  • Sensory awareness of the right big toe is necessary for appropriate biomechanical pushoff (right foot re-supination) to transfer the body from the right foot to the left foot. In other words it is needed to inhibit the left AIC right BC pattern as defined by the Postural Restoration Institute.
    Hey there, my name is Neal Hallinan.

    Hey all you poor bastards dealing with this thing!
    Going on 4 years dealing with this, I’ve seen PTs, Chiropractors, Surgeons, Massage Therapists, you name it.
    Nobody has been able to tell me what is going on with my body.
    I found out this week that it’s most likely my teeth. Insane.

      Have you since found relief? (And if so, what did you do that helped you the most?)

      @@sarahcraigs5879 i found out that I grind my teeth really bad at night.
      So the dentist has given me a night guard that also pulls my jaw forward a little bit.
      That’s also contributing to my sleep apnea.
      I also am now working with a speech therapist to strengthen my mouth and tongue.
      I will be working with the speech therapist for a couple months to prepare my mouth for palette expansion.
      My teeth placement is forcing my jaw back, and my forward neck posture is a result of this over a long period of time.
      I’m starting to feel changes down my entire gait already just with the mouthpiece and speech therapy.

      Ive been dealing with a bunch of weird symptoms, not being able to walk straight is one of them, along with breathing difficulties, difficulty swallowing, etc. I traces it back to losing my jaw on the right side of the lower jaw. Tragic. It was a perfectly good tooth that got removed for no good reason.
      Its also changing the shape of my face, as now I have a cross bite and my face is no longer symmetrical.
      And nobody even does wisdom tooth implants…

    I have a palpable and visual indentation on my right rib - it's literally sunk in - along with the inability to feel my right big toe. When you shared the images of your clients laying on their sides to check for the patterns, I did not have to lie on my side to test them on myself. I had already noticed those same patterns on myself a while back. When I then applied a paper towel to my right big toe like you did in the video, within 5-10 mins of standing my right calf was on fire 🔥! So much so that the muscle went into a minor spasm! It was neat feeling. Because of your videos I've managed to search and find a PRI practitioner near me and I'd just like to say thanks. Thank you 🙏

      I'm happy that the video was illuminating. I can't say that feeling the calf on fire is necessarily a good sign, but it does go to show how our muscles respond to sensory input.

    You just described all the things I do the flared rib, the lifting my right toe , the entire posture pattern. I have a longer left leg illusion.
    It is just twisted pelvis making the left leg more forward and the right back.
    This makes my right hip tight.

    Gently standing and orienting my gaze left or right definitely shifts center of gravity. Walking with eyes orienting on my lessor aware side is creating very surprising vertigo. This is fascinating!

    Much of this resonates with me. The right big toe, the imbalance etc.. the on difference is my tightness is in my left side and my weight is on the left mostly.

    If I could exume all my old shoes/workboots, every outside, right heel would show significantly more wear or would be completely gone. Essentially, they have all been worn to a wedge shape. I just bought a pair of well supporting sneakers and am on my way to better posture with he help of your videos. Thanks!

    Your patient had functional hallux limitis. Because of the functional limitation of the 1st MPJ, on weightbearing, instead of getting dorsiflexion of the 1st MPJ with plantar flexion of the first metatarsal, you get dorsiflexion of the first ray at the first metatarsal cuneiform joint which includes adduction of the first metatarsal which is what causes the bunion. What you used is called a Cluffy wedge which is helpful. More helpful than that is an orthosis that allows the first metatarsal to drop. As far as the cause, we are dealing with a failure of the tensegrity structure of the body to keep the bones in the right place due to faulty neurologic input. This could be anywhere in the forefoot. The most common is the periosteum of the 4th metatarsal as a result of the collapse of the transverse metatarsal arch as a result of a strain-counterstrain or reverse strain-counterstrain of the adductor hallucis. I agree that it occurs more commonly on the right and if it is on the left then you have to look for dysponesis. As far as why on the right, I don't have an explanation and yours seems very plausible. I do enjoy your videos. They add another dimension to how things work.

