Impression:
At L4-L5 level: A 4-4.5mm central and bilateral paracentral disc protrusion is noted asymmetric to the left. Moderate left lateral recess stenosis is noted with abutment on left traversing L5 nerve root.

Diffuse L-3,L-4 and L-4,L-5 disk bulges causing indentation on thecal sac associated with mild narrowing of B/L neural foramina.

Means the disc component is close to nerve roots
Effacement is thinning or narrowing of canal
