Another good point is when you are diuressing and not achieving adequate goal is ensure have good blood pressures or target a certain SBP to ensure adequate perfusion to get diuretic effects. Many times have seen that pts with heart failure may even require some ionotrope support to help with diuresis. The other is if they have low albumin can give albumin and chase with lasix or even a bolus of hypertonic saline.

To add there are times if they are on any blood pressure meds to it’s a good idea to hold to achieve that high Bp for perfusion and to help achieve goal net neg fluid balance!

Hi ninja nerds , i think the patient at first should be presented as CKD 5 stage since she got GFR=12 , and in 8:47 its 4 to 2 to 0.1 not 1 , and lastly i think we should keep an eye on Ca++ if i started with loop diuretics alone without thiazide .

I love you guys! I’m a 2nd-year med student and have a few questions. How do I know the patient’s normal weight before the illness to determine when to stop diuresis? Also, if I diurese too much, are there cases were more potassium or sodium to balance their levels, or just stick to change the diuretic to fix it?