    I’m loving your videos..I’m guessing this would be the same with the eyes ..I’m right handed but my left eye has better eyesight than the right..20% better…my shoulder falls forward lying on left side and backwards slightly on the right..what a conundrum..thank you..🙏🏻

    coincidentally I was forcing my righter foot whenever I was sitting towards the ground to tense it up and feel more of my righter foot muscles, and you upload this as I try this method to have some relief (which actually worked for today). Mostly when I find out new stuff like this it works for a week and then sadly the miracle relief no longer occurs.

      Well in the case of this video, adding paper towel to the right big toe is a minor component of an overall Postural Restoration program. By itself it won't do much.

      Right foot pronation by itself? I don't think so. Pronation of the right foot doesn't have much meaning to the brain if it's not coupled with left foot supination (heel strike) and a shift of the pelvis to the left at the same time...which is what 80% of all PRI techniques are working on.

      @@asgeraskersen4361 If you can pass the adduction drop test and can feel your left hamstring on a PRI hamstring technique, then you are sensing your left heel.

      @@NealHallinan i can feel the heel only if i pull down. I doubt i feel it during the walk, my neck hurts. But when the right arch touches it goes away, a neck pain i try to escape for 3 and a half years. Its not going all the way ofc, if im not careful im back into the pattern and the neck pops back immediately. Its much better tho, your videos gave me a lot of relief, i was in horrible pain, i am so right dominant now, you can see i have a lot more muscle on the right than on the left, a sad situation:)

    Great video Neal ! One question though, should you get any type of feet bunions removed prior to attempting to master pri techniques ? Specifically bunions on the outside of my big toes ?

      I wouldn’t do that. The only way you know whether the bunion is a problem or not is if you have an experienced therapist evaluating you.

    Hi Neal, jus went to see a behavioural optometrist for the first time today, quite fascinating as I'm -4.75 (right eye)
    -2.75(left eye) however I'm favouring my right eye(worse) and not using my left. I've been prescribed a -4 in my right eye along with exercises to combat my binocular issues. Does this sound right to you? Thanks for all your help and I'm gonna smash out all the techniques and do exactly what I'm told and write back to you in a month. Thanks for all your help Neal.     Hey Neal! Since I incorporated your techniques into my Physical therapy while recovery from a major knee surgery I’ve had less asymmetry and making better progress. My question is, when I did finally feel like I got my right side to decompress, the amazing sensation of breathing fully again was almost immediately interrupted by a sharp sensation in top of hip/lower abdomen. It was painful but more of a good pain. Is this normal? What could be the cause or problem area?

      There is no PRI program for everybody. It’s all about each individual. If you start searching for “PRI exercises/techniques” you’ll find them on YT and IG. But it’s impossible to know what you should do without being evaluated first.

      This will not work for everyone. Anything I do with PRI depends on the individual. Everyone has a different brain and body. Neck tension has many different potential origins.

    I have lost vibration and therefore proprioception in both feet over past 4 years. Idiopathic. I still feel pain and touch. Left AIC/Right BC has recently become symptomatic - think they are related? Golf scores have gone from mid-70’s to mid-90’s over this period. Right glute, right SI, right hip (recently replaced), right obliques all painful.
    Any hope for me? I’ve started the exercises 10 days ago.

      I don’t think they are beneficial for resolving the left AIC pattern. If you get your pelvis neutral and stable first, then they are fine.

    This is why modern day shoes and socks are ruining feet and mind and body connection. It is key to wear zero drop shoes with minimal sole thickness so the brain can sense what the feet are feeling while walking, running etc… there is so much sensory feedback happening below your feet. Imagine wearing boxing gloves on your hands your entire life and trying to pick up a spoon, fork, drive a car or do pull ups etc… you hands would be weak and unstable. This is the entire purpose behind wearing minimalist shoes with foot appropriate toe box. Many brands are out there like Splay Shoes (Vans equivalent), Be Lenka (casual) and outdoor/workout shoes are Vivobarefoot. If you wear shoes that are minimalist with a foot shaped toe box a lot of your issues will go away and your feet will develop strength and flexibility and mobility in the process. Also your toes will eventually spread out naturally over time so no need for crazy shaving surgeries. You can wear socks like Injinji and toe spacers in your minimalist shoe and you will notice a difference like being more grounded, better posture and body mechanics. All of that just from going away from shoes that baby your feet and create havoc to the rest of your body and posture in the process. Of course just changing your footwear will not resolve all your imbalances but it will be a huge step in the right direction.

