The moment I heard ‘depressurisation’ my mind instantly thought of Helios 522, a 737 whose pressurisation switch was set to ‘Manual’ and was depressurised mid-flight before slamming into a mountain outside of Athens, Greece.

And it was all because the pilots failed to realise the switch was in the wrong position, even after multiple routine checks before takeoff. I also thought of Helios 522 as well.

Cause of helios522: during maintenance when a crew reported seeing ice near the rear door, the maintenance worker set the pressurization to Manuel. After the maintenance, the staff didn't set it back to auto. When 522 climbed, takeodd config sounds but the crew had seconds to react but the crew didn't react in time. After 15-20 mins, everyone on the plane in incapacitated and the plane circled until it ran out of fuel and crashed. No one survived.

It's rare that case happenning in real life but it can be very scary if you didin't set the cabin pressure properly or maybe a mechanical failure regarding Oxygen. Before Helios 522 accident, in 1999, a Learjet 35 with 2 pilots, Payne Stewart and his 3 friends crashed in South Dakota when the plane run out of fuel hours after departing Orlando and sadly the cause of lose of cabin pressure remains unknown

helios-522: the plane had some problems with the door l2, and while repairing the plane, ground crew left pressurization on manual mode instead of automatic. The crew failed to manage situation because they didnt even recognise the allarm.
