Shouldn't the first 4-bit ADC at 22:41 output LSB instead of MSB? Say I have analog input of 8, that's 0000.1111 for 8-bit and 1111 for a 4-bit. That "1111" in the 4-bit is the LSB of the 8-bit representation.

I thought pipeline ADC illustrated are now using 2 4bit ADC, so there are 2*2^4 comparators vs 2^8.

no. The second ADC is a refine of a single quanta (delta) of every digit of the first ADC