      The right side of the ribcage should move in front of the left. The “winged” right shoulder is often already forward. The shoulder doesn’t have to be changed much, just the ribcage that it is sitting upon .

      @@NealHallinan ahh now it makes sense why i always had that tenancy to look for the right big toe and shoulder to be a bit in front .. PS : got new glasses 3 weeks ago : im down to 0.50 on my right eye and 0 in my left, with the the astigmatism correction on and the axis changed from 0 to 90 degrees. still hurts but its a good pain :D

    Can this cause clawing of the middle to outer toes on right foot too. I dont have a bunion but i feel im not pushing off on big toe like you said ...alsoi have all the other symptoms. Im wondering is it all caused by my poor vision/lazy left eye or is it all caused by the clawing of the toes and my tendancy to sit leaning to the right. Either way im going to show this video to my physio in my first appt next wwek because you are able to explain propetly whereas when i spk to docs i just seem to be coming out with a multitude of different issues which dont seem related to them. Thanks for all you do to let us know were not going mad and imagining these issues

      If vision is a problem, then yes, it can lead to the improper foot mechanics. Inappropriate vision "ungrounds" you, and you'll often see toes doing weird things due to this ungrounding effect. Visual sense, or lack of visual sensory processing, will overrule the feet every time.

    My problem was this toe ALL ALONG. I looked back for this video after I discovered it myself. I still don't know whether to have joy or cry. The brokenness has been so long. And then what am I going to say to anyone, "it was my big toe." I need this video from Neil just to have credibility. It's an unspeakable topic for me at the moment. Just going to put hot sauce with a side of Wasabi on my right toe with an extra sensory order of onion rings around it. wtf

      Well, if you can't push through your right big toe effectively, you can't transition to your left side without compensation. So the right big toe is a really big deal. That's why someone may need orthotics. To allow for that transition.

    I had suspected my right big toe was the problem. When I try to curl my toes, it's almost as if they dont bend.
    I would put a hard massage ball against a wall, keep my right foot flat, put my right big toe resting on top of the ball pushing down while trying to lift my other 4 toes (hold for 15 seconds). I would get instant neck relief and could feel my right arch when walking for a short time after. I could never explain why and it wouldn't last long.
    I tried your method in the video but slightly modified it so the support is closer to the metatarsal (not high up on the toe) and I've been walking around sensing the ground all day. It' as if I needed a slight artificial fulcrum so my body would activate my right toe.
    The reallt weird thing now, is when I'm sitting (while wearing the idea and not moving) I can feel my right toe vibrating (similar to weight lifters trying to lift really heavy weights and they start shaking). It feels like ther nerves a firing to stabilize something that hasn't been used in years. I have no bunions at all btw, I just cant curl my big toes which I'd assume has something to do with this.

    This is one reason I can't understand why PRI encourages tapered toe box shoes, one of the main causes of bunion formation (which btw is not a "bone growth" so much as misalignment of the 1st metatarsal).

      @@jmitch0062 those are only "wide" relative to other shoes on the market... not the actual shape of a natural human foot. They're still tapered at the toes.

      I would say it's likely you have a very unstable left hip, but beyond that I can't speculate. Sometimes someone may overuse their right eye for upright postural stability when they have postural instability. But there is no way to know unless you have a very experienced PRI therapist testing you.

      You need to stabilize your pelvis at the same time. I'd recommend finding a PRI therapist to help. It's too hard to do on your own.

