultaneously the right hip should feel like its 'revolving' inwards or medially?
      I ask, as the left leg is naturally straight in the above description and the tendency might be to overly straighten the left leg too much since the left aductors are weak on that, which would obviously be bad for the left knee.

      @@ruebenandrews9914 the left hip is pulled forward and externally rotated hence the lack of ability to activate left side hamstring and adductors. Adductor Pull back exercises will reorient your left side pelvis to posterior tilt allowing anterior fibers of glute medius to contract, adductors and hamstring and obliques to engage. If you can’t pull back on either side you can’t have proper gait mechanics of heal strike then push off big toe.

    In PRI do you ever consciously try to use your muscles correctly? Like, should I ever think about the right glute pushing action or my hip position when I walk or should it just be automatic after achieving the correct position with the techniques?

      That's a rather lengthy discussion. If you realize that everyone is a left AIC right BC pattern, you kind of know what you are going to get. The difficulty is figuring out which people are "patho" and/or cranially driven. That's why my questionnaire heavily screens dental and vision.

    In the Left AIC pattern in another video he mentions your right hip would be elevated then. When i go to the chiropractor my left hip is always elevated but its also the hip that is turned inwards. Is this still a left AIC pattern?

      @@NealHallinan Sorry, just to be clear my jaw is slightly shifted left, the muscle under my left rib cage is always tight, my left hip is always tight and seems to be pushed inward because of the inner leg muscle tightness. Its also elevated according to all my chiropractors. My right side i just have a shoulder issue where it seems to be pulled in. This is the Left AIC pattern where i work on my left pelvis area? because some videos i see they want to correct the elevated right hip but that's not what i have.

      @@milkweed07 It’s still the same pattern underneath, but if a jaw is shifted, your your right neck is undoubtedly playing a role. Your best bet is to find a PRI therapist to help you.

    I’m one of the people that began my pain with left SI joint issues. Fast forward 10 years and I began having left sided- shoulder/neck/upper back/rib pain. I ended up having TOS surgery to remove my first rib and release my pec minor. My pain on the left continued. I was recently assessed by a PRI therapist, and now have learned that I’m left AIC and PEC. I am one that favors standing on my left leg. I am keen on data analytics and correlations. I would be very curious to see how many people that are left AIC and favor putting their weight on their left leg also have the left sided shoulder/neck pain versus the right.

    Does Hypermobility affect PRI in any shape or form?
    My body has increased tension after sports which is enforcing my Left Aic, Right BC patterns activating my parasymathetic nervous system causing me stress.
    Could this be because I am hypermobile, and get more unstable during physical activity?

I have both and I'm trying to gather some solid points to talk to my doc about as he had acknowledged my hypermobility but has yet to diagnose left aic right bc pattern.. so far I've gotten told I have slight curve of lower spine and muscular imbalance. I would like for him to fully understand and recognise this in the video is my issue and knowing how hypermobilty slots in would really help. Thanks