    The bunion is in the metatarsophalangeal joint and not the tarsometatarsal one as suggested. Difficult to believe that you know what you are talking about if you lack this kind of basic anatomy knowledge.

    Is there any point in doing PRI exercises if your PRI therapist hasn’t checked whether you need a splint or certain prism glasses? I’m waiting for custom orthotics to be made then I will see him again but in the mean time he gave me a few exercises to do which I feel are pointless and a waste of time without the other two sensory inputs?

      Most people don’t need glasses or a splint. Is there a reason you think you need them? At any rate, orthotics, splints, and glasses aren’t treatment, they just allow the body to gain neutrality, so you still need to do the techniques.

      @@NealHallinan Well after watching a lot of your videos I came to the conclusion that PRI techniques only work if your body is sitting neutral, so I would be wasting my time because when I finish doing the PRI exercises my body goes straight back into the old pattern? The PRI therapist did this one muscle test where he pushed down on my left arm and I failed to hold the resistance until he he gave me these glasses to wear then I could hold the resistance no idea what it would indicate? thanks for replying

    PRI is enlightening, but over the past 5 years, I’ve learned that it is not the fix. It does a great job explaining the asymmetrical patterns of the body. But the way PRI claims to fix it is not the way to fix it. You can sense your left heel until the cows come home, you can reach and breathe and sense the right arch. These things will not fix the real reason for these symptoms, which is space inside your body to move. The reason for everything is that the left side houses more organs and the left side cannot and never will be able to compress as much as the right side. And this has little to do with weight bearing and everything to do with unbalanced respirator muscles. Vision, teeth, feet are not the solution, they’re symptoms. Does that mean PRI doesn’t help? No. What PRI does is teach you to realize these things, which can be a huge relief once you really understand the cause of the symptoms. It can also teach you pseudo ways of getting “on your left side” by creating more compensatory range of motion in certain areas, like the lumbar spine, pelvis, and left scapula. By sticking with PRI, you will learn to center your mass more over your left foot via the aforementioned compensatory ranges of motion. But you will never be able to compress the left ribcage as much as the right, and this you will never decompress the right as much as the left. I think PRI is a wonderful resource, but it can be misleading. If you think you are going to balance your asymmetries and be alternating truly from left to right and all of your problems will subside, it’s just not realistic.

      Well articulated. I feel the same. At first I was a huge believer in PRI but after time I came to the same realisation which you put across so well. PRI is just an overly complicated scientific explanation of what’s happening with you’re body. I think the best type of rehabilitation with incorporate both PRI but mainly getting efficient at natural human movement. Which in most of the ‘developed’ world is robbed from us by sedentary lifestyles.

      Do you have insight into a true fix?
      Would you say insiratory training like the o2 trainer be a fix? I use it and it seems to teach me to properly breath which has had a cascade of positive effects for me.

      @@HUGEACT-MAN I’m sorry if it wasn’t clear in my post, but what I meant was that it isn’t something to be “fixed” because it isn’t something that is “wrong”. It’s the reality of human design. I believe the best remedy is for the individual. We each have to find ways to compensate for our asymmetry. For some, it’s breathing exercises and PRI. For some, it may be yoga or meditation to relax the body. There are two things I know for certain: 1) there is no one size fits all since we all have different lifestyles and demands of our bodies and minds. 2) there is no permanent remedy, meaning, whatever it is that works for you must be some sort of routine. You will have to continuously work at it, and you may have to adapt and find new things over time.
      So perhaps not the answer you were looking for, but the real answer is attention to your mind and body, being adaptive, and all things in moderation that you can. I hope this helps!

      Definitely not. You should she see the founder of PRI take a cheap plastic mouthguard and use an electric grinder to grind it down to fit someone’s mouth so they can feel their teeth differently. People have to feel things differently, otherwise no change will occur. It is fascinating.

      @@NealHallinan If I am putting weight on outside of left foot (region 1) (supinated) and have my right foot (region 3) (pronated) feeling big toe am I doing it right? Should I focus on this for single leg squats or something?
